The Northern Adelaide Local Health Network (LHN) provides care for around 341,000 people living in the northern metropolitan area of Adelaide as well as providing a number of state-wide services, and services to those in regional areas. More than 3,000 skilled staff provide high quality patient care, education, research and health promoting services. The Northern Adelaide Local Health Network (NALHN) provides a range of acute and sub acute health services for people of all ages and covers 16 Statistical Local Areas and four Local Government Areas (one of which crosses the Central Adelaide Local Health Network) and includes the following: Lyell McEwin Hospital(LMH) Modbury Hospital (ModH) Ambulatory and Primary Health Care (including GP Plus Health Care Centres and Super Clinics) Sub-Acute Mental Health Services (under the governance of the Adelaide Metro Mental Health Directorate) Ambulatory and Primary Health Care delivers a range of primary health care services across the northern metropolitan area of Adelaide, with a focus on providing preventive and health promoting programs in the community, and transition and hospital substitution and avoidance programs targeted at chronic disease and frail aged. Clinical leadership of care systems is central to the current national and state wide health reforms. NALHN care delivery is configured within clinical divisions that are patient focused, clinically led groupings of services. Clinical Divisions are responsible for managing service delivery activities across NALHN campuses and units, bringing together empowered experts to directly make relevant decisions. SUMMARY OF THE BROAD PURPOSE OF THE POSITION and its responsibilities/duties LMH will undertake a twelve months trial of an After Hours Senior Clinical Cover model of care. The After Hours Senior Clinical Nursing Lead is part of the After Hours Senior Clinical Cover (AHSCC) Team. The role will provide nursing leadership, clinical care coordination, and support for patients and staff rostered afterhours. The role involves being a clinical resource person, assisting ward based clinical staff, responding to requests for higher level clinical assistance and to provide clinical leadership and mentorship after hours, rostered over 7 nights. The RN3 After Hours Senior Clinical Nursing Lead will be part of a small team who will assist hospital staff to manage and facilitate patient care, ensuring more complex clinical issues are dealt with in a timely manner to prevent delays Employees classified at this level provide clinical nursing/midwifery expertise for specified individual patients/ clients and/or groups and/or patient/ client populations, and may work in a variety of clinical settings. Work at this level is undertaken by employees with a least 3 years post registration experience. Employees in this role accept accountability for the outcomes of nursing/midwifery practices for the specific client group and for addressing inconsistencies between practice and policy. Various practice models may be used to enact this role, including but not limited to: - Primarily providing direct expert nursing/midwifery care for an individual or group of patients/ clients; - Providing clinical leadership to nurses/midwives; - Coordination and leadership of projects and/or programs that contribute clinical expertise to improve patient/ client service outcomes. Includes but is not limited to: Working in collaboration with the After Hours Senior Clinical Medical Lead to communicate and respond effectively to clinical demands across the hospital; Providing clinical leadership, support and advice to the after hours hospital staff to work as a cohesive team, through sound communication processes and role modelling. The RN 3 After Hours Clinical Nursing lead maintains high standards of patient care by creating an environment conducive to learning, establishing a team approach, setting and meeting the standards for nursing practice and standards relating to the manner in which staff interact with others; Providing senior clinical support, education, supervision, mentoring and guidance to nursing staff; Increasing and maintaining staff knowledge and skills relating to the clinical care of designated patient groups through the provision of just-in-time and scheduled education and access to appropriate and contemporary learning resources; Supporting and leading after hours clinical staff to enable early identification of scheduled education by progressing patient care and providing clinical intervention as required; Supporting ward staff to effectively communicate patient concerns through use of ISBAR; Provide education on correct utilization and understanding of current documentation, such as RDR charts, Advanced care Directives, 7 Step Pathway etc; Supporting decision making in advanced clinical care or when appropriate contact the home team during the after-hours shift for specialist clinical advice in collaboration with the After Hours Clinical Medical Lead; Assisting in the safe management and prioritisation of the AHSCC team s clinical workload including effective management of patient s plans thorough assessment and addressing individuals needs for emergency care etc; and Providing transformational leadership and acts as an expert nursing consultant throughout the Hospital to nursing staff and other health professionals during the after-hours period. REPORTING/WORKING RELATIONSHIPS (to whom the person reports, staff for whom the person is responsible, and other significant connections and working relationships within the organisation) The After Hours Senior Clinical Nursing Lead: Works collaboratively with the After Hours Senior Medical Lead Professionally Reports to the Director of Nursing & Site Manager (DoN&SM) Modbury Hospital Dual operational reporting to DoN&SM and Executive Lead for AHSCC Maintains close collaborative working relationships with after-hours medical and nursing staff across the LMH Maintains close relationships with all level 3 Nurses and/or Midwives, particularly after hours staff Provides direct line support and maintains cooperative working relationships within all members of the health care team. Supporting and encouraging staff to identify and implement best practice/evidence based patient care; and Enhancing skills of other after-hours hospital staff by delivering education, both ad hoc and planned. 4.2 Provides leadership to and facilitates professional development of Nurses and/or Midwives within LMH by; Leading the nursing and or midwifery team within the professional practice framework established by the Director of Nursing and/or Midwifery; Developing and maintaining a learning environment, contributing clinical expertise for staff development, individual capability development and performance management; Maintaining productive working relationships and managing conflict resolution; Contributing specific expertise to monitoring and evaluative research activities in order to improve nursing/midwifery practice and service delivery; Undertaking the nursing/midwifery care role with a significant degree of independent clinical decision making in the area of personal expertise; Contribute to the development and sustainability of nursing/midwifery skills for the needs of the specific population group using systems of resource and standards promulgation. 4.3 Contributes to the achievement of nursing and/or midwifery best practice and where relevant facilitates the development and application of relevant nursing and/or midwifery.
