One thing we know is essential to being partners in your health care is communication. This packet was developed to help you understand our processes, your rights, what you can expect from us, and how you can be an advocate in your own health care. We take your health seriously and are honored to have you trust us with your care. Our Mission The fundamental mission of Shawnee Health Service is to improve the health and welfare of southern Illinois and southwest Indiana residents through the promotion, development and administration of quality, comprehensive health and social services, while effi ciently utilizing limited resources. This will include assessing and serving the needs of the underserved/vulnerable populations and designing programs and services which are culturally and linguistically appropriate. To accomplish our mission, Shawnee Health Service provides integrated primary medical and behavioral health care. Additional specialty services include dental care, behavioral health care, OB/GYN, in-house pharmacy, nutrition counseling, and related social services. 4

Our Commitment to You Our commitment to you is to provide you and your family with the highest quality care which includes treatment of chronic health conditions along with personalized preventative care. We focus on best practice medicine, which means our care is based on the best available clinical evidence, research, care concepts, and techniques known to provide high quality medical care. Your Health Home medical team will listen to your questions and help you fi nd your way through the healthcare system. Your medical team will keep a complete record of your medical history, health status, medications, test results, self-care information, and care received from other doctors. By getting to know you, your team can help you understand your healthcare needs and provide you with the information you need to manage your health. Why should you choose a Shawnee Health Care Practitioner? It is important for every individual, at any age, to have a primary care practitioner and a medical home. Your primary care practitioner should be able to understand and manage your medical needs, help you focus on prevention and wellness and be able to offer you appropriate referrals should you need a secondary care specialist. Having a consistent medical home allows you to have a record of all your visits, diagnostic tests, medications, procedures and hospitalizations in one place. This allows your practitioner to have access to all of your healthcare information, thereby improving your care and reducing errors. The benefi ts of choosing a Shawnee Health Care practitioner: A Patient-Centered Medical Home approach to your care A caring, attentive medical team that will focus on helping you achieve positive outcomes, maintain your wellness, and prevent illness A highly qualifi ed professional team with a variety of areas of expertise to meet your needs; team members are Licensed/Certifi ed and all maintain a regular level of continuing education in their chosen healthcare fi eld A team where performance is routinely measured by patient outcomes, peer review, and patient satisfaction Many support services including nutrition education, behavioral health counseling, case management, care coordination, and free enrollment assistance to help you with Medicare, Medicaid, insurance coverage and sliding fee discounts Access to a Patient Portal that gives you 24/7 access to your visit record, the ability to make on-line appointments, advanced check-in for your appointment before your visit, see your lab results, and more The ability to make a same day appointment with your practitioner for urgent medical needs Many convenient locations and hours, reminder calls and text messages about your upcoming appointments, and a Same Day walk-in clinic for minor illnesses and injuries, open days, evenings, and weekends with no appointment needed 6

Appointments Appointments are preferred though walk-in appointments are available on a limited basis. To improve communication and continuity of care, we will try to schedule your appointments with your chosen primary care provider whenever possible. When your primary care provider is not available, a member of their healthcare team will see you. To schedule appointments call (618) 519-9200 or visit the Patient Portal on-line, there is a link on our website at www.shawneehealth.com. To cancel an appointment, it is important that you call us before the close of business on the day before your scheduled appointment. This will help us improve access to care for all of our patients. Be prepared to ask all of your questions. Bring all of your medications. Bring Photo ID and Insurance Information. Be prepared to pay your co-payment or nominal fee. Minors must be accompanied by a parent or guardian, unless prior arrangements have been made. When making your appointment, let us know your special needs. We offer free sign language interpreters and telephone interpretation for languages other than English. Our case managers and our team of bilingual, bicultural outreach workers, professionally trained to provide medical interpreting in Spanish at Shawnee Health Care clinics and at referral appointments by Shawnee providers, may provide case management, health education, outreach services, and help with transportation arrangements upon request. For the health and safety of our staff and patients, Shawnee Health Service does not allow smoking (within 14 feet of our facilities) or weapons, which includes, but is not limited to fi rearms, electric weapons (like Tasers), knives with a blade in excess of three inches, and fi ghting sticks. Law enforcement personnel and on-duty security guards are exempt. Expectations for Cancellations/No Show Policy If you need to cancel an appointment, it is important that you call us before the close of business on the day before your scheduled appointment. This will help us maximize access to care for all of our patients. To honor your time and the time of the other patients with appointments that day, we may need to reschedule your appointment if you are more than 15 minutes late. Patients who do not attend two consecutive scheduled appointments without giving suffi cient notice will be referred to a Case Manager who will work with the patient to determine how we might avoid additional missed appointments. It may be decided that the patient will be placed on Same Day Status for appointments and will need to call or walk into the health center to be seen on the same day as their request for an appointment. To request removal from Same Day Status, you will need to have attended two consecutive appointments, make your request, and then meet with the Health Center Manager and your provider to review your status. You will be notifi ed within ten business days of your request. 7

