Completing the Application Form The type of dentistry services you provide, will determine what sections of the application you need to complete, If you provide both NHS-funded and Private Dentistry services: Complete Sections 1-3 and sign the Declaration at Section 10. You will only need to complete Sections 4-8 if you are NOT currently included in any Local Health Board or Primary Care Trust Dental Performers List. Section 9 must be completed. If you provide only Private dentistry services: Complete all Sections and sign the Declaration at Section 10.
Section 1 Personal Information This section must be completed by ALL applicants. Section 2 Your Work This section must be completed if you provide NHS funded & Private treatment or Private only treatment. The information provided will form the basis for your certificate/s. The address/es supplied needs to be an address at which you can be contacted by patients (which may be your home address). Section 3 Specialist Services This section must be completed if you provide NHS funded & Private treatment or Private only treatment. The information provided in this section will assist with information gathering for your inspections by the Dental Reference Service and for monitoring of compliance with Regulations 14, 15, and 16 of the Private Dentistry (Wales) Regulations 2008. Section 4 Professional/Technical Qualifications treatment and dentists who are NOT on a dental performers list. The information is required to meet Regulation 5 (2)(g) of the Private Dentistry (Wales) Regulations 2008. Section 5 Professional Experience treatment and dentists who are NOT on a dental performers list. The information is required to meet Regulation 5 (2) (g) of the Private Dentistry (Wales) Regulations 2008. Section 6 Clinical References treatment and dentists who are NOT on a dental performers list. Please supply details of 2 referees both of whom are able to comment on your competence to provide dental services (give the name of you current or most recent employer as the first). Neither must be a relative and they must be able to comment on a recent period of employment or post of at least 3 months. Section 7 Entitlement to Work in the United Kingdom treatment and dentists who are NOT on a dental performers list. The information is required to meet Regulation 5 (2)(h) and 5 (2)(i) of The Private Dentistry (Wales) Regulations 2008.
Section 8 Rehabilitation of Offenders treatment and dentists who are NOT on a dental performers list. All convictions must be declared even if they relate to offences many years previously. Failure to disclose convictions which would show up in any event on return of the Disclosure & Barring Service (DBS) disclosure could result in refusal or cancellation of registration. This information is required to meet Regulation 6 of The Private Dentistry (Wales) Regulations 2008. Section 9 Validating your Photograph If you provide both NHS funded and Private dental treatment or Private only dental treatment you are required to provide a recent photograph of yourself as part of this application for Registration. The photograph MUST be validated by a professional person, or someone of similar standing, who has known you for at least two years. The person validating your photograph: must not be related to you by birth or marriage. must not be in a personal relationship with you. must not live at the same address. must be resident in the UK. This information is required to meet Regulation 5(1) c of The Private Dentistry (Wales) Regulations 2008. Section 10 Declaration To be completed by ALL applicants. Please ensure that the declaration is signed and dated prior to submitting, as unsigned applications will be returned and will cause a delay in your registration. All Dentists including those who do not provide private dental treatment are asked to sign the declaration. This will enable Healthcare Inspectorate Wales (HIW) to identify those dentists who do not require follow up action for registration purposes at the present time. In signing the declaration dentists who do not provide private dentistry must be aware that should they wish to undertake private dentistry at a future date, they will first require registration with HIW.
Disclosure & Barring Service (DBS) (formerly the Criminal Records Bureau (CRB)) Disclosure Application The DBS disclosure check is an integral part of the application process, the form is to be completed by you and returned to HIW, who will forward to the DBS on your behalf. Guidance on how to complete the DBS form can be found at www.homeoffice.gov.uk/dbs. When the DBS has processed your disclosure application, your disclosure certificate will be sent to HIW. A copy will also be sent to you directly by the DBS. Before HIW countersigns your DBS disclosure form, we must confirm your identity. The enclosed DBS guidance leaflet sets out the original documentation you must provide to us with your completed DBS form. Once we have confirmed your identity, all original documentation will be returned to you. We are unable to countersign your application without the appropriate documentation or if your application form has not been completed correctly. In these cases, your application will be returned, which will cause a delay in your registration. NB: If you are also applying to be placed on an NHS Performers list and are having a DBS check as part of that process, you will not need to complete a DBS form for this application. You need to state on the application form that this is the case.
Payment Registration is subject to payment of an annual fee of 75 and an enhanced disclosure with the DBS is currently 44. Your registration fee is payable annually. The cost of the disclosure is a one off payment of 44 (to be renewed every 3 years). You have two options in respect of payment: 1. Payment by cheque, made payable to the Welsh Government; or 2. Payment by Credit/Debit card by completing the payment form. We are unable to accept cash. How will the information provided be used? HIW will use the information you provide on this form to: Process your application for registration. Administer and maintain your registration under the Act and Regulations. Process complaints. Compile statistics about the profession.
Data Protection Act 1998 fair processing notice This notice tells you about how we process personal data under the Care Standards Act 2000 and any associated legislation. What is personal data? Personal data, as defined by the Data Protection Act 1998, is information that we have or are likely to have at some point relating to a person. Examples include the following: Information relating to a person s application for registration, or as a referee or an employee (staff) of any person applying for registration. During an inspection or complaint investigation, it can also include service users (including children), and, where appropriate, relatives or representatives of service users. How and why we use this personal data As part of the Welsh Government we use the personal data to: Help us carry out our legal duties and regulate the standards of care under the Children Act 1989, Care Standards Act 2000, other relevant legislation and appropriate national minimum standards of care. Prepare statistical analyses (from which people cannot be identified). Provide information to Welsh Government Ministers, which will help them make decisions relating to policy changes. Organisations which may share the personal data We may share the information we hold with other organisations when the law allows. For example this may be with: The NHS Shared Services Partnership. Various regulatory organisations such as ombudsmen and inspection authorities, where the law says that information must be passed so that they can do their work. The Department of Health, Social Services and other health and welfare organisations where there is a need to share information to protect and support vulnerable people. The Disclosure & Barring Service and the Police.
Recorded data The information we hold may include: Information on the registration application form. A record that a criminal record check has been completed, the date the check took place and if the outcome of the check was successful. References on a person applying for registration. Inspection history. Event history (in other words, specific events which need to be noted). Complaints. Any information relevant to carrying out our legal duties. Due to the nature of our work, it involves going into great detail. We will try to make sure that we only record information that is necessary and it is accurate and kept secure. We will treat the details that you provide in line with the Data Protection Act 1998. We will only use your information: To process an application for registration with us. As part of the on-going inspection process of the registered service. Any other purpose which we may need to use information for in relation to performing all legal duties. Other than when the law allows, we will not release your details outside of the Welsh Government. Your rights under the Data Protection Act 1998 The Data Protection Act 1998 gives you certain rights about the personal data we hold on you. This is not a full list but examples include: The right for any personal data held about you to be processed fairly and lawfully. The right to ask for and receive copies of the personal data we hold about you, although we can sometimes withhold some information. The right, in some circumstances, to prevent us from processing personal data if doing so will cause damage or distress. The right to ask for wrong information to be put right. You also have the right to ask the Information Commissioner, who enforces and oversees the Data Protection Act 1998, to assess whether or not processing your personal data is likely to keep to the conditions of the Act.
