Pearl River Community College Practical Nursing Dual Application Information We are pleased that you have shown an interest in the Practical Nursing Dual program at Pearl River Community College (PRCC). Our Dual program is offered to 12 th grade students attending high school in the Pearl River Community College District Counties. Entry into this program is offered only in the Fall semester. The PRCC Practical Nursing program is accredited by the Mississippi Community College Board. (MCCB) To establish if a high school student is eligible for Practical Nursing (PN) dual enrollment at Pearl River Community College they must meet the following: 1 1. Submit an acceptable ACT score and sub-test scores. Acceptable scores are: composite of 16, with the sub-test score of 14 or above in Reading and a 14 or above in Mathematics. All dual enrollment students must have ACT scores on file with PRCC 2. Completed a minimum of 14 core high school units 3. Earned a 3.0 grade point average on a 4.0 scale, or better, on high school courses, as documented on an official high school transcript 4. Applicants and their parent(s) or legal guardian must attend an information session at PRCC. We will answer questions and address concerns of parents and students. 2. A physical exam is required. The physical is to confirm that the applicant is in good health and possesses the required physical abilities to function satisfactorily within the program and the occupation. TB screening is required as part of the physical exam. The results must render a negative result or treatment protocol. 2

3. Drug screening will be required from a specific lab; Health Works located at 5909 US Hwy 49, Hattiesburg, and must render a negative result in order to continue. 4. Obtain Form #121, Certificate of Immunization Compliance from the Health Department in your county; it should state that you are complete for school, university/college, or work requirements (see specific immunizations on check-off list once accepted). 5. PRCC will submit your fingerprints for a Criminal Background Check (CBC) in accordance with section 43-11-13 of the Mississippi Code of 1972. You must not have any disqualifying event(s); as listed in section 43-11-13 of the Mississippi Code of 1972. This process is necessary for the practical nursing student to enter the healthcare facilities for clinical. All practical nursing students must participate in required clinical activities to progress and complete the PN Program. Those under 18 will be asked to submit a Notarized Affidavit which will be used until the student turns 18 at which time a CBC will be required. This is the first of two Criminal Background Checks required while in the program. 6. CPR (Health Care Provider) is required prior to any clinical activity, and must not expire prior to graduation from the Practical Nursing program. 7. Meet all other clinical facility requirements including provide proof of health insurance. 8. Qualified alternates will be placed on a waiting list and may be selected to fill any vacancies that occur prior to the end of late registration. 9. Critical Elements As a student seeking admission to the Practical Nursing program, we would like you to be made aware of some critical elements for performance and completion: 3 Stand for prolonged periods, up to eighty percent of clinical time Walk, bend, stoop, kneel, squat, push, pull, lift, and reach overhead Possess sufficient gross and fine motor coordination to perform nursing procedures See, talk and hear with sufficient acuity to perform client care

Tolerate exposure to disinfectants, soap, cleansers, plastic and other materials used in protective equipment and for procedures Complete all requirements for high school graduation Obtain an average of 80% (79.45) or greater in each course to progress in the program. All dual students must provide their own transportation to and from the class/lab and clinical sites. PRCC does not provide student transportation. The Practical Nursing Dual program grant will pay for all tuition, fees and supplies until the student graduates from high school, as long as funds are available. PRCC will inform students and their parents if funding has been approved prior to the program starting each fall. After high school graduation each student will be assessed fees in accordance with the fees other Practical Nursing students incur. A high school student who is dually enrolled in courses at Pearl River Community College is not eligible for PRCC institutional scholarships, state, or federal student financial aid. After high school graduation students are then eligible to apply for any available scholarships, state or federal student aid. Should a student decide to withdraw from the Practical Nursing Dual Enrollment program prior to high school graduation the student must have the high school counselor and their parents complete the withdrawal forms. Should a student decide to withdraw from high school, they are no longer eligible for this program. Upon graduation from PRCC, the practical nursing program graduate will receive a Certificate of Proficiency and may be eligible to write the National Council Licensure Exam (NCLEX) for practical nurses. Licensing of practical nursing is regulated by the Mississippi Board of Nursing (MBON). The MBON requires a separate fingerprinting and background check conducted by their criminal investigations department prior to graduation. This will be the second Criminal Background check you will have completed while in the program. You will be assessed a fee for this service by the MBON. Conviction of a misdemeanor or felony offense may be grounds for refusal by the board to issue a license. This will be scheduled during the fourth semester of the program. 4 Definition of Practical Nursing The nursing faculty supports the description of the practical nurse from the State of Mississippi Law and Rules and Regulations, 2012. The practice of nursing by a licensed practical nurse means the performance for compensation of services requiring basic knowledge of the biological, physical, behavioral, psychological and sociological sciences and of nursing procedures which do not require the substantial skill, judgment, and knowledge required of a registered nurse. These services are performed under the direction of a registered nurse of a licensed physician or

licensed dentist and utilize standardized procedures in the observation and care of the injured and infirm; in the maintenance of health; in action to safeguard life and health; and in the administration of medication and treatments prescribed by any physician or licensed dentist authorized by state law to prescribe. Dual Enrollment Student/Parent Agreement Student: Social Security Number School: Course Term We, the student and parent (or guardian), agree for the named student to enroll in the Dual Enrollment Program offered in coordination with the school district and Pearl River Community College. We understand the course selection for enrollment will be authorized each term by the high school counselor for courses that are creditable toward the high school diploma. We agree to abide by guidelines of the Dual Enrollment Program, as well as the College policies and procedures, while enrolled in PRCC. We will cooperate with both the high school and College in fulfilling responsibilities. We understand that any courses registered for, or grades received, become a permanent part of the student s college record. At the end of each semester, we authorize PRCC to forward grades to the high school. We understand it is the student s responsibility to receive approval from the high school counselor for permission to drop or withdraw from a Dual Enrollment course. We certify that all the information submitted is true to the best of our knowledge. Family Educational Rights and Privacy Act (FERPA) The Family Education Rights and Privacy Act (FERPA) (20 U.S.C. & 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education. FERPA gives parents certain rights with respect to their children s education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are eligible students. Parents or eligible students have the right to inspect and review the student s education records maintained by the school. Schools are not required to provide copies of records unless for reasons such as great distance, it is impossible for parents or eligible students to review the records. Schools may charge a fee for copies. Parents or eligible students have the right to request that a school correct records which they believe to be inaccurate or misleading. If the school decides not to amend the record, the patents or eligible student then has the right to a formal hearing. After the hearing, if the school still decides not to amend the record, the parents or eligible student has the right to place a statement with the record setting forth his or her view about the contested information. Generally, schools must have written permission from the parents or eligible student in order to release any information from a student s educational record. However, FERPA allows schools to disclose those records, without consent, to the following parties or under the following conditions (34 CFR & 99.31) School officials with legitimate educational interest Other schools to which a student is transferring Specified officials for audit or evaluation purposes Appropriate parties in connection with financial aid to a student Organizations conducting certain studies for or on behalf of the school Accrediting organizations To comply with a judicial order or lawfully issued subpoena Appropriate officials in cases of health and safety emergencies and State and local authorities, within a juvenile justice system, pursuant to specific State Law Schools may disclose, without consent, directory information such as a student s name, address, telephone number, date and place of birth, honors and awards and dates of attendance. However, schools must tell parents and eligible students about directory information and allow parents and eligible students a reasonable amount of time to request that the school not disclose directory information about them. Schools must notify parents and eligible students annually of their rights under FERPA. The actual means of notification (special letter, inclusion in a PTA bulletin, student handbook or newspaper article) is left to the discretion of each school. 8
