Caffiero Scholarship PT Rehab Science For many years, Al Caffiero has been one of the most prominent physical therapists in Western New York. Throughout his distinguished career, he has displayed a caring attitude, altruism, creativity, clinical insight, dedication to the profession and an inspiring degree of optimism. Consistent with his vision for the profession and his caring attitude toward students, he and his friends have established this scholarship. Candidates must have a history of community/professional service, financial need, professional promise and academic performance. Physical Therapy students who are in their first year apply; Awards are given to those first year students in their second year. SCHOLARSHIP NAME: J. Warren Perry Scholarship OT & PT SPHHP Honoring the first dean of the School of Health Related Professions, the late Dr. J. Warren Perry was the Dean of the School of Health Related Professions from 1966 until 1976 and his brother, the late Dr. Charles D. Perry, Professor Emeritus of Classics at the University of Alabama, have generously contributed funds to the University at Buffalo Foundation, Inc. in support of scholarships for undergraduate and graduate students in the UB Departments of Exercise & Nutrition Sciences (ENS) and Rehabilitation Science (RS). Candidates are evaluated on the basis of academic achievement and professional potential. Eligible students are either (1) undergraduate ES students who are in their senior sequence, or (2) current ENS or RS Varies: Amounts typically range from $1,000 $2,000 for each student. Competition for the scholarship is announced in April of each year. Shaffer Memorial Scholarship OT Rehab Science This scholarship is awarded in memory of Kathy Shaffer, a 1987 graduate of the UB occupational therapy department baccalaureate program and senior
therapist at Erie County Medical Center. Shaffer s contributions to UB included course instruction and fieldwork supervision of students. She is best remembered for her excellent clinical skills and mentoring relationships with students. SCHOLARSHIP NAME: Stonegraber Scholarship Fund OT & PT Rehab Science Established in memory of Eleanor C. and David P. Stonegraber. Mrs. Stonegraber was an occupational therapist in the Buffalo and Rochester areas and director of occupational therapy at the Rochester Psychiatric Center at the time of her retirement in 1959. The Stonegraber Scholarship provides partial funding for occupational therapy and physical therapy students entering their last year of study. To qualify for this scholarship, an applicant must not have previously received a Stonegraber Award and must: 1. Have on file in the Office of Financial Aid a current Financial Aid Form (FAFSA, http://www.fafsa.ed.gov ) at the time of application, AND 2. OT and PT students entering their last year of study Occupational and physical therapy students at UB. Amounts typically range from $500 to $2,000 Students interested in the Stonegraber Scholarship should file a financial statement with the Financial Aid Office during the fall semester. Will need to provide supplemental material with the application: 1. Academic/Professional Motivation, Goals & Accomplishments: 4 or fewer double spaced, typewritten pages, on which you provide a brief biographical sketch describing how you became interested in OT or PT, your professional goals, and any related accomplishments. Mussenden and the professional and personal values she espoused, the Maureen R.L. Mussenden Scholarship has been established to help support the education of students pursuing a graduate degree in one of the five UB Health Sciences Schools. Candidates must be currently enrolled or have been accepted into a graduate program in the School of Public Health and Health Professions. (List the other 4 schools?) Residents and Fellows are NOT eligible. Applicants must be first generation students (whose parents did not receive a college degree) and a priority will be given to women. The Scholarship will provide one third of the in state tuition in effect the year the application is made for up to four years. SCHOLARSHIP NAME: SPHHP Tuition Scholarships to Promote Student Diversity OT & PT SPHHP 1. How do students apply? 2. Is there an application for this? 3. What is the process? The School of Public Health and Health Professions (SPHHP) using a cost sharing arrangement with each department, offers tuition scholarships to students who have been admitted to graduate or professional study, who will pursue such study and be enrolled on a full time basis, and who will contribute to the diversity of the student body in the graduate or professional program in which they will be enrolled. Eligible students apply directly to their home department. The home department may submit up to two student nominees per year to the SPHHP Dean s office for award consideration. To be eligible for consideration, nominees for support must meet all of the following criteria: 1. Be a U.S. Citizen, Permanent Resident, or Non immigrant in legal status. Scholarship eligibility for International Students may also be dependent on the individual s visa status and any related terms or limitations. 2. Be accepted in a UB SPHHP graduate degree program or a professional degree program at the graduate level. 3. Have met all minimum entrance requirements for their home department. 4. Be able to satisfy the diversity criteria for the program as described below. Diversity Criteria: The scholarship tuition program is designed to assist SPHHP departments in the recruitment, enrollment and retention of students in doctoral and master s level programs who can demonstrate that they would contribute to the diversity of the student body, especially those who can demonstrate that they have overcome a disadvantage or other impediment to success in higher education. Such categorical circumstances may include academic, vocational, social, disability or economic impediments or disadvantaged status that students, as evidenced by their previous academic performance, have been able to overcome. Also relevant are other characteristics that constitute categorical underrepresentation in their particular graduate program such as gender or racial/ethnic status. Note that in awarding such tuition scholarships, full consideration is given to students who are of a race or ethnicity that is historically underrepresented in graduate and professional programs at UB; however, SPHHP does not make awards based solely on racial/ethnic status. Anyone accepted in a UB SPHHP graduate degree program or a professional degree program at the graduate level. Tuition Scholarship: The maximum number of tuition scholarships awarded annually is dependent on available funding. Scholarships awards are limited to a maximum of three years, but annual renewals are subject to annual review.
The submission should include a testimonial from the student that was either submitted as part of their application materials, or separately solicited, which contains statements supporting the basis for this diversity tuition scholarship. The program director or department Chair must write a brief letter, again stating how this student meets the diversity criteria as described above, and commits to providing scholarship funds that equal or exceed the award given by the Dean s office. The Dean or his designee will determine which student(s) will receive the SPHHP tuition scholarships. Fall semester applications must be received by the Dean s office by February 15, and decisions will be made by March 1. Spring semester applications must be received by the Dean s office by December 15, and decisions will be made by January 1.
