Purpose. To provide guidance for specific entries, board package requirements, ranking board schedule, and timeline for administrative completion of Lieutenant Commander (LCDR) Periodic Fitness Reports (FITREPs). 2. Applicability. This notice applies to personnel assigned to Commandant of Midshipmen (DANT), Professional Development Division (PROD EV), and Leadership Education and Development Division (LEAD). 3. Responsibilities a. Commandant of Midshipmen Administrative Officer (AO). Provide ranking spreadsheet to the Deputy Commandant of Midshipmen (DEPDANT) that includes a list of all LCD Rs assigned to the DANT. Spreadsheet shall consist of FITREP data, ranking input, and other pertinent information. b. Deputy Commandant of Midshipman (1) Monitor the progress of the ranking board. (2) Provide board results to the DANT for approval. c. Sub-Cost Center Heads (SCCH). Provide FITREP inputs to the AO by the due date. See paragraph 7 for timeline. 4. Specific Entries on FITREP. a. Date Blocks. All dates are required to be in YYMMMDD format (i.e. 170CT31). b. Block 6 - UIC: 00161

c. Block 7 - Ship/Station: USNA ANNAPOLIS MD COMDTMIDNNOTE 1610 d. Block 8 - Promotion Status: Enter the member's promotion status as follows: (1) REGULAR: Regularly promoted to the grade entered in block 2. (2) FROCKED: Frocked to the grade or rate entered in block 2, but not yet regularly promoted. e. Blocks I 0-13 - Occasion for Report: Mark X in Periodic block. f. Block 14 - Period of Repmi - From: This date will be the day after the date in Block 15 of their last FITREP. g. Block 15 - Period of Repo1i - To: Enter l 70CT3 l. h. Block 16 - Not Observed Report: Mark X in Not Observed Report only if member has been onboard less than 90 days. I. Blocks 17-19 - Type of Repmi: Mark X in Block 17 - Regular Repo1t. j. Block 20 - Physical Readiness: Enter appropriate coding in accordance with reference (a). For most LCD Rs assigned to Commandant Staff, this will be "PP" indicating Cycle I and 2 of2017 PFA completed. k. Block 21 - Billet Subcategory: Enter NA. l. Block 27 - SSN: Enter 000-00-0000 r. Block 28 - Command Employment: EDUCATE AND TRAIN MIDSHIPMEN. To develop midshipmen morally, mentally, and physically and to imbue them with the highest ideals of duty, honor, and loyalty to graduate leaders who are dedicated to a career of naval service. s. Block 29 - Primary/Collateral/Watchstanding Duties/PF A: (1) In box enter Primary duty. 2

Block 32 - Signature of individual counseled: Ensure member signs when debriefing FITREP. w. Blocks 33-39 - Performance Traits: Enter an X in the appropriate box 1.0-5.0. Ensure compliance with reference (a). x. Block 40 - Recommendation for career milestone or billet: ( 1) Enter recommendation for next billet or assignment. If member is not recommended for future assignment, enter "NONE". Do not leave blank. (2) Example: WAR COLLEGE I J-PMP y. Block 41 - Comments on performance. Enter performance comments in this format: (1) Strong opening statement (two lines). (2) Tlu ee or four (depending on space) strong bullets in the following format. Example: - EXCEPTIONAL LEADER. "Text" - MISSION DRIVEN. "Text" - MANAGERIAL SKILLS. "Text" - ASTUTE SKILLS. "Text" (3) Strong closing statement with recommendation. Use this to fortify your opening statement (one to two lines). (4) Avoid using passive voice. Avoid giving credit to member's traits (eg. His/Her 3

exceptional talent created... ); instead stick to member, then action verb (eg. Through exceptional talent, he/she created... ). z. Block42 - INDIVIDUAL: Enter X in block you think the member should receive. Final determination will be made by the DANT. aa. Block43 - SUMMARY: Leave this block blank,commandant Admin will fill in. bb. Block44 - Reporting Senior Address: Enter as follows: COMMANDANT OF MIDSHIPMEN UNITED STATES NAVAL ACADEMY 101 BUCHANAN ROAD ANNAPOLIS MD21402-5107 5. Board Package Requirements. A blue folder per member delivered to Commandant Admin with routing sheet will contain the following support documents: a. Latest chop of FITREP. b. Copy oflastfitrep. c. Copy ofmidte1m Counseling (if applicable). d. Copy of PRIMS data not covered on last FITREP. 6. Ranking Board Schedule. The DEPDANTwill chair the LCDRranking board on Monday, 2 October 2017 and will provide results to the DANT. 7. Timeline for Administration Completion of FITREPs a. SCCHs will provide initial FITREP draft to AO no later than Monday, 2 October 2017. b. AO will review FITREPs and route to the DEPDANT for review and modifications no later than Friday, 6 October 2017. c. Final drafts of FITREPs will be delivered to DANT by Monday, 16 October 2017. d. DANT will review, modify, and sign FITREPs no later than Friday, 31 October 2017. e. SCCHs will debrief members and return FITREPs to the AO no later than Monday, 13 November 2017. f. Commandant Admin will review and verify FITREPs have been correctly signed in all respective blocks and will mail original FITREPs to Navy Personnel Command as per reference (a), no later than Wednesday, 15 November 2017. In addition, Commandant Admin will ensure members receive a copy of their FITREPs. 4
