The Metro 4-H Awards & Recognition Brochure is published annually in May for use by 4-H leaders, members, and parents enrolled in Clackamas, Washington, and Multnomah counties. Materials contained in this brochure may be copied and distributed as appropriate for members. Additional copies may be requested from the Extension office. NO DROP BOX or other drop-off place is available in Multnomah County, members may use another county s drop box. 4-H Member Awards & Recognition Calendar Leaders: Duplicate this information for your 4-H members now so they will know what is required for regional, state, and national 4-H recognition and awards. September 2017 How To Prepare A Competitive Record Book... for members, leaders, & parents 9 Saturday Clackamas County Extension annex 10:00 am to 12:00 noon 9 Saturday Washington County Fairgrounds (tentative) 10:00 am to 12:00 noon Records Evaluator Training... for anyone interested in being a Record Book Evaluator 9 Saturday Washington County Fairgrounds (tentative) 12:30 to 2:30 pm 16 Saturday Clackamas County Extension annex 10:00 am to 12:00 noon 26 Tuesday Clackamas County Extension annex 7:00 to 9:00 pm RSVP to your county Extension office is required. If no one RSVPs, clinics will be cancelled. Other times are available by appointment. All persons evaluating 4-H records are REQUIRED to attend a one-time METRO training session. Updates on Evaluation Day are provided annually as needed. If you have attended a METRO records evaluator training, and have evaluated records recently, you do not have to attend the 2017 training. However, there were changes to the permanent record in 2015, so experienced evaluators are strongly encouraged to come for refresher training. October 2017 2 Records Due, County Extension offices, 5:00 pm. In 2017 record books are due in the Extension office by 5:00 pm on Monday, October 2, but will be accepted in drop boxes, if available, before 8:00 am on October 3. Why are Record Books Important? A member s record book is the key to many opportunities and awards. Certificates of recognition will be given for record keeping skills based on how well the member has communicated information about his/her 4-H work. REMEMBER, 4-H records help members to: 1. Establish goals 2. Measure their results (financial, personal) 3. Record a personal history How do I find the Record Forms? Use only official record forms; do not make up your own. For best results, use Mozilla Firefox for your browser. My 4-H Resume can be found online at http://oregon.4h.oregonstate.edu/info/forms/myresume. Print the regular.pdf version or download one of the fillable versions. You can choose to use the Word version or the Fillable.PDF version if you want to type your Resume. IMPORTANT: Follow the instructions on the website to save and use fillable forms. Once you fill up the first page of each section, you can download Individual Pages of each section from the state website. All 14 sections must be included and in correct numerical order even if there are no entries. Many project records are also available online. Go to http://oregon.4h.oregonstate.edu/projects/home and click through to your project area. Look for Project Resources then files with the word Record in their titles. Print each record that you need. Most project records are now available as a Fillable.PDF. Be sure to save the file to your computer. What Other Materials/Forms do I need? Cover: use a flat ACCO cover; A green Member s Record cover may be purchased at Extension office for a small fee or you can find your own appropriate folder. Each member must complete the Metro 4-H Awards & Recognition 2017 Application, found on the last page of this brochure and online at http://extension.oregonstate.edu/ clackamas/4h/recordbooks. 4-H Story or Notes, use your own paper or My 4-H Notes form, 4-H 039R, available from the Extension office. Tab dividers; you can find these in any office supply store. Optional: Advancement program guides, available at the Extension office or on the state website in Project Resources. Advancement completion certificates may be available at your Extension office. How to Submit Your Records for Evaluation 1. Assemble your records in the order found on page 4. 2. Label your front cover in the upper right corner. You can use either a stick-on label or staple on a 3x5 index card. Your label should list your: Name Birthdate month, day, and year County 3. Submit records to your Extension office by October 2, 2017. Records are handled in a professional manner and are reviewed only by the evaluators, Awards & Recognition Committee members, and Extension office personnel. 2017 Graduates are encouraged to turn in Record Books! Contact your local Extension office if you need help finding or using computerized records. Page 3

Assemble Your Records in this Order Requirements & Options Dividers with secure, labeled tabs are required. Insert where shown. Use a separate divider for the permanent record (My 4-H Resume), the 4-H story, each project record, photos, and letters of recommendation (if applicable). 1. COVER: Label your front cover in the upper right corner. You can use either a stick-on label or staple on a 3x5 index card. Your label should list your Name, Birthdate, and County. Use a flat cover with the records well secured. DO NOT use a 3-ring binder. A good type of folder to use is either an Acco-type record folder/cover or the official 4-H Member s Record folders/covers available at the Extension office or from www.4-hmall.org for a nominal fee. 2. 