There are numerous benefits to participating in the program increased energy and improved sleep habits, for example that can help you be more engaged and more productive at home and at work. I urge you to make physical activity, healthy eating, and work-life balance priorities through this program. 3 Keeping your good health in mind, we encourage you to get a biometric screening and complete the Aetna online health assessment. In 2014, your rewards for participating will be different than in past years, when we gave you a one-time cash payment. This year, if you complete your screening and assessment by Oct. 31, 2013, you will see a monthly health plan savings. UVa remains dedicated to your health. All employees and spouses enrolled in the UVa Health Plan are eligible to participate in Hoo s Well. We want to help you stay healthy. That s why we ve invested in comprehensive wellness programming that helps you know more about your current health status and how to make improvements. Hoo s Well provides strategies to support better health and well-being through every stage of life. Hoo s Eligible? All Academic and Medical Center employees (spouses too!) who are enrolled in the UVa Health Plan can participate. Retirees, Cobra enrollees, and their spouses enrolled in the UVa Health Plan may participate in a number of programs including the biometric screening, Aetna wellness programs, and Quit For Life Tobacco Cessation Program, but are not eligible for rewards. Get Rewarded As an employee, you can earn rewards by taking these first two steps to improve your health: Step one Have a biometric screening AND Step two Complete an Aetna online health assessment New this year: If you participate in a biometric screening and an Aetna online health assessment this fall, you will see $40 per month in health plan savings. Look for a mailing with new health plan option descriptions in a few weeks. Important: Complete both items by Oct. 31, 2013. The CDC recommends that healthy adults get 150 minutes of moderateintensity aerobic activity every week along with at least two days of muscle strengthening activity. You all are doing a great job with so many things! Several months ago, I d asked for quit smoking information to give an employee who asked about it you replied immediately, I passed along the info and when I saw the employee the other week, he said he had been cigarette free for five weeks. You are making a GREAT difference in our employees quality of life!
Hoo s Well Steps Step one Have a biometric screening to know your numbers. I would never have come into the gym without the Hoo s Well program. Screening Options When How OPTION 1 Biometric Screening UVA-WorkMed will conduct screenings at various locations on Grounds. OPTION 2 Physician Screening Form You may elect to use lab results from Jan. 1 to Oct. 31, 2013 instead of attending a biometric screening. OPTION 3 NEW My Chart My Chart is the UVA Health System s medical record system you use to manage information about your health. For access go to https://mychart.healthsystem. virginia.edu/mychart/ or call 434.243.2500. New this year, you may elect to use lab results in My Chart from this calendar year instead of attending a biometric screening. Attach the results from My Chart to your completed Physician s Screening Form. (See Option 2). No physician s signature needed. Have your screening between Aug. 19 and Oct. 11, 2013. Sign & submit the completed Physician Screening Form by Oct. 31, 2013. Forms must be received at UVA- WorkMed by Oct. 31, 2013 in order to qualify for monthly health plan savings. Your Health Information is Private All information obtained from or provided by you as a result of your participation in the Hoo s Well program is kept private and secure, and is not shared with the University of Virginia as your employer. Consider placing reminder notes on the refrigerator or images of healthy people around your house or office for inspiration.
Important: In order to earn My Rewards, you must complete a biometric screening and the Aetna online health assessment in the fall of 2013. I worked in private industry for 13 years prior to starting my career at UVa and did not experience this level of care for my well-being. 11 Faculty and Employee Assistance Program (FEAP) FEAP provides free and confidential counseling, problem solving, and referral services for employees and their families. Hoo s Well-related programming includes worklife seminars, stress management and consultation on depression, anxiety, family concerns and other issues. Hoo s Losin It is a two-prong approach for individuals who need to make lifestyle changes to correct obesity and blood sugar problems. Programs Include: Balance, weight management, nutrition education Nourish Relax, stress management Breathe, tobacco cessation Overcoming Depression Overcoming Insomnia Aetna Disease Management Program This program provides one-on-one support and advice from trained disease management nurses for over 30 health conditions, including diabetes, asthma, osteoporosis, and heart disease. The Disease Management Program does not compete with your doctor, but helps you better understand and follow your doctor s advice and treatment plan. Garths Well. It was enlightening to learn that proper nutrition is more appropriate than dieting. Working with a peer group facing the same dietary and physical challenges helped make the time commitment easier. Physician Screening Results You may elect to use lab results from your annual physical from the current calendar year instead of attending a biometric screening. Important: You and your physician need to complete this form in full and sign. Fax the completed form to UVA-WorkMed s virtual fax at 434.244.9434. Dial the full number. This is not a UVa internal fax. Only fully completed forms are accepted. Due by Oct. 31, 2013. As the participant, you are responsible for confirming receipt of this form. Labs sent without this form will not be accepted. Save your fax confirmation. If your physician is sending this form, we recommend obtaining a copy of the form and fax receipt for your records. NEW: You may use lab results from My Chart in lieu of a physician s signature. Go to www.hooswell.com for specific instructions. Employee Release: By submitting this form, I am requesting that my physician report my biometric screening results to UVA-WorkMed so they can be included as part of Hoo s Well, the UVa Health Plan sponsored employee wellness program.
