True life is lived, when tiny changes occur Leo Tolstoy We are ready to start FY 2016-2017 and we have made some new changes this year in how your Healthy U employee wellness program works! Take a few minutes to read this Guide and see what we have in store for you as we continue to evolve, create and change the culture of health at DMPS!! Healthy U wants you to be as active in our employee wellness program as you want to be, the choice is always yours! Our goal is simply to help you become the healthiest you that you want and can be however that looks to each person! Who Can Participate In Healthy U: All DMPS who work more than 10 hours per week are invited to participate at different levels while earning wellness incentive dollars. Show Me The Money How Much Can I Earn: Here at Healthy U, we always earn our incentive dollars in arrears. What that means is that the Healthy U Wellness incentive dollars you earn this year in FY 2016-2017 will help pay your Employee Contribution costs for your FY 2017-2018 DMPS Health Insurance! This year you will have an opportunity to earn either $600 or $400 depending on which employee group you belong in. *AFSCME employees can earn $400 and DMPS $600. The activities for ALL employees are the same, the only difference is in the incentive dollar amount you earn! *AFSCME stands for American Federation of State, County and Municipal How Does It Work: Starting with FY 2017-2018, all DMPS employees who are the Primary on the DMPS Health Insurance will have an increased annual employee contribution cost depending on your employee group. All AFSCME employees are $400 and ALL other DMPS employees are $600. The good news is, you are able to earn that ENTIRE dollar amount back by your individual participation in Healthy U Wellness Activities!!! You have the opportunity to have your Employee Contribution level range from $0-$600 depending on what you choose to do!! It s a Win/Win!!! What if I am a DMPS employee who is covered as the on the DMPS Health Insurance: You also have an opportunity to earn Healthy U Wellness Dollars! All DMPS employees who are the covered dependent on the DMPS health insurance plan have an opportunity to earn (taxable income per the Federal Guidelines) by the completion and self-reporting of all four wellness activities online via the Wellmark WebMD Wellness Center per the Tier schedule on the pages that follow. You will access this using your personal and separate Wellmark Wellness Center ID card number. What if I am a NON-Insured Employee (including part-time employees working more than 10 hours per week) You also have the opportunity to earn (taxable income per the Federal Guidelines) by the completion and selfreporting of all four wellness activities online via the Wellmark WebMD Wellness Center and Tiered schedule. You will access this using your personal Wellmark Wellness ID card number. (Subs and casual labor are not eligible). How do I access the Healthy U Wellness Reporting: Access to Healthy U Wellness reporting is completed through a single sign on via the Wellmark Website. Simply visit www.wellmark.com. First Time Users click Register now. If you have already created your account with Wellmark, then proceed to enter the User ID and Password you created! Once on the MyWellmark homepage, click on: Visit the Wellness Center and you will be redirected to Wellmark WebMD Wellness Center! To self-report your completed activities, click on the Rewards tab. Self-Reporting Changes: Healthy U has changed how and what you will report this year! As the Primary on the DMPS Health Plan, the Biometric Screening and Online Wellness Assessment will continue to automatically report for you upon completion. The Self-Reporting of your four Wellness Activities has changed. You will self-report two activities from July 1-December 31, 2016 in Tier 2, and the remaining two activities from January 1, 2017-June 1, 2017 in Tier 4! PLEASE read and follow the Healthy U 4-Tier Schedule on the pages to follow. Each has a dollar amount assigned to it. In order to earn that amount, each must be reported within the correct timeframe. Activities NOT completed and within the correct timeframe or by the stated deadline will not receive incentive credit.