2017 APPLICATION, METRO 4-H AWARDS & RECOGNITION: Complete the 2017 application found on the last page of this brochure. A new, current application page is required each year. This is the first page inside your records after the cover. DIVIDER: My 4-H Resume 3. MY 4-H RESUME: This record is to be current and is meant to include all 4-H and community activities in which you have participated up to the date of turning in your records. All categories, 1-14, must be included even if there are no entries. Use additional sheets if necessary. For more information on the Resume, see page 3. DIVIDER: My 4-H Story 4. MY 4-H STORY or NOTES (maximum six pages): A new story is required each year. It should include your 4-H experience over the past year. This can be in diary, journal or story form using either the official My 4-H Notes form, 4-H 039R, or your own paper. If your own paper is used, title the page My 4-H Story. May be typed or hand-written. Must be on one side of the page only, double-spaced, with a 1 ½ inch margin on the left side and a 1 inch margin on the right, top, and bottom of the page. If typed, use Times New Roman, Arial, or a similar font and 12 point type. DIVIDER(S): Examples: Horse Foods Photography Each project needs its own divider 5. PROJECT RECORDS: One project record form for every project in which you are enrolled and have participated in during the year just completed. New forms are required each year (for exceptions, see page 6). Remember that Leadership is a project and all leadership can be reported on 4-H Youth Leader Record (MAR 0408). 6. ADVANCEMENT PROGRAM RECORDS, ADVANCEMENT CERTIFICATES, AND PHOTOGRAPHY PROJECT SAMPLES: Optional, but recommended. Place BEHIND respective project records without a divider. Participants in the photography project may include up to ten 8 ½ x 11 inch pages, one side only, of sample pictures taken in the 4-H photography project. Include these pictures behind your 4-H photography record. DIVIDER: If needed Photos 7. PHOTOS and NEWS CLIPPINGS: (Optional) You are encouraged to mount photos and news clippings on one side of up to six 8 ½ x 11 inch pages. Mount pictures on stiffer paper and including a description of what is taking place in each photo is encouraged. Photocopies or color print-outs are acceptable. Plastic protective sheets for photo pages are optional. Ribbons or other scrapbook items are not to be included. DIVIDER: If needed: Letters of Recommendation 8. LETTERS OF RECOMMENDATION: Two required if applying for the Citizenship Award. Page 4

What Record Forms Do I Need This is a common question. This chart should help you figure out what project record forms you need to include. Remember to fill out new project records each year for each project in which you are enrolled. Records are available at the Extension office or online at http://extension.oregonstate.edu/clackamas/4h/recordbooks (see page 3). Criteria for Awarding Seals All members will receive a recognition certificate and seal for achievement in record keeping skill. Members will be recognized at their county awards night program. Gold Seal - assembled in correct order and records appear complete, neat, clean, as legible as possible, and dividers used. Criteria for Awards Tips for preparing an awardwinning record book Records may be done with pencil, pen, or computer. Whichever method you choose, records should be neat and easy to read. Page protectors may only be used for photo pages. COVER: Flat, clean, labeled cover (no 3 ring binders). Silver Seal - assembled in close to correct order and when an attempt is made to complete the records in each area. Bronze Seal - awarded for records turned in and most sections represented. Criteria for Awarding Medals Members are eligible if they were an Intermediate or Senior in the 2016-17 4-H year. 1. Members may receive only one project medal per year. 2. If merited by outstanding accomplishment, members may receive the same medal once as an Intermediate and once as a Senior. 3. Member must be enrolled in the project area, or show expertise in one of the general areas, such as Achievement, Citizenship, Judging, Leadership, etc. 4. Record books are evaluated according to medal selection criteria: 4-H projects and activities:...50% 4-H leadership:...25% Citizenship/Community Service:...25% 5. Committee will look for growth and participation. 6. Members must participate in both Fashion Revue judging and public show to qualify for the Fashion Revue Medal. Page 6 APPLICATION PAGE: Be sure to use the 2017 version Fill out completely All signatures required MY 4-H RESUME: MUST USE CORRECT STATE FORM FOR ALL ENTRIES SINCE 2015. If previous years have not been transferred, place your old Permanent Records behind the Resume. Include items which are in correct categories, upto-date, and show growth and expansion in participation - something new each year. 4-H STORY OR NOTES: Talks about the year just completed (2016-17 4-H year). Reflects specific 4-H experiences in your project areas, citizenship, and leadership. Can be in diary, journal, or story form. PROJECT RECORDS: A record for each project (including leadership) for the year just completed (2016-17 4-H year) Record is accurate in columns and figures, complete, up-to-date, reflects clearly stated goals, and records are dated. If there is a space for a signature, it must be signed. OPTIONAL: Judges enjoy seeing advancements, photos of you and your project, and newspaper clippings in their appropriate places in your book. All medals are subject to availability, those marked with * are in very limited supply. Special Awards Available Through Record Books State awards are subject to change State 4-H Citizenship Award Nominee 4-H Age 12 and up Record Book Evaluation Required State 4-H Communication Award Nominee Separate application required due Feb. 20 State 4-H Leadership Award Nominee These state level awards will be presented to an Intermediate and a Senior 4-H member who demonstrates outstanding contribution in the areas of Citizenship, Communication, or Leadership. County nomination will be based on record books submitted for the October evaluation. To be considered at the state level, nominees must submit an application by February 20 to the county office. Members can apply for only one award each year. The State 4-H Recognition Committee will review the county nominations and select the winner and honorable mention winners. Statewide 