2018 EXHIBITOR PROSPECTUS The Seventh Annual Meeting of the American College of Wound Healing and Tissue Repair This unique meeting will address the need for creating a formal wound care specialty and will also focus on high level issues regarding scientific breakthroughs, policy and regulatory issues facing the field and technological advances that will change the landscape of the wound care industry.

WELCOME We invite you to join us at the Seventh Annual Meeting of the American College of Wound Healing and Tissue Repair (ACWH) on October 4-6, 2018 at the Hilton Chicago, Illinois. The College is a non-profit organization committed to advancing the field of wound care through education, research, and advocacy, with the goal of designating wound care as a board-certified medical specialty. During this 2 1/2 day meeting, over 500 leaders will gather from multidisciplinary fields in wound care, including physicians, podiatrists, nurses, physical therapists, and researchers. The program features live OR and clinical cases, panel discussions, and symposia from more than 30 faculty members. Friday and Saturday's agenda will focus on clinical topics, live demonstrations, systems biology, health policy, and research and innovation. EXHIBITOR POLICIES COMPLIANCE By signing the Exhibit Application your organization understands and agrees to abide by all outline regulations outlined. The signer of this agreement also agrees to share the regulations with your organization s representative(s) who will attend the conference and will be responsible for the conduct of your attendees. All regulations will be enforced without exception. Any violation will be addressed by ICM, which reserves the right to dismiss exhibitors without refund or appeal for redress. All exhibitors will abide by the ACCME Standards of Commercial Support outside the Exhibitor area. Arrangements for commercial exhibits or advertisements will not be a condition of commercial support. SPACE ASSIGNMENT Exhibit space is limited and assigned on a first-come, first-served basis. To ensure availability, please reserve your exhibit space as soon as possible. Exhibitors will be notified of space assignments the week of July 10, 2018. ACWH reserves the right to reassign space if it is deemed necessary. No subletting or sharing of booth space is permitted. BADGE POLICY All company representatives must be registered for the conference. After the Exhibitor Space Application is received, your company contact person will receive a link to register all company representatives. A registration is required for all representatives who will be in attendance. Onsite, badges will be given and are required at all times during the conference for admission into the Exhibit and Education Ballrooms. Registrations/badges beyond your limit may be purchased online for $350 each. HOLD HARMLESS CLAUSE The exhibitor assumes the entire responsibilty and liability for losses, damages, and claims arising out of exhibitor's activities on the Hotel premises and will indemnify, defend, and hold harmless the Hotel, its owner, and its management company, as well as thier resective agents, servents, and employees from any and all such losses, damages, and claims. PAYMENT Final exhibit payment must be received by November 1, 2018, or the exhibit space will be reassigned and the exhibitor will forfeit all deposits paid. Payments received after October 16, 2018 will incur late fees: within 30 days - 2%, 60 days - 5%, and after 60 days - 10%. CANCELLATION POLICY Requests for cancellation of reserved exhibit space must be made in writing to brandy@acwound.org. Refunds less a 15% administrative fee will be granted for requests received on or before October 5, 2017. After this date, refunds for reserved space will not be granted. Page 3

EXHIBITOR POLICIES INSTALLATION For safety and security reasons, only your representative(s) directly responsible for booth installation is/ are permitted in the exhibit hall during installation. All exhibits, back walls, and decorations must be limited to 8 in height and not extend more than 3 from the back wall. Exhibits should not project beyond the allotted space nor obstruct the view of or interfere with traffic to other exhibits. Exhibits cannot block aisles or fire exits. Exhibit installation must be completed during the alloted hours. If an exhibit or materials fail to arrive in time for installation during these hours, your organization is still responsible for the exhibit fee. No exhibit may be installed after the hall opens. GENERAL CONDUCT Exhibit representatives must not deface the walls or floors of the building, the booths, or the equipment in the booths. Any and all damages, losses, expenses, and/or costs (including but not limited to attorney s fees) resulting from failure to observe these rules shall be payable by the exhibitor. Exhibitors or their representatives, who fail to observe these rules of general conduct or who conduct themselves in an unprofessional or unethical manner, may be dismissed without refund or appeal for redress. PRODUCT DISPLAY Exhibitors may display technologies, programs, products, or services only in their assigned booth space. The distribution of all medication samples (i.e., anything that has a potential therapeutic effect) is not allowed. INVESTIGATIVE PRODUCTS To comply with the Food and Drug Administration s Guidelines on Notices of Availability, any exhibits showcasing off-label or unapproved uses of drugs, devices, technologies, programs, products, or services must disclose that fact. EDUCATIONAL MATERIAL Distribution of educational materials by exhibitors or their representatives is limited to the booth space in the exhibit hall (not in conference meeting rooms). INDUSTRY MEETING SPACE Social functions or meetings may be held by exhibitors during the ACWH conference as long as it does not conflict with the conference education or symposia events. Meeting space is available at the conference hotel on an approved and first-come, first-served basis. To request space, complete and submit the Meeting Room Request form that is included in this prospectus. Only approved 2018 ACWH exhibitors will be allowed to utilize the meeting space and will be charged an additional $750 for use of the meeting room. Approval notification and room assignment will be sent shortly after the request is made. Page 4
