Page 1 of 6 1.0 PURPOSE This procedure provides requirements for the protection of eyes while working at Westlake Vinyls Geismar. 2.0 SCOPE This procedure also defines our prescription safety eyewear policy. The procedure applies to all Westlake employees and selected nested contractors. 3.0 DEFINITIONS 3.1 Selected Nested Contractors As it pertains to this procedure, the selected nested contractors are Turner and Excel. 3.2 Safety glasses Safety eyewear that meets the ANSI Z 87.1-2015. 3.3 Side shields Protectors that may be, but are not always permanently attached to eyewear to provide side protection Side shields must meet ANSI 87.1-2015 standards. 3.4 Stocked glasses Nonprescription safety glasses meeting ANSI Z87.1-2015 standards that are readily available to employees through our warehouse. 4.0 RESPONSIBILITIES 4.1 HSE Department 4.1.1 Review and revise this procedure as required by regulatory and site review requirements 4.1.2 Identify and approve acceptable safety eyewear for the site 4.1.3 Train site employees in eye safety 4.1.4 Conduct hazard assessment for eye protection as part of our PPE Hazard Assessment

Page 2 of 6 4.2 Purchasing 4.2.1 Procuring and stocking all approved safety eyewear 4.2.2 Distribute safety eyewear to the site 4.3 Westlake Employees and Selected Nested Contract Employees 4.3.1 Wear the appropriate safety eyewear when required by: A. Any safety procedure B. Area signs or designated areas C. Job safety Analysis D. Permit E. Safe Operating Procedure F. Any task that creates a hazard to the eye 5.0 PROCEDURE 5.1 Safety glasses with side shields are required anytime we are within the plant fence line. 5.1.1 Exceptions: When riding in closed cabs Control rooms and/or offices During shift change; going to and leaving your assigned work area 5.2 Contact lenses may be worn in conjunction with nonprescription safety glasses. 5.3 Monogoggles 5.3.1 Monogoggles are required: When required by the permit In designated monogoggle areas (These areas may be designated by signs, barricaded or colored curbs). When required by the JSA When the hazard for a chemical splash is present. When the hazard for flying debris surpasses what protection is given by safety glasses alone NOTE: Spoggles can never be used in lieu of monogoggles.

Page 3 of 6 5.4 Faceshields 5.4.1 Face shields are required: When grinding When there is a chemical splash hazard for the facial area Any other task when safety glasses with side shields or goggles do not provide the protection needed to mitigate the hazards NOTE: Face shields can never be worn as the primary (sole) eye protection. It must always be worn in conjunction with either safety glasses with side shields or nonogoggles. 5.5 Certain areas of the plant require special eye protection. When entering these areas, chemical goggles must be worn. Warning signs will designate these areas. Lens color for safety glasses also applies to chemical goggles. 5.6 Westlake employees that do not require prescription eyewear will be provided with non-precription eyewear. 5.7 Westlake employees will be involved in the selection process of nonprescription safety eyewear. 5.8 Prescription Safety Wear 5.9 Prescription safety glasses shall be furnish to Westlake emplyees and selected nested contractors. 5.10 To obtain prescription safety eye wear: 5.10.1 The employee must present a prescription (must not be older than 2 years) to HSE Department. This cost is the responsibility of the employee. 5.10.2 HSE department gives the employee a voucher to procure his/her prescription safety glasses. 5.10.3 The voucher may be redeemed: A. River Parish 10900 Hwy 3125, Suite F, Lutcher, LA 5.11 The employee will select fames from an approved array of choices identified by the HSE department.

Page 4 of 6 5.12 Prescription eyewear inserts for full-face respirators will be made available to employees upon request. Employees may wear their contact lenses when using fullface respirators. 5.13 Employees in the prescription eye wear program are eligible for one pair of prescription safety glasses per calendar year. 5.14 The following feature will be covered by Westlake Vinyls Geismar: 2 pair side shields if not part of the design of the safety eyewear (detachable or permanent) Transition lenses Progressive bifocals/trifocals if they are prescribed by the doctor UV and scratch resistant coating Prescription safety glasses with integrated side shields 5.15 Any other features added to the prescription safety eyewear will be the responsibility of the employee. 5.16 The employee has the option of clear, tinted or transition. 5.17 If an employee damages his/her prescription safety glasses during the year, he/she must present the glasses to the HSE department to obtain an additional pair within the calendar year. There will be no cost to the employee. 6.0 TRAINING 6.1 Initial training for Westlake employees will be conducted and documented through Westlake Vinyls Geismar s MOC process. 6.2 Retraining will occur when revisions are made or employees show deficiencies in eye protection. 7.0 RECORDKEEPING 7.1 Presecriptions will be added to employee s medical records. 8.0 PROCEDURE REVIEWS 8.1 This procedure will be reviewed/revised every 5 years from the last revision. 9.0 REFERENCES
