Guidelines for Completing Annual/Interim and Final Reports General Instructions Access the online reporting system for submitting progress reports via the link https://web.laregents.org/. Click on LOGAN from the main menu at the top of the page. This brings you to the LOGAN Login page. If the PI has not registered on LOGAN previously, click on Need an account? Create one and follow on- screen instructions to register. Once registered, the system will email a password to the provided address. If you are already registered on LOGAN, login by selecting your employing institution, e- mail address, and system assigned password. Next, locate Access BoRSF Project Report Module. A list of all the active contracts associated with the PI appears on the next screen. If your contract is missing from this list, email support@laregents.org with the PI s email address and the contract number(s). The PI clicks on the Project Reports button next to the contract number for which he/she is submitting or updating a report. A list of project reports for the selected contract number will be displayed. Click Edit and Submit Report link next to the report. Complete all forms according to the format listed below. Contact your Office of Research/Sponsored Programs if you need help. When the report is completed, click Submit. The PI will get a reply that the report has been transmitted successfully. This process transmits the report to your institution s Office of Research/Sponsored Programs. They in turn submit the report to the BoR Sponsored Programs staff for final approval. If the report is not acceptable (e.g., the report is incomplete or the correct format was not followed), BoR staff will contact the institution s Office of Research/Sponsored Programs and/or the PI during the summer. The report will then have to be revised and resubmitted. Subprogram: Enhancement (Traditional & Undergraduate) ENH Forms to be completed 1) Key Data Online Form a) A list of all current and former project personnel should be listed. 3) Personnel Narrative PDF upload a) For all key personnel and other staff listed under the personnel section, provide information about the types of contributions made and how these contributions are adequate to the project's requirements. 4) Activities and Findings a) Describe project activities undertaken in this reporting period; b) Describe and provide data supporting the outcomes of these activities; c) Provide the number(s) of undergraduate and/or graduate students affected and describe the opportunities for student participation and benefits to students provided by your project;

d) Provide numbers of faculty impacted by the project and describe faculty outcome(s) related to project activities e) Describe the overall impact of the project on the affected department(s) or unit(s); f) Describe community development and/or outreach activities your project has undertaken, including engagement of the PK- 12 community; and g) Describe any problems encountered during project implementation. h) Upload a single PDF document for this form. 5) Publications and Products a) List any tangible products (e.g., publications, presentations at meetings, patents/licenses, development of web pages, adoption of revised/new curricula, etc.) resulting from the project. For publications and presentations, provide complete bibliographic citations in the format appropriate to your discipline. 6) External Funding Details a) List any external awards generated by, related to, or resulting from project activities. Provide the title of the project, year(s) of award, funding entity, and total funds awarded. 7) Project Contributions a) Summarize the major contributions of the project to research and/or education and provide success stories that demonstrate project impact. 8) Project Revisions a) If applicable, provide explanations of any significant changes in the project work plan since implementation and the impact(s) of these changes on the project. 9) Appendices a) Attach any additional documentation here. Subprogram: RCS, PFund, RCS- 1 Year RCS, PFund, RCS- 1 Year Forms to be completed 1) Key Data Online Form a) A list of all current and former project personnel should be listed. 3) Personnel Narrative PDF upload

a) For all key personnel and other staff listed under the personnel section, provide information about the types of contributions made and how these contributions are adequate to the project's requirements. 4) Activities and Findings a) Describe activities undertaken during the life of the contract, project goals and benchmarks identified to measure success; b) Describe and provide data supporting the outcomes of these activities; c) Discuss progress made toward competitiveness for federal and/or other significant non- State funding; d) Provide numbers of faculty impacted by the project and describe faculty outcome(s) related to project activities; e) Provide the number(s) of undergraduate and/or graduate students affected and describe the opportunities for student participation and benefits to students provided by your project; f) Describe the project's contribution to economic development and diversification, particularly within Louisiana. Include details about any research commercialization and technology transfer activity, including patents, licenses, disclosures, start- up companies, surviving start- ups, industry partnerships, etc.; g) If applicable, describe any variances from the project work plan as included in the original project contract; and h) If applicable, describe any problems encountered during project implementation. i) Upload a single PDF document for this form. 5) Publications and Products a) List any tangible products (e.g., publications, presentations at meetings, patents/licenses, development of web pages, adoption of revised/new curricula, etc.) resulting from the project.
