Box 3472 Quartz Hill, California 93586-0472 SCHOLARSHIP APPLICATION The St. Leonard Murialdo Council Knights of Columbus Scholarship Program recognizes and rewards outstanding student members of the local community. The Scholarship Committee seeks to identify those students, 21 years of age or younger who will attend or are currently attending high school or college, and who in addition to excelling in school have demonstrated good character and leadership through participation in extracurricular and community activities. Winners will be notified by mail, email, or by phone. The grant is payable directly to the applicant and their family to offset costs incurred while attending either private or public high school or college. The scholarship will be granted prior to the end of the current school year. For the 2015-2016 school year, the Council will award two scholarships in the amount of at least $500 each; one for the 13-17 year age group and one for the 18-21 year age group. ELIGIBILITY CRITERIA The Scholarship Committee will consider five specific areas for judging applicants: 1. Practicing Catholic. Specifics are on the attached page. Please respond to each question. Applicants are encouraged to obtain recommendation letters from their parish pastor, priest, or deacon. 2. Knights of Columbus affiliation is considered but not required. Respond to the questions on the attached page if appropriate. 3. School and community involvement is important. This can indicate character strength and leadership. List the areas of involvement on the application. Character reference letters from responsible adults (i.e. school counselor, teacher, coach, employer, etc.) will be considered. Applicants shall use the attached Recommendation Form for this purpose. 4. Demonstrated financial need shall be taken into consideration, reviewable on an individual basis. The application outlines this factor. 5. School Grades and writing communications skills. The applicant will submit a copy of their current school grades and a one to two-page essay addressing the topic identified on the attached application. Essays will be judged on content and quality of presentation. See attached sample scoring sheet used in judging application package.
