Procedures and Guidelines for the PNPA Selection Process 2016 1. The list of minimum requirements is indicated in the Selection Process Data Sheet (Please Check Below). Eligibility of the prospective service provider shall be checked and evaluated by the Selection Committee; 2. The prospective service provider should have completed within three (3) years from the date of submission and receipt of proposal, a contract similar to the project (Photography) equivalent to at least 50% of the amount of Contract Price of granted Project Proposal supported by Acceptance Certificate; 3. Selection process will be conducted through a modified competitive selection procedure and the Selection Committee will adopt pertinent provisions of RA 9184 and its Implementing Rules and Regulations (IRR). One of the modifications is that the potential service provider will be required to bring samples of the required projects or similar products during the opening of proposal and Audio-Visual Presentation of the company to be rated both by the Selection Committee; 4. Interested service providers are required to submit their Letter of Intent and Company profile, which includes the company history, list of photography equipment, facilities, list of clientele and organizational structure to PCINSP RODEL GUROBAT TRUMATA, OIC, Corps Publications, PNPA, Camp General Mariano N. Castañeda, Silang, Cavite (E-mail Add: rodel_one7seven@yahoo.com on or before the Selection Process on or before July 24, 2016; 5. The Corps Publications Selection Committee will hold a Selection Conference open to all potential service providers on July 24, 2016 (Sunday); 1:00 PM at the PCAI Hall, PNPA, Camp General Mariano N Castañeda, Silang Cavite. 6. Only prospective service providers who participated in the Selection Conference will be qualified to submit their proposal for the selection process. 7. Quotation shall be sealed in a long, brown envelope and must be submitted to OIC, Corps Publications, PNPA, Camp Gen. Mariano N. Castañeda, Silang, Cavite on the day of the Selection Process. Proposal will be opened and evaluated by the Selection Committee in the presence of the service provider and/or his or her official representative who signifies to participate in the Selection Process. Late submission of Project Proposals shall not be accepted. 8. Price Proposal should be inclusive of freebies and a project proposal for the benefits of the Cadet Corps, PNPA. 9. Interested service providers may inquire for other details from the Officer in- Charge that can be contacted thru Mobile Phone No. 0927-580-9853. Interested service providers should not directly coordinate and transact with the members of the Corps Publications without the approval of the Officer-in-Charge. 10. The Cadet Corps Philippine National Police Academy reserves the right to accept or reject any project proposal, to annul the selection process, and to reject all proposals at any time prior to contract award, without thereby incurring any liability to the affected service provider or providers.

Certified True Copy of Audited Financial Statements (Balance Sheet and Income Statement) for the last 3 years (2013, 2014, 2015); 6. Certified True Copy of Company/Business Income Tax Return (ITR) for the last 3 years (2013, 2014, 2015); 7. Original Letter of Authorized or Secretary s Certificate showing the photograph of authorized representative/s; 8. List of clients/customers (past and present); and, 9. Sketch of Office/Plant/Factory. B. OTHER REQUIREMENTS: 1. EQUIPMENT AND FACILITIES: a. Printing Machines for the photography; b. High-end cameras and video cams; c. Equip with high-end computer units for editing; d. In-house framing and printing; and, e. Accessible and convenient working areas to accommodate clienteles; 2.PERSONNEL/MANPOWER: a. In-house personnel who are experienced photographers, videographers, and graphic artist/designer; b. Availability of personnel to cover/document events and other activities during on call basis ; and c.
