Koehler) Pages: 7 This supplement implements and extends the guidance of Air Force Instruction (AFI) 91-202, Air Education and Training Command (AETC) Supplement, The US Air Force Mishap Prevention Program, establishes the local mishap prevention program requirements, assigns responsibilities for program elements, and contains local program management information. It applies to all individuals involved in the Vance Air Force Base (VAFB) mishap prevention program to include tenant units, Air Force Reserve (AFRES), and Air National Guard as specified in host tenant agreements and implements the VAFB Mishap Prevention Plan. This instruction requires collecting and maintaining information protected by the Privacy Act of 1974 (5 United States Code (U.S.C.) 552A) and AFI 33-332, Air Force Privacy Program. Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with (IAW) Air Force Manual (AFMAN) 33-363, Management of Records, and disposed of IAW Air Force Records Information Management System (AFRIMS) Records Disposition Schedule (RDS), or any updated statement provided by the Air Force (AF) Records Management office (SAF/CIO A6P). Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using AF Form 847, Recommendation for Change of Publication; route AF Form 847 from the field through the appropriate functional s chain of command. See Attachment 1 for a Glossary of References and Supporting Information.

2AFI91-202_AETCSUP_VANCEAFBSUP 22 MARCH 2017 CORRECTIVE ACTION APPLIED ON 10 SUMMARY OF CHANGES This document is substantially revised and must be completely reviewed. This publication is revised and procedures must be completely reviewed. This supplement realigns paragraph numbers to match the revised parent AFI and AETC Supplement. 1.8.16.13. A roster of personnel with access to Air Force Safety Automated System (AFSAS) with privilege information will be compiled during the first quarter of each year. 71st Flying Training Wing Safety (71 FTW/SE) will conduct training to these individuals. All briefings with privilege information will begin with a brief overview of privilege information protection to ensure that all members of the audience are trained. 1.8.21.13. VAFB mandates the use of the AETC Form 29A, Commander's Newcomers' Safety Briefing. 1.8.22.5.1.1. (Added) VAFB mandates the use of the AF Form 55, Employee Safety and Health Record, to document safety, health, and certain emergency management and anti-terrorism training requirements. A pre-filled AF Form 55, with the minimum Vance requirements, is available on the Vance Sharepoint site; https://vance.eis.aetc.af.mil/71ftw/se/default.aspx. AF Form 55 is used to add training items as needed to meet the unique needs of that work center. A new AF Form 55 must be accomplished any time an individual changes Organization, Office Symbol, Facility, or Permanent Change of Assignment (PCA). 1.8.22.5.1.2. (Added) Supervisors may pen and ink changes to existing AF Form 55s in order to add new items. Note: Ensure all employees know who their Antiterrorism Representative, Emergency Management (EM) Representative and Unit Safety Representatives (USR) are and how to contact them if necessary for additional information. Following are the standard minimum training items that should be listed in Part III of the AF Form 55 at VAFB. 1.8.22.5.1.2.1. (Added) Supervisor Safety Training (SST): Supervisors must attend SST (see AFI 91-202, paragraph 2.3.2.1., for details on who must attend). If the employee is not a supervisor, mark this item N/A. SST is scheduled through USRs. This is a one-time training requirement. Dates are tracked in Military Personnel Delivery System (MilPDS), but also documented on the Form 55. Unit Training Managers (UTMs) have access to MilPDS to retrieve dates. 1.8.22.5.1.2.2. (Added) Risk Management (RM) Fundamentals: All AF personnel must complete RM Fundamentals training through one of the methods outlined in AFI 90-802, Risk Management, paragraph 10.1. This is a one-time requirement, and to satisfy the requirement, employees may complete the AF RM Fundamentals course via (ADLS). 1.8.22.5.1.2.3. (Added) Personal Protective Equipment (PPE): Train employees on the proper use, location, fit, care and limitations of any PPE required within the work center. If PPE is not required, mark this item N/A. This must be accomplished each time an individual changes work-center location and/or a new hazard is introduced to the work-center and/or PCAs. 1.8.22.5.1.2.4. (Added) Job Safety Training Outline (JSTO): Train employees IAW the work center s JSTO. AFI 91-202, as supplemented by AETC and VAFB, directs every supervisor to develop a JSTO for use in providing safety and health training to his/her employees. Contact 71 FTW/SE if assistance with JSTO development is needed. This must be accomplished each time an individual changes work-center location and/or PCAs.

