NHS GREATER GLASGOW & CLYDE JOB DESCRIPTION JOB IDENTIFICATION Job Title: Responsible to (insert job title): Band 5 Occupational Therapist Lead Occupational Therapist Department(s): Directorate: Operating Division or GGHB: 1. JOB PURPOSE The post holder will manage a defined caseload and will carry out occupational therapy assessments, treatment and evaluation of this intervention to identify mental and physical barriers to an individual patient s level of function in daily life-skill activities. The occupational therapist will work with the patient on an individual or group work basis to enable and rehabilitate their level of function in these activities in order to maximise their level independence within their living and community environment. The occupational therapist will work as part of a multidisciplinary & integrated multi agency team and engage in team activities such as mental health assessment, team risk assessment, team audit and research.

3. ROLE OF DEPARTMENT Occupational Therapy staff in Mental Health work with multi-disciplinary & integrated multi agency teams to provide specialist services which enable patients to achieve health, well-being and a level of independence through participation in graded treatment programmes thus limiting impact of physical and mental health problems on ability to function independently. Occupational therapy assessments are utilised within the multi disciplinary team and external agencies (including 3 rd sector agencies) to identify the person s functional ability and provides a baseline on which levels of support which may be required/ identified to maintain a level of independence in a variety of living environments. To develop and maintain an evidence based OT service Occupational Therapists uses a range of OT specific and generic standardised assessments. This would include assessment for and prescription of adaptive equipment and assessment of risk. 4. SCOPE AND RANGE Occupational Therapy staff within the Mental Health system work with multi-disciplinary & multi agency teams to provide specialist services which enable patients to achieve health, well-being and a level of independence through participation in graded treatment programmes thus limiting impact of physical and mental health problems on ability to function independently. By engaging in the professional occupational therapy supervision process with a senior occupational therapist the post holder will participate in the delivery of an evidenced based occupational therapy service. The occupational therapist will be expected to participate in the clinical development framework within their area and professional training opportunities available, including the grade specific Mental Health Development programme for occupational therapists which includes a monthly training facility and access to a mentor. The Occupational Therapist will provide leadership for junior staff as delegated and supported by their superviser. 6. MAIN DUTIES/RESPONSIBILITIES Clinical To plan and implement client centred individual and/or group interventions, using specialist skills to assess, grade, modify, review and evaluate clinical intervention to deliver therapeutic goals & maximised functional independence and rehabilitation potential within their specialist area. To undertake occupational therapy assessments which will identify factors affecting the patient s functional performance. These factors may include physical as well as mental health barriers to function. To work in partnership with the patient to establish realistic and achievable occupational therapy goals and contribute to the overall multidisciplinary & multi agency treatment approach. To apply evidence based approach to clinical interventions including the use of standardised assessments and outcome measures allowing clinical evaluation of the occupational therapy process. To manage designated clinical caseload efficiently & effectively and apply principles of organisation e.g. to document all clinical interventions, organise appointments, liaise with other disciplines and inform the overall multidisciplinary approach. As part of the multi disciplinary team the post holder will contribute to and participate in the multidisciplinary assessments process, e.g. risk assessment, single shared assessment, mental health assessments and diagnosis of conditions. To demonstrate knowledge of mental illness and offending behaviour and its potential impact on the individual, their carers and their living and working environment. To undertake clinical risk assessment in order to inform the multi-disciplinary & integrated multi agency care planning process & assist in the creation of risk management plans.

Will work in a psychologically minded way with patients and utilise evidence based low intensity interventions; following relevant training, as deemed appropriate for the service. To apply acquired skills and knowledge of professional practice in order to develop competence and fitness to practice as an occupational therapist, facilitated through engagement in the professional clinical supervision process. To demonstrate ongoing professional development through participation in internal and external development opportunities e.g. mentoring programme, recording learning outcomes in a portfolio and statutory training. To be aware of risk issues and contribute to the risk assessment and risk management planning for individuals. To demonstrate understanding of national guidelines and legislation relating to health and social, care and their impact on service provision. To be aware of equality and diversity issues in order that the patients receive interventions appropriate to their cultural and religious needs. Educational and Research To participate in the induction, training and education of students and other staff in this setting To demonstrate the ability to critically evaluate current research and through discussion with professional line manager apply to practice. To participate in the delivery of the occupational therapy learning plan. To research evidence base for clinical interventions and assessment relevant to the local clinical practice. To participate in the development and audit of impact of relevant clinical guidelines ensuring clinical effectiveness to optimise patient care.

