Vision The Tennessee Primary Care Association s vision is this: Every Tennessean will have access to evidence based, patient centered primary health care; The overall health of Tennessee s citizens will improve; Community health centers will play a pivotal role within a transforming health care system; The Tennessee Primary Care Association will contribute to this vision through services that demonstrate excellence and are guided by its mission. 2012 Strategic Goals Assist CHCs become health care homes Help to develop workforce resources for CHCs Secure funding to grow and sustain CHCs Increase the number of Tennesseans served by a CHC Deliver relevant, accessible training and technical assistance Increase & diversify TPCA funding needed to accomplish goals 2
