TIP Project Information Form Project Description, Federal Funding by Source and Amount Must Agree With That Indicated on the PPI and LPA. The Federal Fiscal Year in the TIP For the Project Phase Must Agree With the PPI and the LPA Agreement.

SUPPLEMENTALS Use Appropriate BLR Form 5610 or 5611 Description of Work Must Justify the Request for Additional Funds CECS only for Supplemental Work Requires Revised PPI, Amendment to the LPA Agreement & TIP Change In Most Cases Supplemental Work May not Proceed Until the Amendment to the LPA Agreement is Executed by IDOT
