ABOUT UNITED IN EXCELLENCE United In Excellence is a national network of Joliet Township High Schools alumni and community members. We work together to fundraise and advocate for student and community programs community in our hometown of Joliet, IL. Our organization is named after an afterschool program, United In Excellence ( UIE ). UIE was a Pan Hellenic ( Greek-style ) step-team that served over 100 students between the years of 1996 2006. The program was created by five, female students who practiced in parking lots and church basements on the east-side of Joliet. AVAILABLE SCHOLARSHIP OPPORTUNITIES WARREN C. & RITA H. DORRIS SCHOLARSHIP for Promising Community Leaders United In Excellence seeks to support students who have demonstrated leadership in their school and the community through a scholarship named after long-standing community leaders, Warren C. & Rita H. Dorris. We recognize and appreciate their continued efforts to serve and support the people of Joliet. In addition to their successful careers in business and banking, Warren and Rita have been committed to being change-makers through politics, community organizing, faith-based initiatives, and mentorship. AWARD AMOUNT United In Excellence Fund, in partnership with the JTHS Foundation, is pleased to announce that it will award one (1), $1,000.00 Dorris Scholarship to a student at Joliet Township High Schools, District #204 in the spring of 2017. The awardee will also receive ongoing support from our national network of over fifty UIE alumni, donors and community members. These benefits include mentorship and networking opportunities, care packages, a trunk party, and ongoing support from United In Excellence. STUDENT CRITERIA - A senior-level student attending Joliet Township High Schools, District #204-3.25 GPA at the time of application - Demonstrated leadership in an out-of-school time program (after-school, church, employment, community organization, etc.) - Proof of enrollment in a post-secondary institution - Obtain one (1) letter of recommendation from an adult supervisor from an out-ofschool time program - Complete a 500-1,000 word essay based on the following prompt: Described the importance of leadership and community service and how you ve demonstrated these characteristics." Fine & Performing Arts Scholarship for Students of Color pursuing Theatre, Music, Dance or Visual Arts United In Excellence seeks to support students of color who are pursuing fine and performing arts programs in postsecondary institutions. This scholarship will foster creativity, innovation, and entrepreneurship for students who are largely underrepresented in fine and performing arts programs at colleges and universities across the nation. Further, we believe that continued support from our network of alumni will propel our scholars to become creative leaders and executives in the arts. 3

AWARD AMOUNT United In Excellence Fund, in partnership with the JTHS Foundation, is pleased to announce that it will award two (2), $500.00 Fine & Performing Arts Scholarships to students at Joliet Township High Schools, District #204 in the spring of 2017. One student will receive the award at JTHS Central Campus and another at JTHS West Campus. Awardees will also receive ongoing support from our national network of over fifty UIE alumni, donors and community members. These benefits include mentorship and networking opportunities, care packages, a trunk party, and ongoing support from United In Excellence. STUDENT CRITERIA - A senior-level student attending Joliet Township High Schools, District #204-3.0 GPA at the time of application - Identify as American Indian or Alaska Native, Asian, Black or African American, Hispanic or Latino ethnicity, Native Hawaiian or Pacific Islander - Proof of enrollment in a post-secondary institution - Obtain one (1) letter of recommendation from an adult supervisor from an arts-based, out-of-school time program - Complete a 500-1,000 word essay based on one of the following JTHS alumni of color and your personal connection to their career or artistic product. Examples include: Katherine Dunham (1926), Lynne Thigpen (1966), Lionel Richie (1967), Ann Nesby (1973), Jermaine Stegall (1995). Dr. Timmothy D. Bailey Scholarship for the Next Generation of STEM Leaders United In Excellence seeks to support students who have the desire to major in a Science, Technology, Engineering, or Math (STEM) fields through a scholarship named after JTHS alumnus Dr. Timmothy D. Bailey. Dr. Bailey (1975-2013) was an active student at Joliet Central High School. He fulfilled his dream of giving back to his community by earning his medical degree and opening his own practice in Joliet, Illinois. His dedication to excellence was shared with his family, community and profession. He departed from this life unexpectedly in June of 2013. AWARD AMOUNT United In Excellence Fund, in partnership with the JTHS Foundation, is pleased to announce that it will award two (2), $500.00 Bailey Scholarships to students at Joliet Township High Schools, District #204 in the spring of 2017. Awards will be competitive across the entire district. 4

Awardees will also receive ongoing support from our national network of over fifty UIE alumni, donors and community members. These benefits include mentorship and networking opportunities, care packages, a trunk party, and ongoing support from United In Excellence. STUDENT CRITERIA - A senior-level student attending Joliet Township High Schools, District #204-3.0 GPA at the time of application - Participation in at least (1) extracurricular activities (school or community) - Proof of enrollment in a postsecondary institution - Obtain one (1) letter of recommendation from former or current science teacher on the applicant s potential in a postsecondary STEM program. - Complete a 500 1,000 word essay based on the following prompt: How can STEM improve the community in which I live? TIMELINE, SELECTION & DISBURSEMENT AWARD TIMELINE & DISBURSEMENT Award letters will be sent to the postsecondary institution during the first semester. Funds will be delivered to the postsecondary institution during the second semester of enrollment. Awards are renewable through an annual reporting and application process each spring. APPLICATION PROCESS Both online and paper versions of the application will be made available on Monday, February 27 th. Application materials must be submitted to your home school by Thursday, March 30 th at 5:00pm. SCREENING PROCESS Final candidates will be asked to interview with a selection committee between April 2-11, 2017 in Joliet IL. 5

BEFORE YOU SUBMIT Make sure you have a complete application. Incomplete applications will not be considered. Your proposal should include: - A complete application form (pages 6 and 7) - Proof of Enrollment in a post-secondary institution (acceptance letter or other verifying document) - One (1) letter of recommendation - 500-1,000 word essay APPLICATIONS ARE DUE THURSDAY, MARCH 30 th All application materials must be delivered in one package/envelope to your guidance counselor. Program Renewal & Reapplication. Scholarship recipients may reapply to renew their scholarship each year. More information on renewal will be sent to your home address and e-mail listed on the application during the 2017-2018 program year. Additional Opportunities. United In Excellence will host additional opportunities to support scholarship recipients. Participation in these opportunities is highly recommended, but not required. Opportunities include: hosting a shared trunk-party for all of our scholarship recipients, receiving care packages from our network, receiving a college visit from our network/alumni, participating in formal and informal networking opportunities, participating in mentorship and professional development opportunities from United In Excellence alumni and community members. Waiver & Release. Scholarship recipients must consent to the participation in interviews, the use of quotes and the taking of photographs, movies, or videotapes. Awardees give United In Excellence and the JTHS Foundation the right to edit, use, and reuse said products for non-profit purposes, including use in print, on the internet, and all other forms of media. Scholarship recipients release United In Excellence, JTHS Foundation, and their agents and employees from all claims, demands, and liabilities whatsoever in connection with the above.
