Preference given to applicants with Post-graduate qualifications in Pain Medicine but not essential. 5. Excellent written and verbal communication skills. 6. Demonstrated ability to work in a multidisciplinary team. 7. Demonstrated ability to build and maintain networks across multi-disciplines. 8. 2. Context and Scope HammondCare is an independent Christian charity that exists to improve quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence based best practice in its services and we provide these services through home care, subacute hospitals and residential aged care. Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances. Summary of Position The role of the Clinical Nurse Consultant works within the Mission and Mission in Action of HammondCare to ensure clinical leadership and use of advanced nursing practices, incorporating research, education and management to achieve optimal outcomes for patients within a designated case load. The service will involve the provision of clinical pain management support services and education by Greenwich Hospital Pain Management Service to clinicians and patients within Far West NSW LHD. The collaboration will involve the use of various approaches to help provide clinical support and build local capacity for the management of chronic non-cancer pain. The essence of this role is to: as a senior nurse in the pain clinic to assist in developing a new telehealth chronic pain service to Far West NSW local health district (LHD), serving as an initial point of contact for patients and health professionals and developing an efficiently run service. Coordinate pain clinics, liaising with patients, health professionals and colleagues at Greenwich Hospital to organise individual consultations, case conferences and multidisciplinary clinic meetings Participate in and provide clinical care, education and training by face-to-face visits and videolink to Far West LHD. Support and initiate the development of policies and guidelines regarding clinical pathways and protocols to assist provision of care. Assist with reporting requirements to NSW Health and NSW Agency for Clinical Innovation 3.
