REQUEST FOR PROPOSALS COMMUNITY PROJECT FUNDING West Virginia University Extension Service, in collaboration with state and community partners will provide funding of $500 to $4,000 for community organizations in Barbour, Gilmer, and Pleasants counties. The purpose is to promote active, healthy environments and educational activities for young children (ages 2-5) and their families. If your organization is interested, please complete and submit the proposal below. These are one-time mini-grants. They are competitive, and will be objectively ranked by a selection committee. The total number of projects funded depends on the requested amounts and the strengths of the proposals. Eligible organizations must email the completed application by the deadline and they must comply with all guidelines and procedures. INSTRUCTIONS & APPLICATION Due August 1, 2016 GUIDELINES & PROCEDURES: One application per organization is permitted. An organization should be an entity with a tax ID number and the capacity to manage the funds and carry out the proposed project. The application must be complete and follow all instructions. Applications received after the deadline will not be considered. Requests may range from $500 to $4,000. Projects will be reviewed by a selection committee. They will be prioritized and funded based on the availability of funds. Projects will be given priority which show: 1. community/school collaboration 2. creativity 3. potential to effectively engage children (ages 2-5) and their families 4. sustainability (example: not one-time events) Funding decisions will be made and applicants notified by September 15, 2016. The period to complete the project is October 2016 to September 2017.
PLEASE NOTE: We reserve the right to request revisions to an application to improve the strategy and/or cost-effectiveness. If funded, you will be asked to complete required financial documents. Once these are received, a check payable to your organization will be sent to you by West Virginia University. You are expected to comply with best practices for financial management of funds. Documentation of all expenditures must be available for audit. Any unused funds must be returned to West Virginia University. Notify your WVU Extension Agent in writing/email if there is any change in circumstances that might affect your ability to carry out the project. Project team members may visit organizations/communities to learn about and see funded projects. You will be informed before any visit. The Final Project Report and at least five digital photos are due by email on October 15, 2017. PROJECT EXPECTATIONS: Have a clear and concise plan and stick to it. Propose a plan to improve the health of young children (ages 2-5) and their families through increased opportunities for physical activity and healthy eating. Examples: enhance areas for active play, community/school gardens, family engagement programming. Involve many community members and develop new partnerships. Be creative and make health fun. Plan ways to measure your project results. Use resources wisely.