Onsite Biometric Screening Online Registration begins 8/22/16 Screening must be completed through Healthy U. Screenings done at other locations will not earn the incentive. HEALTHY U 4-TIER INCENTIVE SCHEDULE FY 2016-2017 Timeline Sept 1-Dec 7, 2016 12/7/16 *AFSCME TIER 1 *AFSCME stands for American Federation of State, County and Municipal Non-Insured or Covered as $0 You may participate at no cost and self-report as a wellness Complete & Self-Report 2 of the 4 Wellness Activities Only 2 Activities can be completed and between July 1-December 31 st to earn the incentive. Reporting Time Frame July 1-Dec 31, 2016 12/31/16 AFSCME $50 TIER 2 Non-Insured or Covered as Complete 2 Activities Must Complete and Self-Report All 4 to earn the YOUR Healthy U Goal by the end of the FY 2016-2017 wellness year will be that you have completed and ONE from each of the 4 different wellness buckets shown below for a total of four wellness activities. You will only selfreport 2 activities in Tier 2 during this reporting time frame. The order in which the activities are completed is your choice. Wellness Bucket 1: Healthy U Preventive Health Wellness Bucket 2: Healthy U EDU Wellness Bucket 3: Healthy U My Personal Adventure Wellness Bucket 4: Healthy U Challenges A list of qualifying activities is located within the Guide, @DMPS Resource Page or Wellmark WebMD Wellness Center Online Wellness Assessment Complete your Online Wellness Assessment during the open campaign timeline. Any Assessment completed prior to or outside this timeline will not earn the incentive Timeline AFSCME TIER 3 Non-Insured or Covered as $0 You may participate at no cost and self-report as a wellness Jan 1, 2017- March 31, 2017 3/31/17 The Online Wellness Assessment is located on the Wellmark WebMD Wellness Center Homepage. Complete/Self-Report on the 2 Remaining Wellness Activities from January 1- June 1 st to Earn The Wellness Activities must be completed within the reporting time frame to earn the incentive. Reporting Time Frame Jan 1, 2017-June 1, 2017 6/1/2017 AFSCME $50 TIER 4 Non-Insured or Covered as Complete 2 Activities Must Complete and Self-Report All 4 to earn the Complete and self-report the 2 remaining wellness bucket activities in Tier 4 during this reporting time frame. Qualifying Activities Tier 2 and Tier 4 Wellness Buckets Bucket 1: Healthy U Preventive Health Complete/Self-Report 1 Activity in Either Tier 2 or Tier 4 Yearly wellness exam (also called your physical or preventive health exam) Yearly gynecological exam (females) including pap smear if appropriate Yearly screening mammogram (females age 40 and beyond) Yearly PSA blood test or DRE (digital rectal exam-males, age 50 and beyond) Yearly dental exam/cleaning Yearly vision exam Screening colonoscopy (age 50 and beyond) Establish yourself with a consistent medical home. Find a Primary Care Provider (PCP) that you can use for your routine medical care Bucket 2: Healthy U EDU Complete/Self-Report 1 Activity in Either Tier 2 or Tier 4 Attend a Healthy U Health chat or attend a wellness type class on your own, such as classes offered through Community Ed, Hospitals, Fitness Centers, ISU Extension, and other community offerings Participate in a community type event, (I.E. 5k, walk, bike, marathon, warrior dash, Healthiest State Walk, etc.) Volunteer Your Time (I.E. help with school/community gardens, coach a team, mentor, DMPS graduation walk, etc. on Unpaid Time), Create and/or Participate in a challenge you do in your own building amongst staff, Live Healthy Iowa team or a team sport, exercise classes, bowling league, etc. Complete any additional Challenge from Bucket 4 Do you have an idea of something you would like to count? If you do not see it here, please contact Healthy U! Bucket 3: Healthy U My Personal Adventure Complete/Self-Report 1 Activity in Either Tier 2 or Tier 4 My Goal, My Way! Create your own Personal Wellness Goal Minimum Length of 6 weeks. Create your own Nutrition goals, Weightloss goals, Emotional health goals, Quit tobacco, Use a pedometer, Fitbit, Physical Activity, etc. It s Your Goal, Your Choice, and All Your Way, YOU Are In Control!! Simply write it up using the downloadable form, located on the @DMPS Resource Page, under Healthy U or on the last page of this Guide. Here are just a few ideas/suggestions to get you thinking about YOUR PERSONAL ADVENTURE! Focus On You Health Coaching Program through Unity Point 1-515-241-5061 (Up to 4 sessions, minimum 2 sessions scheduled 3 weeks apart, FREE for all employees!) Medical Provider Office Health Coaching Program (Typically FREE from your medical office) Mental Health Coaching (stress, sleep, life satisfaction, life balance. etc) one option is to use the FREE DMPS Employee Assistance Program (EAP) through Magellan HealthCare 1-800-356-7089. and family members are eligible to receive 3 sessions per issue per year. Other options might be counseling services through your health insurance coverage, etc. Nutrition, Financial, Spiritual Counseling/Guidance, Tobacco Cessation, Diabetes, Fitness, Life Coaching, Weight Management (EXAMPLES Not all Inclusive ideas): Attend weightloss group, Profile by Sanford, Weightwatchers, Jenny Craig, TOPS, Bariatric Support, or programs through local fitness facilities) Online Health Coaching Through Wellmark WebMD Website IT CAN BE ANY GOAL YOU WANT!! No Cost, No pressure, Just YOU Taking Care Of YOU for the next 6 weeks!
