GREATER MANCHESTER MENTAL HEALTH NHS FOUNDATION TRUST Job Description for NHS Locum Full Time Consultant in Learning Disability Psychiatry 10 programmed Activities for Community Post in Salford This is a full time appointment for a consultant to work with the Learning Difficulty and Mental Health services in Salford to deliver a service to people with a dual diagnosis of learning disabilities and mental ill health. The post is well established and the cover is for maternity leave from November 2017 until September 2018. INTRODUCTION Greater Manchester Mental Health NHS Foundation Trust GMMH came into being on 1st January 2017 as a new large organisation after the acquisition of Manchester Mental Health and Social Care Trust (MMHSCT) by the Greater Manchester West Mental Health NHS Foundation Trust (GMW). The new trust GMMH now provides a wide range of local services to Bolton, Salford, Trafford and the City of Manchester with a population of 1.176 million (Manchester 483,700; Bolton 262,400; Salford 218,000; Trafford 211,800). Specialist services are provided from the Prestwich site. There are also a number of smaller community bases. The Chief Executive of the Trust is Ms Beverley Humphrey. The Chair is Mr Rupert Nichols. Greater Manchester West is managed as three district directorates covering Bolton, Salford and Trafford as well as specialist service directorates. The Medical Director is Dr Chris Daly and the Associate Medical Director and Director of Medical Education is Dr Margaret Campbell. 1
City of Salford The City of Salford has a population of approximately 234.000. Overall the area is characterised by high deprivation but includes some affluent areas (overall Mini in Salford is 112.2). In Salford 85% of residents were born in England and 92% speak English. Other languages include Polish 1.6%, and Arabic 0.5%. Salford is adjacent to Manchester the commercial centre of the North West. Salford itself has undergone major development in recent years including the innovative Salford Docks area with Media City and the Lowry Project. The motorway network allows easy access to major cities and towns in the North West regions, Manchester International Airport and areas of natural beauty including the Peak District, Yorkshire Dales, Lancashire Moors and the Lake District. Salford Learning Disability Service The Consultant will be based at Meadowbrook, Department of Psychological Medicine, but most work is in the community. There is a very close working relationship with the community learning disability service. In terms of numbers of people with learning disabilities within Salford, there are about 350 children and 800 adults who are on the dedicated registers. National data suggests that about 0.6% of the population have a severe/profound learning disability and about 3.5% have a mild/moderate learning disability. Salford has an integrated Learning Disability Service with a functional community team. The community team is made up of Nurses and Social workers who work alongside Psychiatry and other Allied Health Professionals including Psychology, Art Therapists, Speech and Language Therapists, Occupational Therapists and others. The teams work in close partnership with the commissioning process of LD services. The model of support operated in Salford looks to change the environment in which people live and reduce their frustration and any challenging behaviour. We have two priorities: that people remain living in Salford and that any Salford resident is supported to return to Salford. This requires building the capacity of Local Services and skilling-up staff throughout the service with a strong clinical lead from Psychiatry and Psychology. Adults with learning disability and mental illness are admitted to Meadowbrook (General Adult Psychiatry Unit) if necessary, and this is exceptional owing to the strong community support model. Clinics are held at local Mental Health and Primary Care Centres, plus there are domiciliary visits to patient s homes, hostels and nursing homes There are 8 Salford patients in secure hospitals that will require repatriation. These patients are discussed at the Allied Health meetings 2
Salford Directorate Services Inpatient service The inpatient service is based at Meadowbrook; this is in the grounds of Salford Royal Hospital. There are currently 4 inpatient wards. Eagleton Ward is a male recovery ward with 23 beds There is a Psychiatric Intensive Care Unit called Chaucer ward which has 8 beds. Consultants retain responsibility for their own patients on this ward There is a female recovery ward called Keats Ward with 22 beds There is a male 15 bedded recovery ward for Manchester district inpatients - Mc Coll ward All four acute wards provide treatments and interventions that are evidence based, individualised and needs lead. Each service user s care is based on care plans that are developed collaboratively between the patient, their Multi-Disciplinary Team, primary nurse and families. There are 11 core trainees who work in Meadowbrook with both inpatient and community responsibilities. In addition to this there are Seven Senior Trainees on the rotation. Community Service There are three community Mental Health Teams which are now aligned with the GP clusters. The Community Consultant at Prescott House is Dr P Taylor who works with clusters 6, 7 and 8. He is supported by an Associate Specialist. At Cromwell House the Consultants are Prof P Haddad who covers clusters 2 and 3 and Dr D Hughes who will cover cluster 5 patients. There is a Speciality Doctor who is attached to both Consultant posts at Cromwell House. Ramsgate House covers clusters 1 and 4. Dr M Campbell covers cluster 1. There is a Speciality Doctor who works with Dr Campbell. The Consultant for cluster 4 is Dr M Birtwhistle. Old Age Service The Older adult Service has 4 Consultant Psychiatrists, these are Dr Mc Mahon is the consultant for Salford inpatients 3
