Privacy (PPI) Training This training is mandatory. In other words, anyone working in Home and Community Care must take the Privacy Training. This is a 2-3 hour course taken online. You only have to take the course once. To access the training, you need to: 1. open the internet browser 2. enter http://ppi.fnihis.org/ in the URL (address) bar 3. click on Click here to start 4. click on English 5. enter in Username 6. enter in Password 7. work through the module Once you have completed the training, print out the certificate, and fax to the home care nurse advisor for your area. Creating a New Report 1. Once the current month is successfully uploaded: Example for month of JANUARY: esdrt_jan 2010_your name_communityname.xls 2. Open your current existing file in your Excel program. Excel 2003: Click on File on the top left hand corner and then click on SAVE AS Excel 2007: Click on the Microsoft Office button on the top left hand corner and a drop down will show, then click on SAVE AS Excel 2010: AS Click on File on the top left hand corner and then click on SAVE (Ensure when you are saving in Excel 2007 or 2010 that you save the file with the extension.xls) 3. Change the file name to the next month. Example for month of FEBRUARY: esdrt_feb 2010_your name_communityname.xls 4. Click on Save 5. On the Client Information Page, you need to change the date of the report in the box on the top right hand corner. Example: from 2010/01 to 2010/02 Ensure when you change the date that you use the TAB key to leave the date box, your cursor should then show up on Line 9 in Column A, click on save again. If this is done correctly the date will be changed on all the pages. 6. Go to the Home Care Services Page The Date should automatically be changed if the correct method has been followed. 7. On the Home Care Services Page, delete all the home care services of the previous month. A quick method of doing this is to place your cursor on the first row of the (#9). The 9 th row should appear black or blue under the cursor arrow; hold the left side of the Mouse down and pull the cursor DOWN through the rows of data. All the rows will be contained in this action. Release the mouse left hand clicker and everything is should be highlighted. On the spreadsheet go to the button Delete Row(s) and press. A message box will pop up asking if you want to delete the rows; press YES. All rows will be deleted and the data gone. Press anywhere in the spreadsheet to un-highlight the area. 8. Click on Save The spreadsheet is now ready to enter in the next month s data. Example: esdrt_feb 2010_your name_communityname.xls

Uploading e-sdrt Reports 1. On your Desktop, Double click on your Internet Browser OR double click on e-sdrt upload icon (if you have it set up on your computer). Enter in Username and Password and region Alberta, click on Login 3. On the top of the page click on Administration to go to the Administration page 4. On the left hand side of the Administration page click on e-sdrt/hcc Excel Spreadsheet Upload 5. Click on the Browse button and a window will open that is says Choose File on the top. 6. Find the file (month) on your computer that you want to upload, click on it to highlight then press open. Your file will go onto the Browse box. 7. Click on Upload 8. If the report is not accepted, fix the errors on the file. These errors will be noted on the Upload page. 9. Follow Steps 5-7 until a successful upload has occurred. 10. Click on Log Out on the top right hand corner of the page when the uploading has been successful.

Accessing e-sdrt Reports 1. On your Desktop, Double click on the Internet browser icon OR double click on the e-sdrt upload icon (if you have placed this on your computer). Click on Login 5. On the top of the home page, click on Reports 6. On the left hand side of the page that opens, click on Home and Community Care Reports 7. Click on Service Delivery Reports Report time: Service Community: Fiscal Year: Period: Language: Community or TC/HA (community name) (desired fiscal year or present one) (desired period) (desired language) 8. Click on Get the List of Reports Choose the desired report and double click on the pdf, which will bring up the desired report (must have adobe reader on the computer). If you encounter issues with this, you may also right click on pdf. and save the report to your hard drive and open it from there.

How to Document Travel Travel time is not a service provided to the client so it is not captured on e-sdrt. Charting Charting time should be captured under the (E) code under (E) Case Management. This includes charting for home care staff including HCAs and support staff. To capture charting most accurately, keep a running total of the amount of charting you do for each client. At the end of the month provide a total amount of time and you may chart this under your last client. The reason for this is due to the limitation of the e-sdrt template to only capture a minimum of 15 minutes as 0.25 and nothing shorter. Phone Calls Phone conversations for appointment scheduling, intake information, etc. should be captured under Case Management Services using the most appropriate code. Example: Code B Initial Assessment (Care Planning); Code H Establishing Links/Liaison, etc. Case Management Services codes may also be used for time spent: Making referrals, consults, access to professional therapies, etc. Use Code H Establishing Linkages/Liaisons; Consulting with housing. Use Code H Establishing Linkages/Liaisons; Completing forms when the need for alternate sources of care has been identified as part of an initial or re-assessment process, e.g. LTC. Use Code C Review/Assessments Therapeutic Care Visits done for the purpose of reassuring the client or providing family support should be captured under Nursing Services-Code D- Therapeutic Care Ordering Equipment Time spent ordering client specific equipment may be included under the most appropriate Case Management Services code. Two staff going out for one visit This varies depending on the case: If 2 staff members are going out for a bath assist, 1 staff member should report on the bath. If 2 staff members are going to a home visit to provide separate services (ie. Health Care Aid and Nurse), they should document individually on the services they provided. If 2 staff members are going as a safe visit plan and 1 is on stand by while the other is providing the client services, the staff member that is offering the direct client service should document on ESDRT.

Clients that drop-in for Counselling/First Aid You capture the time spent with these clients by admitting them to e-sdrt, enter the appropriate time and code, then discharge them the following day. You may also follow this procedure for clients who are assessed for potential admission to HCC but who do not meet the acceptance criteria. Home Visits/Events When more than one service is provided to a client and you have already captured the home visit on the first entry (you entered 1"), then enter 0" (zero) for any subsequent service provided. Activities spent at the Health Center or away from a home visit should be counted as an event ie. Case management activities such as, case conferences or telephone contacts with other health care providers and staff Cumulative Information Cumulative information can be entered when the same service is provided to the same client on a repetitive basis? For example, if you have done a half-hour daily dressing change for the past 14 days, you can enter all visits in one entry. Enter the Hours of Service Provided (Column N ) as a cumulative total (i.e., 7 hours), up to 240 hours per row and enter 14 as the Number of Home Visits (Column O ). You can also enter each visit individually. Cumulative information can be captured for charting time, as a total monthly amount.
