Dear Prospective Debutante, Ivy Vine Charities, Incorporated, the non-profit foundation of Alpha Kappa Alpha Sorority, Incorporated, Theta Omega Omega Chapter, is pleased to announce its 2017 Debutante Cotillion program which will formally begin in Spring 2017. The program is open to young ladies who are enrolled in high school as freshmen, sophomores, juniors or seniors during the 2017-2018 school year and who have demonstrated academic stability (current GPA of 2.5 or higher). We cordially invite you to submit an application to join the esteemed alumnae of Montgomery County and the Metropolitan DC area who have participated in our program. Historically, a cotillion was characterized as a formal, social dance. In recent years, it is most often recognized as a formal presentation of young ladies referred to as debutantes. These young ladies are presented to society after they have developed social skills and acquired a sense of responsibility and achievement that prepares them to conquer life s challenges as a lady. Ivy Vine Charities, Incorporated embraces this concept with a program that affords young ladies the opportunity to develop and enhance their personal, educational, cultural, spiritual and social lives. This unique program culminates with a formal Debutante Cotillion Ball to be held in November 2017. Please submit a Cotillion Application Package consisting of: Completed Debutante Cotillion Application Form and Participation Certification; Cotillion Statement of Qualification; Official High School Transcript (Sealed); Two letters of recommendation (non-relative and one teacher or counselor); Typed essay; (500 words or less); Headshot Photo (up to 4 x 6) Please send the completed Cotillion Application Package to: Ivy Vine Charities, Incorporated Attn: Debutante Cotillion Application 43 Randolph Road PMB 102 Silver Spring, MD 20904 Application must be Received by Tuesday, February 21, 2017 For additional info or if you have any questions please email: info@ivyvinecharities.org Please note that Ivy Vine Charities, Incorporated reserves the right to eliminate any incomplete, unsigned, late or illegible applications A Debutante Cotillion fee is required and the young ladies must adhere to the rules and regulations set by Ivy Vine Charities, Incorporated. The fee is due upon acceptance in the program but may be payable in installments. However, any young lady who does not adhere to the rules and regulations set by Ivy Vine Charities, Incorporated, may be removed from the program and no refunds will be issued. Page 1

Eligibility Requirements Debutantes Must: Fees and Awards Fee Covers As of the 2016 2017 School Year be in 10th or 11th grade; Demonstrate academic stability (current GPA of 2.5 or higher); Be in good standing in her school and community; Exhibit high moral character; Have a commitment to public and community service. Debutantes will be expected to pay a cotillion fee of $500 (which can be paid in installments), purchase their personal Cotillion attire, secure souvenir journal advertisements and patrons, and sell tickets for the Cotillion. The $500 fee is also counted towards the amount that Debutantes are requested to raise. Recognition of debutantes includes 1st, 2nd, and 3rd place winners and certificate for participation. Two (2) tickets to the Debutante Cotillion presentation and dinner dance; Two (2) tickets to the Mother/Daughter Tea Luncheon; Admission to Cultural Events; Commemorative photo in the 2017 Debutante Cotillion Souvenir Journal; and Accessory package including but not limited to gloves, pearls, flowers and a copy of the Cotillion DVD. Junior Debutantes Must: Fees and Awards Fee Covers As of the 2016 2017 School Year be in 8 th or 9th grade; Demonstrate academic stability (current GPA of 2.5 or higher); Be in good standing in her school and community; Exhibit high moral character; Have a commitment to public and community service. Junior Debutantes will be expected to pay a cotillion fee of $250 (which can be paid in installments), purchase their personal Cotillion attire, secure souvenir journal advertisements and patrons, and sell tickets for the Cotillion. Recognition of junior debutantes includes certificate for participation. One (1) ticket to the Debutante Cotillion presentation and dinner dance; Admission to Cultural Events; Commemorative photo in the 2017 Debutante Cotillion Souvenir Journal; and Miscellaneous items including but not limited to gloves, pearls, flowers and etc. Page 2

The program is scheduled to kick off in March 2017. A calendar of events and activities will be provided at the Orientation Session. What are the expectations of each Debutante once selected? Deb and family must attend the orientation session(s). Deb must attend all rehearsals and arrive on time. Deb must adhere to the rules and regulations set by Ivy Vine Charities, Inc. Deb must participate in Debutante Cotillion activities. Deb and family must identify a male Escort for the Cotillion. Deb must meet the minimum financial requirements for fundraising. o Fundraising can be done over the summer months Page 6

Statement of Qualification It is the expectation of Ivy Vine Charities, Incorporated that participants in the Debutante Cotillion program adhere to the tenets of high ethical standards, moral behavior and those attributes that are up-lifting in mind, body and spirit. As a participant, she shall, at a minimum refrain from inappropriate use as determined by Ivy Vine Charities, Incorporated of the social networks, i.e., Instagram, Snapchat, Twitter, Facebook, etc. Ivy Vine Charities, Incorporated retains the right to exercise exclusive control, regarding the participation of any applicant in the Debutante Cotillion Program by declining an applicant s participation and withdrawing the privilege of participation. Each participant is requested to affirm the truthfulness of each of the following statements by signing the bottom of this statement. I, confirm that I meet all the Debutante Cotillion participation criteria listed below and understand that I must maintain these standards throughout the 2017 Cotillion season: 1. Cumulative grade point average of 2.5 or better (verified by official transcript); 2. Demonstrated interest and involvement in public service and community related activities; 3. Demonstrated goals, aspirations, and plans to pursue an education beyond high school; 4. Evidence of good moral character, which includes but not limited to: a. No record of poor discipline at school or in the community; b. No criminal record of any kind; c. Must not be married; d. Must not be a parent or pregnant; e. No use of illegal drugs or alcohol; f. In the event of a change of circumstance after date of this certification regarding any matter contained herein ( items 4a 4e), I will immediately and forthrightly notify the Cotillion Chairmen of the details of any such matter and 5. Attendance of rehearsals and activities that occur throughout the Cotillion season; and 6. No use of electronic media/devices allowed during Debutante Cotillion events as determined by Ivy Vine Charities, Incorporated.