(Added) Active Shooter Reaction Training: Train employees on active shooter responses based on their assigned building. Conduct a facility walkthrough to show escape routes, danger areas (rooms with no protection due to open space or large/many windows) and how to lock doors/windows as applicable. Further information can be received from the employee's Unit Antiterrorism Representative. This must be accomplished each time an individual changes work-center location and/or PCAs. 1.8.22.5.1.2.6. (Added) Workplace Emergency Preparedness: Provide employees with an EM trifold and review its contents this must be accomplished each time an individual changes workcenter location and/or PCAs. This is a part of the JSTO requirements. 1.8.22.5.1.2.7. (Added) EM Workplace Walkthrough: Conduct a facility walkthrough with employees to show them Shelter in Place (SIP) and Take Cover locations/procedures. Point of Contact (POC) is EM: 213-6409/5791. This must be accomplished each time an individual changes work-center location and/or PCAs. 2.1.4.1. Wing Flight Safety Officer (FSO) Selection Procedures: Each wing FSO should be highly qualified in his/her aircraft and be a flight commander or flight commander quality individual. 71 FTW/SE will notify the unit commander that will provide the replacement FSO 60 days prior to a wing FSO s PCA or permanent change of station (PCS). The commander will provide a replacement s name within 30 days of notification. This will allow 30 days of overlap training with the departing wing FSO. The unit commander s nomination will be coordinated through 71 FTW/SE. 2.1.4.2. Each of the following units will appoint a primary and an alternate Squadron Assigned Flight Safety Officers (SAFSO). The primary SAFSO and alternate SAFSO may also be the primary and alternate ground USR, respectively. The primary and alternate SAFSOs may be assigned from another squadron operating the same type aircraft, as long as both commanders approve. Each unit will provide 71 FTW/SE with an appointment letter: 3rd Flying Training Squadron (FTS), 8 FTS, 25 FTS, and 33 FTS. SAFSOs who are also acting as USRs will also be trained IAW paragraph 2.2.1. 2.2. Appointment of USRs. 2.2.1. Each of the following units will appoint a primary and alternate USR. Each unit will provide 71 FTW/SE with an appointment letter. Personnel should have at least 1-year of retainability at the time of appointment. 2.2.1.1. (Added) 71st Operations Support Squadron (71 OSS). 2.2.1.2. (Added) 71st Force Support Squadron (71 FSS). 2.2.1.3. (Added) 71st In-Service Support (71 ISS). 2.2.1.4. (Added) 71st Medical Group (71 MDG). 2.2.1.5. (Added) 71st Security Forces Squadron (71 SFS). 2.2.1.6. (Added) 71st Student Squadron (71 STURON). 2.2.1.7. (Added) Wing Staff Agencies (WSA). 2.2.1.8. (Added) Air Force Office of Special Investigations (AFOSI), DET 438.

(Added) 71 FSS will manage the safety program for the 71st Mission Support Group (MSG) Command Section (CSS). 2.2.1.14. (Added) 71 OSS will manage the safety program for the 71st Operations Group (71 OG) CSS. 2.2.2.8. (Added) USRs will maintain documentation in their USR safety program management book. 7.3.5.1.5. Conduct semiannual safety self-inspections using the Major Command Inspection Compliance Tracker (MICT) Communicators USR Responsibilities and Supervisor Occupational Safety Responsibilities (if the USR and SAFSO are the same person, only one semiannual safety self-inspection is required). 7.3.7. (Added) VAFB Safety Investigation Board (SIB) Program. 7.3.7.1. (Added) Potential SIB members for the positions of Board President (BP), Investigating Officer (IO) and Maintenance Member (MM), will be acquired via a 71 FSS s monthly query of MilPDS to determine which personnel on base have attended the appropriate courses to allow them to serve in that role. When a new member is identified for the SIB roster they will be advised (along with their USR/FSO) to provide 71 FTW/SE with the following information: rank, unit/office symbol, duty and home/cell phone numbers, type of formal school training (Board President Course (BPC), FSO, Aircraft Mishap Investigation Course (AMIC), and/or Aviation Safety Program Management (ASPM)), date of training, previous SIB experience, aircraft flown, and total flying hours. If needed, potential SIB members will be provided the required annual training to accomplish. Month and year of annual training accomplished will be annotated on the SIB roster. 7.3.7.2. (Added) Pilot members, recorders, weather specialists, and air traffic control specialists for a SIB/Intermediate Staging Base (ISB) list will be determined on an as-needed basis per the following. 7.3.7.2.1. (Added) When 71 FTW/SE receives a request from AETC/SE and/or HQ AETC/SE for available candidates to participate on a mishap board, 71 FTW/SE will notify the 71st Flying Training Wing Commander (71 FTW/CC) of the tasking availability of unit members listed on the VAFB SIB/ISB list (including pilot members, recorders, maintenance members and any additional members as required by HQ AETC/SE). 7.3.7.2.2. (Added) 71 FTW/SE and/or their representatives will review the list and rank order personnel based on inputs and alibis from individuals. 7.3.7.2.3. (Added) Based on submitted information 71 FTW/SE will forward a list to the 71 FTW/CC for final approval prior to forwarding onto HQ AETC/SE. 7.5.4. 71 FTW/SE will distribute an SIB refresher presentation annually to serve as member training. Month and year of annual training accomplished will be annotated on the SIB roster.

The 5 FTS will manage its safety program IAW AFRES directives and the host/tenant support agreement. 8.5.11. (Added) Conduct semiannual safety self-inspections using the MICT Communicators USR Responsibilities and Supervisor Occupational Safety Responsibilities (if the USR and SAFSO are the same person, only one semiannual safety self-inspection is required). 9.4.11.