EQUIPMENT & MACHINERY To use relevant alarm systems within the service areas. To assess, prescribe and fit a range of therapeutic equipment which will maintain and/or restore the patient s level of function in a range of activities and environments. To utilise specialist knowledge of equipment, adaptations and assessment of clinical risk to assess, fit and educate patients and carers on correct use of equipment. To use a range of everyday equipment / machinery within therapeutic, patient s home and community environment relating to policies / guidelines and risk assessment.

7b. SYSTEMS To ensure that security check systems are maintained and utilised in accordance with departmental standards. To access the NHSGGC Equipu Store or other local authority systems to order equipment and adaptations for client care. To ensure that up to date written and electronic records and activity data are maintained in accordance with health & social care partnership, professional & local multidisciplinary standards and provide specialist occupational therapy reports to the practice setting. To be responsible for delegated administrative tasks, e.g. monitoring stock, advising on resources required to do the job. 8. DECISIONS AND JUDGEMENTS To independently use professional clinical reasoning skills with regard to patient care & rehabilitation including assessment of clinical risk and using appropriate networks to respond to clinical crisis situations, in adherence to professional codes of conducts. Following occupational therapy assessment the post holder will decide on tasks which can be delegated to technical/support staff and will monitor staff performance in relation to these specific clinical tasks. Occupational therapists will inform the wider departmental supervision process in relation to junior staff and students in relation to clinical performance. To demonstrate understanding of, and participate in the implementation of policy in the service area. The occupational therapist would be expected to seek out clinical supervision as required to facilitate and support their clinical reasoning & decisions and judgements as required. 9. COMMUNICATIONS AND RELATIONSHIPS To establish communication networks with patients, carers and other health care workers and agencies. Occupational therapists routinely require to communicate details of the potential impact of symptoms / diagnosis on ability to carry out everyday tasks to users and carers, taking into consideration the sensitive nature of information relating to mental health problems and using communication skills to establish, develop and maintain therapeutic relationships.

To gain consent from the patient for the sharing of information across agencies and their participation in OT intervention using negotiation skills. To participate and work as a member of the multidisciplinary & multi agency team, contributing to decisions with regard to patients care programmes and communicating outcomes of occupational therapy assessment and treatment plans. This will include attendance at clinical and development meetings, reviews, case conferences, CPA etc. To promote awareness of the role of occupational therapy within the multidisciplinary team, patients and carers and informal and external visitors who engage within the multidisciplinary team. Occupational therapists will routinely communicate with patients who experience barriers to their communication as a result of presenting mental health problems, Occupational therapists may require to communicate with patient s and carers who may not use English as a first language due to cultural diversity. To actively engage in the development of professional relationships within the wider occupational therapy structure in order to ensure effective networking and maximise use of support and supervision frameworks to aid all aspects of professional development. 10. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical Demands Supporting clients when walking frequent short periods. Manual Handling of clients frequent short periods. Transporting / using equipment e.g. bathing equipment frequent short periods. Supporting therapeutic movements frequent short periods. Working in confined spaces in clients homes infrequent long periods. Manual handling of clients on a regular basis. This includes assisting clients of varying levels of dependence with transfers in a variety of environments, e.g. confined, cluttered and awkward areas.

Significant element of walking, climbing stairs, standing and working within confined and awkward spaces on a daily basis. Frequent transportation of patients is required either in car, hospital or public transport following risk assessment. Regular transportation of equipment from office, to car, to client home. This requires moderate physical effort and dexterity. Occasional manipulation and installation of equipment such as, toilet frames, chair raisers. This requires dexterity and moderate physical effort, often manoeuvring within confined spaces. Assisting clients when walking on a regular basis. May adopt static postures for lengthy periods within some treatment processes. Spend frequent short periods daily using IT equipment requires awareness of own postures and positioning of equipment/seating etc. Dexterity and co-ordination skills demonstrate range of treatment media.
