On a day-t-day basis the rle will primarily be based in the cmmunity team and will pick up a 50% patient and/r caching lad depending n the acuity levels and verall need. The rle will have verall respnsibility fr bth nursing teams ensuring the prvisin f high quality, seamless care. This rle will als wrk clsely with the Clinical Develpment Specialist and the Referral Crdinatr. Key Accuntabilities and Expected Results Accuntability Expected Results Ensures the prvisin f Ensures clinical service prvisin fr the Hspice is driven by rganisatinal values, visin and strategy quality, cmprehensive palliative nursing Takes primarily respnsibility fr the daily versight f what is ccurring in the cmmunity team and versees the IPU Team Leader t ensure the nurses are well supprted and patient/family needs are being met care Ensures relatinships are established and maintained with external prviders: primary and cmmunity prviders, such as the DHB s, District Nurses, Te Puna Haura, ARCF s Attends rganisatinal meetings, multidisciplinary meetings, case review and family meetings as required Organises and runs mnthly, frmal team meetings t facilitate exchange f infrmatin and discussin f team and rganisatinal issues Prvides supprt fr Hibiscus and Warkwrth Wellsfrd Hspices as directed by the Clinical Services Manager.

Empwers the Team Effectively manages resurces within an agreed annual budget Undertakes the delivery f internal and external educatin such as the Fundamentals f Palliative Care Effectively leads the nursing teams by: Rle mdelling required behaviurs t facilitate psitive team culture Using caching techniques with individual staff t ensure expectatins are understd and negative behaviurs are discuraged. Supprting the wrk f the team by facilitating the planning, implementatin and evaluatin f care t patients and families in a manner which prmtes excellence and reflects rganisatinal values.
