Proposal Submission Instructions The complete proposal must be received by close of business (5 PM PST) August 30, 2013. Proposals submitted after this date will not be reviewed. Prior submission and approval of a Letter of Intent is required for proposal submission. Proposal submissions without prior Letter of Intent approval will not be reviewed. Submit an electronic copy of the complete application either as a Word document or a text-accessible PDF file by e-mail to STAward@coopervision.com Proposal Formatting Instructions All proposal should conform to the following requirements: Use Arial typeface and a font size of 11 points or larger Type density, including characters and spaces, must not exceed 15 characters per inch Type may be no more than six lines per inch Use standard letter size (8 ½ x 11 ) Use at least one-half inch margins (top, bottom, left, and right) for all pages All page limits specified refer to single-spaced format using the above formatting requirements Required Proposal Components All applications must include the information detailed below. Applications that do not provide all the required information will be considered non-responsive and will not be reviewed. Applicants are discouraged from submitting information considered proprietary. 1. Face Page Research Proposal, Fiscal Year 2014 The Face Page must be completed and signed by the Principal Investigator and the official signing for the applicant organization 2. Abstract (150 words maximum) Provide an abstract describing the proposed research in lay terms for a non-scientific audience.
3. Executive Summary (1 Page maximum) The executive summary serves as a succinct and accurate description of the proposed research when separated from the application. It should be informative to other persons working in the same or related fields and insofar as possible understandable to a scientifically literate reader. It should contain: Rationale behind the proposed research Specific Aims precisely stating the goals of the proposed research Key technical challenges and proposed technologies A concise description of the research design and methods for achieving the stated goals Innovative claims describing the novelty and uniqueness of the proposed research relative to the current state-of-practice 4. Specific Aims and Rationale (2 Page maximum) Describe the overall goal and rationale for the proposed research. List the specific aims to be achieved over the duration of the award project. (For multi-year awards, list the specific aims to be achieved during each year of the award.) 5. Preliminary/Supporting Data (2 Pages maximum) Discuss the PIs preliminary studies, data, experience, or accomplishments and work in closely related research areas. Preliminary data can help to establish the likelihood of success of the proposed project. Seedling applications may include preliminary data, however, reviewers will be instructed to place less emphasis on the preliminary data in Seedling proposals than Translational Research proposals. 6. Detailed Project Description (5 Pages maximum for Seedling, 10 Pages maximum for Translational Research Award) The Project Description should describe the overall strategy, methodology and analyses to be used to accomplish the specific aims of the proposed research. The Project Description should include: Technical approach describing experimental plan and methods to achieve the specific aims of the proposed research, and justification why the approach is feasible. Discussion of potential pitfalls of the technical approach and alternate approaches Comparison with other ongoing research indicating advantages and disadvantages of the proposed research Innovative claims describing the novelty and uniqueness of the proposed research relative to the current state-of-practice Future relevance describing how CooperVision key focus areas will be advanced if the aims of the proposed research are achieved
7. Statement of Work (2 Pages Maximum) The Statement of work should provide specific, quantitative milestones to measure research progress. Delineate key milestones and timelines for the proposed project Cite the specific tasks to be performed in connection to the milestones Include the general description of the objective for each task or subtask Include the exit criteria for each task a product, event or milestone that defines its completion Timeline may be shown in a Gantt chart 8. References (No limit) Provide a list of references for the publications cited in support of the proposed research 9. Budget Page (2 Pages maximum) Use the budget form to provide an itemized budget for the support requested for each fiscal year (FY = Nov 1 Oct 31) of the award and the total requested period. In the budget include: Direct Labor Costs: include name, role, and percent effort Supplies: Itemize by category (i.e. polymers, chemical reagents, molecular biology reagents, glassware, etc.) Animal Costs: Itemize by category (i.e. number of animals, housing, etc.) Travel: Provide the purpose of the trip, departure and arrival destinations, and number of days per trip. Other Costs: Itemize Indirect Costs: Must show base amount and rate In addition, please describe any contributions or synergistic activities that will augment this project. Examples include: Staff time that is covered under other funding but which contributes to this project Costs associated with facilities which are covered elsewhere Students working for academic credit rather than pay 10. Facilities and Resources (1 Page maximum) Provide a general description of the institutional facilities and resources available to achieve the goals of the research. The information provided is of major importance in establishing the feasibility of the research goals.
11. Key Personnel List all key personnel involved in the proposed research, their title, affiliation, role in research, and percent effort on the proposed research. Although the research must be headed by a single Principal Investigator, co-investigators may be identified here. Provide a biographical sketch (4-page limit; NIH biosketch acceptable) in the Appendix Materials for the PI and all key personnel. The biosketch should include Affiliations, Education and Training, Research and Professional Experience, Current and Pending Source of ALL research support (title, percent effort, funding amount, and budget period). The biosketch should identify any companies where the principal investigator or key personnel serve as a director, officer, advisory board member, or consultant 12. Collaborators List all proposed collaborators of the application and their affiliations Provide letters of collaboration in Appendix Materials for all collaborators and their institutions 13.