How to Stay Connected We will be asking you to confi rm your preferred method of communication. Once you have selected this you will receive appointment reminders, lab or imaging results, and reminders to schedule your preventative and chronic care appointments. You are always welcome to call us to get assistance: (618) 519-9200. For your convenience, you can view and manage your personal health information, securely, anytime, day or night, through the Patient Portal. Patient Portal With the Shawnee Health Service Patient Portal, you will be able to: Request Appointments, Receive Appointment Reminders, and Check In prior to an Appointment Correspond with your doctor and healthcare team on-line via our Secure Portal View, Download, or Share your Personal Health Record, including data such as: Medications, Immunizations, and Test Results Request Prescription Refi lls Pay Your Bill On-line To sign up for the Patient Portal you can fi nd the link on our website at www.shawneehealth.com, or speak with any staff member at one of our health centers for assistance. WE RESPECT YOUR PRIVACY. Receive notices to your personal e-mail account when there is important information awaiting you in your Patient Portal account. If you choose to use the Patient Portal, your name and e-mail address will be treated with the same care and privacy given to your medical records. WHO CAN SIGN UP? Patients and their legal guardians may sign up for Patient Portal in accordance to the Terms and Conditions of the website. Just ask your doctor about enrolling and you are on your way to connecting with your healthcare team. Concerns or Questions For non-emergency concerns, call (618) 519-9200 during offi ce hours and our staff will assist you. You may also send your non-urgent questions to your healthcare team through the Patient Portal. After Hours / Emergencies For all EMERGENCIES, call 911 or go to the nearest emergency room. For urgent medical concerns outside of offi ce hours, call (618) 519-9200 and our on-call provider will assist you. For most medical problems you should see your regular healthcare team. You get the best care there because they know you and your medical history. Shawnee Health Care, Same Day (walk-in health center) For untimely minor illnesses and injuries our Same Day health center is open 7 days-a-week, evenings, and weekends. No appointment needed. Same Day Hours: Monday through Saturday: 9:30 am 7:00 pm Sunday: 11:00 am 5:00 pm 9

Referrals Shawnee Health Service is your fi rst stop for health check-ups and wellness advice. Regular visits with your primary care provider can help you stay healthy. When you need the attention of a specialist, we will help with referrals to qualifi ed local and regional medical and allied health professionals. To request a referral, speak with your physician, nurse practitioner or physician assistant. In-House Pharmacy Shawnee Health Service has established an in-house pharmacy for our patients called Shawnee Health Care, Pharmacy, located within Shawnee Health Care in Murphysboro. Our pharmacy has its own drive through window on the south side of the building. Our pharmacy s ScriptCenter locations allow secure pickup of non-refrigerated prescriptions in Carbondale, and Marion as well. Refi lls and Prescriptions To request a prescription refi ll, please contact your pharmacy directly. If you have no refi lls remaining, the pharmacy will send a request for a new prescription to your provider. Prescription refi ll requests typically take 3 days to fi ll. If you have requested a refi ll and it has been more than 3 days, please call (618) 519-9200 to learn the status of your prescription refi ll. Prescriptions cannot be fi lled after hours. 10

Payments and Insurance Shawnee Health Care is committed to providing quality health care for our patients at the lowest possible cost and ensuring affordable medical and dental services are available to everyone in the community. For Patients with Insurance: Physicians, Dentists and Providers at Shawnee Health Care are enrolled in Medicaid, All Kids, Medicare and most forms of commercial insurances. It is your responsibility as the Patient to understand the terms of your insurance coverage. Please call the phone number on the back of your member identifi cation card if you have questions about your insurance coverage. Shawnee Health Service will submit claims to your insurance company for you. Insurance deductibles, coinsurance, and copays are due at the time of check-in. Cash, personal check, debit card or major credit cards are accepted. We offer on-line payment on our website at www.shawneehealth.com or through the patient portal. We also offer our patients the option of making a payment plan for large outstanding balances. For Patients with Limited or No Insurance: At Shawnee Health Care, we want you to be able to access the primary medical, dental and behavioral health services you need and help you with coverage and payment concerns. SHAWNEE FINANCIAL ASSISTANCE PROGRAM Shawnee Health Service is able to discount services provided at Shawnee Health Care locations through grant funding from the U.S. Public Health Service, Bureau of Primary Health Care for patients at or below 200% of the Federal Poverty Guidelines. People who are uninsured and people with insurance who have spend-downs, deductibles, co-insurance and copays can receive discounts for services. Discounts apply only to services provided at Shawnee Health Care locations. Eligibility is determined by your family size and gross income (before taxes and deductions) An application form must be completed and requires proof of income Processing may take up to 2 business days Co-Pays and/or Nominal Fees are due at the time of service 11

Your Rights and Responsibilities What you need to know about your Rights as a Patient You have the right to: 1. Receive services without discrimination. 2. Obtain complete and understandable information about your health care. 3. Receive information necessary to give informed consent prior to the start of any procedure. 4. Know the name and professional background of anyone working with you. 5. Have an advance directive (such as a living will, health care proxy, or durable power of attorney for health care) to the extent permitted by law and Shawnee Health Service policy. 6. Refuse services or treatment, or any part of it, or to stop services or treatment when you wish. 7. Privacy during all interviews, examination and treatment procedures, and counseling services. 8. Review records pertaining to your care and to have the information explained or interpreted as necessary, except when restricted by law. 9. Know the charges for all services. 10. Expect to be treated fairly, courteously, and with respect at all times. 11. Understand the business and fi nancial relationship between Shawnee Health Service and other health care organizations. 12. Expect reasonable sharing of health care information when appropriate and as required by law. 13. Confi dentiality in all records and in all communication. What you need to know about your Responsibilities as a Patient: You have a responsibility to: 1. Call your Health Center in advance if you are unable to keep an appointment. 2. Provide accurate information that affects your health care, past and present. 3. Let us know if you are treated unfairly. 4. Ask questions until you clearly understand your health care information. 5. Follow your plan of care. 6. Accept undesirable outcomes if you refuse treatment or fail to follow your plan of care. 7. Take an active role in your own health. 8. Make payment at the time of service. 13