Drs Malik and Kareem cover the CMHT s for older adults Dr Javeed is the lead for the memory service and the care homes practices Home Based treatment Team There will be 4 part-time Consultants who will work within the Home Based Treatment Team. These Consultants are also aligned to the three CMHT and inpatient Consultants. Dr Chinedu Umeadi is the Home Based Treatment Consultant for the Prescott House area. There is a vacant post for Ramsgate House area and Dr Haitham Nadeem provides input to this population. Dr Saba Nazir is the acting Consultant for Cromwell House Other Community Services There is an Early Intervention Service based at the Broadwalk in Salford. The Consultant in this team is Dr I Ellory There is a Mental Health Liaison Service and the Consultants for this service are Dr R Overshott, Dr N Perera and Dr S Shah who work with a team of practitioners for patients above the age of 18. This service operates entirely within the general hospital setting (Salford Royal Hospital). There is additional funding for a further consultant post There is a Rehabilitation Service. Dr D Hughes works on a rehabilitation ward in Meadowbrook named Copeland Ward Dr N Singh and Dr S Farrell are both Rehabilitation Consultants working in another service called Braeburn House RESPONSIBILITIES OF THE POST HOLDER: Consultant outpatient clinics at the Cromwell House Community Mental Health Centre and Pendleton Gateway Admission to adult psychiatric wards at Meadowbrook Mental Health Unit for people who need inpatient assessment and treatment for mental illness. The post holder will be the Responsible clinician for these patients. The ward staff co-ordinate the inpatient care and liaise with the community care coordinators to ensure they attend the weekly review Planned joint working between mental health and learning disability services in Salford 4
Jointly supporting a proportion of patients via the Care Programme Approach (CPA) with Case Managers from either mental health or learning disability services. The post holder will not be the care co-ordinator for any of these patients Referral of suitable patients, who have mild learning disability and mental illness, to the Community Mental Health Teams (CMHTs) and psychology services. Admission, as required, of adults with a learning disability and forensic needs to Specialist Secure Hospitals The key objectives of the service will be to: Provide assessment, treatment and supervision of patients with learning disabilities over the age of 18 Maintain continuing clinical responsibility for all of the patients under his/her care Provide a service to the adult population of Salford. This will be achieved through liaison with, and the provision of advice to, General Practitioners as well as close working relationships with the Mental Health Team, Salford Royal Hospital and the Joint Learning Disability Service (also known as New Directions Joint Learning Difficulty Service). Provide an outpatient clinic in the Community Mental Health Centre and in a GP Practice in Salford. On average there are 2 new patients seen a week and when there is a substantive consultant the waiting list is less than 4 weeks. If the waiting list escalates or there is a sudden increase in referrals the MDT are able to take the lead with new assessments. Outpatient appointments are also attended by care co-ordinators and support workers. There are an average of 14 follow up appointments a week. Undertake home/community visits for patients unable to attend the outpatient clinics The post holder will have a personal secretary and a designated office space within Meadowbrook unit. They will have access to their own computer. The post holder will be expected to be Section 12 approved and to become involved in Mental Health Act work on the ward and community Mental Health Act work for their own outpatients. 5

General Requirements and Conditions of Service The post holder must be registered with the GMC and be on (or eligible for inclusion on) the specialist register. It would be desirable for candidates to have a higher qualification in psychiatry (MRCpsych or equivalent) and specialist training and experience at ST4-6 level (or equivalent) of a minimum of 3 years. The post holder will have 32 days a year annual leave pro rata. Cover for leave will be arranged between consultants in the district. The Trust complies with national terms and conditions for annual, sick, special and study leave. Medical practitioners are advised to be members of a Defence Society. The candidate s appointment will be subject to medical clearance at the Trust Occupational Health Service. The candidate s appointment will be subject to a satisfactory DBS check. 7
Salary Scale This will be based initially on 10 programmed activities under the new consultant s contract and will be reviewed via the job planning process. Starting salary will depend on seniority.
