Consultation Times Monday, Tuesday, Wednesday, Friday 08:00 12:00 15:30 18:00 Thursday 08:00 12:00 Extended Hours Monday, Wednesday, Friday 18:30 19:00 Saturday 08:30 10:00 Appointments : Doctors/Nurses/HCA We operate an appointment system for Doctors and Nurses. As per our Access Policy we are required to give an appointment within 48 hours with a health care professional. Appointments can be booked online or by telephone on the day or in advance. Whilst we try to give an appointment on the day you request it, we cannot guarantee you will see the Doctor of your choice. The nature of general practice is such that we cannot guarantee you will be seen at the appointed time; some consultations are prolonged and your Doctor may be delayed in an emergency. Telephone Triage Many things can be dealt with, on the phone and can save an unnecessary visit to the surgery. If you wish to speak to a Doctor or Nurse, please ring the surgery and ask for a telephone consultation giving the demographics and a brief outline of the advice needed and you will be contacted by the clinician or the on call Doctor triaging the session. Home Visits Patients who are severely disabled, elderly or too ill to attend the surgery, could request for a home visit before 10.00am (not having transport is not a valid reason to request home visit by the Doctor). All requests for home visits have to be agreed by the Doctor. When the Receptionist asks for the symptoms please co-operate as she is merely carrying out instructions to ensure that urgent cases are given the priority they require. Out of Hours If your problem is not urgent you are urged to wait and phone when the surgery is open. The out-of-hours service is operated by PELC, based at King George Hospital. In the case of an emergency when the surgery is closed phone the out-of-hours service on 0845 075 0497. For advice call NHS 111 (open 24 hours). Services Provided At The Surgery In addition to general medical services, we provide specialist support for patients with Hypertension, Diabetes, Asthma, COPD and CHD. We also offer Travel Vaccinations, Child Health Surveillance, Healthy Heart, Childhood Immunisations, Minor Surgery, Cryotech, Ante Natal and Post Natal, and New Patient Health Checks. For reasons of safety and security we cannot accept requests made verbally or by telephone. Test Results Please telephone for test results between 12.30-13.30 on Monday, Tuesday, Thursday and Friday. Cervical and Breast Screening Cervical smears are for the early detection and prevention of cancer of the neck of the womb. They are taken by the Practice Nurse or Doctor by an arranged appointment. We recommend you have this service if you have not had a smear within the last three years and you are aged 25-49 or within the last five years if you are aged 50-64. If you are unsure we will advise you from your records. Breast screening for women over 50 years of age is very important. The NHS Breast Screening programme saves 1400 lives each year. Immunisations Immunisation are very important for children. Illnesses such as diphtheria, tetanus, and polio are now rare due to immunisation policies, but if contracted can be crippling or even fatal. We strongly recommend children are vaccinated. If you have any worries or queries please discuss them with the Doctor or Practice Nurse. Freedom of Information Publication Scheme The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the classes of information the practice intends to routinely make available. A copy is available on request. Patient Confidentiality and Data Protection We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team. Comments And Complaints The Practice Manager is responsible to the Doctors for the administration of the practice. Comments, suggestions or complaints should be addressed to the Complaints Officer by completing a form available on request at the reception. We are committed to giving you the best possible service at all times within the constraints of our limited resources. Please help us to help you. What We Aim To Do For You Treat you with courtesy and respect. Not discriminate on the grounds of race, social class, age, religion, sexual orientation, appearance or medical conditions. Discuss with you the care and treatment we can provide. Maintain complete confidentiality. Allow access to your medical records within the limitations of the law. Provide good quality care by appropriately qualified staff. Whenever possible offer patients their services from a particular clinician if a preference is expressed. Provide emergency care when required. Refer you to a consultant acceptable to you when your GP thinks appropriate. Arrange a second opinion when your GP agrees it is necessary. Give a full and prompt response to any complaints about our service. What We Would Ask Of You Treat us with courtesy and respect. Tell us if you are unsure of the treatment or advice you are offered. Only use the emergency service when absolutely necessary. Request a home visit only when the patient is unable to attend the surgery through illness and infirmity. Inform us as early as possible if you cannot keep your appointment. If you repeatedly fail to keep appointments and not inform us in advance you may be removed from our list. Inform us of any change of name, address or telephone numbers. Tell us about any concerns as soon as possible to allow us to deal with them. Let us know how our service can be improved. (In writing please). Remember that book on day appointments are for acute, immediate problems only. Have a realistic expectation of the service. Switch off your mobile phone whilst in the surgery. Any patient who is violent will be removed from the practice list immediately and the incident will be reported to the Police. To Register At Ilford Medical Centre You will need to be living within the practice area, the boundaries of which are IG1. We have disabled access and do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance or medical conditions. Please bring in your medical card and a proof of ID and address. You are requested to complete a form for yourself and any other family members wishing to register. All people over 16 must sign the form themselves. If you have previously been registered with a doctor in the UK you will need to let us know your NHS Number this can be obtained from your previous GP. Forms are available from reception. If you have not been registered with a doctor in this country you will be asked to bring in official documents and proof of address. You will be offered a Health Check with our Health Care Assistant. This check is very important as the practice will then have some information about you. If you are on regular medication please bring this with you. If you have children under five years of age they do not need a health check but it is imperative that you bring in their Child Health Record Book and a record of immunisations.