Qualifying Activities Tier 2 and Tier 4 Wellness Buckets Bucket 4: Healthy U Challenges Complete/Self-Report 1 Challenge in Either Tier 2 or Tier 4 Challenges are 4-6 weeks in length and will be offered online through the Wellmark WebMD Wellness Center, or via the use of a paper tracker form. Complete and self-report a minimum of one challenge during the year to earn credit. Challenges will be related to physical, nutrition, stress, financial, sleep, emotional health, and etc. Be watching for more specific details on specific programs and registration via your building Wellness Champion, through DMPS Employee Email, or access on the calendar for Healthy U on the @DMPS Resource page. Healthy U Help and Support! If you feel you need extra help, you got it! Healthy U wants to make sure you have the extra help and support you need in completing your Healthy U Wellness activities! Each building this year will have a designated Wellness Champion to support you and keep you pumped up and excited about wellness! Your Wellness Champion will be sending out an email once the new school year begins so you will know who to reach out to for any Healthy U questions or issues! Your Wellness Champion is ready and willing to keep you on the Healthy U Track! You may also access a list of all district champions on the @DMPS Resource page under Healthy U. Confidentiality, Participation and Record Keeping! Confidentiality: Please remember ALL Healthy U program information is confidential! Your information is always protected and respected! Participation: Participation in any Healthy U Wellness activities and earning the incentive dollars is completely optional. You always have the choice to be as inactive or active as you want to be! Of course, Healthy U hopes you will complete and participate in all our activities, we encourage you to become, continue and stay the Heathiest YOU possible! Record Keeping: As part of our program integrity, Healthy U will continue to conduct random program audits. In addition to our self-reporting system on the Wellmark WebMD Wellness Center Rewards Page, we ask that you keep a manual file/record of your completed activities.
MY Personal Adventure Goal Form (Bucket 3) My Goal, My Way! Welcome to Bucket 3 MY PERSONAL ADVENTURE! Here is where you create your own Personal Wellness Goal! Remember it must be a Minimum of 6 weeks long. Your goal is just that your goal! Its personal, it s unique and you are in control of it! In this personal adventure you decide and write a goal that is specific to you! Here are just a few ideas/suggestions to get you thinking about what you may want to do for YOUR PERSONAL ADVENTURE: a nutrition goal, an emotional health goal, a weight goal or quit tobacco, maybe a daily step goal using your Fitbit or other tracking device, or etc. The sky is the limit, because it is Your Goal, Your Choice, and All Your Way YOU Are In Control!! A great start to writing a goal is using the SMART way! It makes your goal Specific, Measurable, Achievable, Realistic and Time sensitive. Read on below for a more in depth look at the SMART Process. Specific What is it exactly that I would like to achieve? Describe your goal in as much detail as possible! The more you define it the better it will be! Measurable How will I know when I ve got it? Put dates, times and places! This helps you know the process of your goal! It will help you stay on track, and you can see and feel your progress!! Achievable/Attainable This means that you can achieve your goal! It doesn t mean it will always be easy, instead it means you are creating something that is reasonable and yet pushes you to the end! Realistic/Relevant This means that this goal is really important to you and that it is something you would really, genuinely like to do and achieve. It helps you look at the possible and attainable! Timed You are giving your personal adventure a deadline! For this personal adventure it is a minimum of 6-weeks! You can keep going or create a new one or even add on to this one if you want!