4-H Ambassador Team Age 14-18 Training Application Required The State Ambassador team is made up of 10-20 outstanding 4-H members and is run out of the state 4-H office. You must be at least 14 to apply to be a State Ambassador. Ambassador team members are selected at Summer Conference and can be reappointed each year until they graduate. Although it is an honor to be selected as a state ambassador, this is a working position. Ambassadors are expected to attend and actively participate in at least half of all yearly activities. Selection for the Statewide Team is based on the application, participation in the training, and an interview conducted during Summer Conference. The Application includes the member s My 4-H Resume, but submitting your records in October is not a requirement to participate in this opportunity. Ambassador Training applications are due in May and are available at your county Extension office or online: http://oregon.4h.oregonstate.edu/programs/ambassadors. 4-H Metropolitan Citizenship Award 2+ Years 4-H Required 2 Letters of Recommendation Required Age 15 and up Record Book Evaluation Required The 4-H Metropolitan Citizenship Award will be presented to a maximum of two youth in each county who exhibit the qualities of dependability, citizenship/community service, leadership and patriotism. Must have 2.0 GPA or above. Two letters of recommendation made by adults must be included in the back of your record book behind a tab marked LETTERS OF RECOMENDATION. Letters should address the member s dependability, motivation, attitudes and citizenship activities. These may not be made by family members. Youth Leadership Award Age 15-18 Record Book Evaluation Required This award recognizes up to two youth from each county who have demonstrated emerging leadership. Candidates should demonstrate leadership that involved risk and stretching; should demonstrate a different kind of leadership than they did as an intermediate member. Selection for a trip to Congress is an exceptional honor based on the member s achievement in 4-H projects, leadership, communications, citizenship and community service as presented in the State 4-H Resume. College Scholarships Scholarships, subject to funding, are available to college-bound 4-H members, through Record Books and through separate applications. This information is current as of April 2017. Washington County Scholarships Record Book Evaluation required Due October 2 2018 Graduates Separate application required due April 2 The Megargel 4-H Scholarship and the Washington County 4-H Record Book Scholarships are offered to high school seniors who have completed two years of active membership as Washington County 4-H members and are currently active Washington County 4-H members. Applicants for the Record Book Scholarship must have turned in their record books by the October deadline of their senior year in high school or as a home schooler. By doing so, they will qualify to receive the application information on or before March 1. Applicants must then submit their My 4-H Resume (updated and complete through March 31) and other application documents by April 1 of that year or by the next business day if that is on a weekend. The Megargel 4-H Scholarship is available to all high school seniors by separate application. The requirements for this scholarship will be listed in the county newsletter in January or February. Applications are also due April 1 or the next business day following April 1. Preference will be given to those with more than one project area. The recipients must be enrolling in an accredited college, university, community college, technical school, or certification program by fall of the current or following year. Number of scholarships and amounts are determined annually and are dependent upon funding raised through the Walk-athon and endowment funds. Clackamas County Scholarships Record Book Evaluation Required Due October 2 2018 graduates Clackamas County scholarships are provided by project advisories and private donors. Amounts vary. They are awarded based on the content of record books turned in October 2. Some scholarships are project-specific, while others are open to all qualifying 4-Hers. Members must be in their last year of high school to apply. Winners are announced at the Awards and Recognition event on November 18. State 4-H Scholarships Separate application required 2018 graduates Due to county Extension office February 20 Application for the State 4-H Scholarships My 4-H Resume, My 4-H Story, a copy of high school grades, and an official copy of SAT or ACT scores. Detailed instructions are provided on the scholarship application form. Applications are submitted to the applicant s County Extension office by the due date established by the County. Counties then forward approved scholarship applications with a county nomination form to the State 4-H office by March 1. Please be sure you meet eligibility criteria before selecting scholarships you are applying for. Fairs 4-H Scholarships Separate application required 2018 graduates Due to county Extension office February 20 These scholarships are open to 4-H members who have exhibited at county and/or state fairs. Applications must include school transcripts, and two reference forms. They must be accompanied by an essay that explains the benefits of fair participation for youth and why such opportunities should continue in the future, as well as outlining your educational and career plans. Also required is a summary of your leadership and exhibits at pre-fairs, county fair, and state fair. Hofer Scholarship Separate application required Students enrolling or enrolled at OSU Due to State 4-H office March 1 Applicant must have been enrolled in a 4-H program for a minimum of five years. There is no financial need requirement. Recipient must be a female student majoring, or intending to major in Merchandising Management in the Department of Design and Human Environment (DHE) at OSU.
