On a day-t-day basis the rle will primarily be based in the cmmunity team and will pick up a 50% patient and/r caching lad depending n the acuity levels and verall need. The rle will have verall respnsibility fr bth nursing teams ensuring the prvisin f high quality, seamless care. This rle will als wrk clsely with the Clinical Develpment Specialist and the Referral Crdinatr.
