Application Procedure We do not accept bookings by telephone. Applications for places on study days must be submitted using the Tissue Viability Application Form provided at the back of this schedule and sent to the Tissue Viability Office by either fax (01223 723002), email cpm-tr.tissueviability@nhs.net or post. Incomplete forms or bookings made without a form will not be accepted. Papworth Hospital staff booking on the mandatory Pressure Ulcer day should fax their form to Education dept at the hospital 01480 364730. Applicants will be notified by email approximately one month before the study session to inform you if your application has been successful. Please ensure that your email address is clearly stated this will be used to notify you when you are allocated a place. We reserve the right to refuse applications if the number of applications exceeds the room capacity. Nursing and Care homes may apply for up to 2 places per study day, for each home. Cancellations & Non-Attendance Candidates who are absent on the day without prior notification will be sent a charge of 80 to their team s budget. This is to ensure that all places are filled, reducing any waiting lists that we may have. Refreshments Tea and coffee will be provided at a charge. Lunch will not be provided at any of the study sessions, so please remember to bring your own.
