ROLE OF THE PHYSIOTHERAPY SERVICE The Department of Medicine for the Elderly Physiotherapy Service exists to deliver high quality physiotherapy to patients and their carers, referred to South and West Glasgow. The key responsibilities of the service are to provide assessment, diagnosis and treatment to a diverse range of conditions and patient types, in a variety of environments. Therapeutic management utilises a range of physical, behavioural and cognitive modalities to regain function following surgery, illness, injury, dysfunction or disease. This includes the management of critically ill patients whose survival and recovery depends on appropriate physiotherapy intervention. There is also a key role in health promotion. The service incorporates service management, planning and development within a defined budget and in a multi-professional framework. All staff within the physiotherapy service must comply with Division and Departmental Policies in addition to complying with Chartered Society of Physiotherapy Professional Standards and Rules of Professional Conduct 3. JOB PURPOSE Takes responsibility for the physiotherapy service within DME with a highly complex caseload incorporating patient assessment, diagnosis, treatment and discharge from care. As an experienced practitioner is responsible for planning, co-ordinating, delivering and evaluating the physiotherapy service provided to patients within DME, with input to future planning for care and management in conjunction with the physiotherapy superintendent. Plays a key role in the education and development of physiotherapy and other staff within the DME service. As physiotherapy expert acts as an educational resource to physiotherapy staff. May be actively involved in research pertaining to DME within the Sector, city wide and nationally. Provides an expert resource and specialist advice to other health care staff/ patients/carers/other agencies.

Deputises for Clinical Specialist Physiotherapist as required. Supervises Band 5 Physiotherapists, Physiotherapy Support Workers plus students on clinical placement. 4. SCOPE AND RANGE Works as a physiotherapy expert in the clinical area of DME Works as an autonomous clinician within DME taking full responsibility for an identified complex caseload Has responsibility for prioritising own caseload within DME Has a key role in local audit and clinical effectiveness projects this may extend to formal research projects Provides support, guidance and teaching, to rotational staff and support workers Including staff from the multi professional team, in role as senior staff member and mentor. Manages poor performance Monitors and evaluates the service provision within DME in conjunction with the line manager and identifies areas for development or improvement Supervises students on clinical placement and is responsible for their formal assessment

MAIN DUTIES/RESPONSIBILITIES Clinical Is professionally and legally accountable and responsible for all aspects of own work, including direct and indirect patient care, within scope of practice Assesses capacity, gains valid consent and has the ability to work within a legal framework with patients who lack the capacity to consent Acts independently to assess, analyse clinical and non clinical information and provides a comprehensive clinical diagnosis for individual patients, to determine their need for physiotherapy intervention. This includes patients with multiple pathologies and complex clinical presentation Acts independently to plan, implement, evaluate, treat and progress patient care to maximise their rehabilitation potential. This includes decision making regarding discharge from care and ensuring appropriate ongoing care. Manages an identifiable caseload of patients effectively and efficiently Works as part of the DME multi professional team to ensure effective communication and delivery of care. Incorporates additional skills gained from continuing professional development into clinical practice and disseminates these to others. Maintains accurate, comprehensive and up to date patient documentation, records and accurate mandatory statistical information to reflect care provided and meet professional, local and national standards Communicates effectively, works collaboratively with, and makes recommendations to all relevant colleagues to maximise patient care and promote effective multi-professional working. This includes frequent attendance at case conferences and ward rounds Provides highly specialist advice and treatment within DME Works independently to provide emergency respiratory on-call care to support delivery of the 24 hour service to acutely ill patients Managerial Takes responsibility for the day to day management and delivery of services to DME As a mentor, provides support and guidance to identified staff including participation in the departmental Personal Development and Performance Review system to promote personal and service developments. Develops a robust framework to monitor and evaluate standards of care and clinical outcomes. This may include benchmarking with similar units. Demonstrates professional leadership within DME and with peers

Identifies gaps and pressures in DME and discusses and implements remedial action with line manager Is actively involved in clinical audit and clinical effectiveness projects to support the Clinical Governance Agenda Is actively involved in the recruitment and selection of staff through the interview process Develops and implements clinical guidelines within DME in conjunction with Line Manager Implements and monitors Health & Safety issues within DME in conjunction with AHP Lead Complies with the organisational and departmental policies and procedures and to be involved in reviewing and updating as appropriate. Collects and collates statistics required by the Physiotherapy Department Educational Attends and contributes to the various on-going in-service training programmes to promote own personal development and that of staff these may be physiotherapy or DME specific Provides both spontaneous and planned specialist advice, teaching and training to other members of th multi-professional team, relatives and carers, voluntary sector and other agencies, to promote knowledge of physiotherapy management to enhance patient care. Provides formal and informal teaching sessions for Senior II, Staff Grade and Physiotherapy Support Workers and students (physiotherapy, nursing and other professions) in DME Is responsible for the clinical training and formal assessment of undergraduate and post graduate physiotherapy students within DME Contributes to the development of undergraduate students or other health care professionals outwith the Sector, through involvement in external lecturing and teaching

SYSTEMS AND EQUIPMENT Has shared responsibility for the maintenance, repair and security of physiotherapy equipment within the DME physiotherapy service Understands, appropriately selects, and monitors safe use of physiotherapy equipment including electrotherapy, exercise prescription and equipment, orthotic appliances and walking aids. Understands, appropriately selects and ensures safe use of additional patient care equipment including specialist beds, hoists, stand aids, sliding sheets and wheelchairs. Is competent in the use of current documentation systems e.g. Integrated Care Pathways and is involved in the development of new systems e.g. electronic records Demonstrates a working knowledge of relevant IT systems and software packages e.g. Physiotools (exercise programme package) Is up to date with, and has a full understanding of the current systems for student assessments and formally assesses students on clinical placement 8. DECISIONS AND JUDGEMENTS Accepts referrals and then assesses, plans and undertakes highly complex Physiotherapy interventions using clinical reasoning (a systematic approach to reaching decisions regarding interventions relating to presenting factors)to decide how to continue or discontinue treatment, taking all presenting factors into account regarding the clinical condition and utilising an holistic approach to individual care Works autonomously making clinical decisions within scope of practice Advises relevant personnel regarding on-going treatment and progress throughout the episode of care and discharge Undertakes risk assessments regarding patient condition and environment to ensure patient and staff safety Balances clinical, managerial and professional demands to ensure quality of care within DME Takes responsibility for local staff management problem solving e.g. time keeping and demonstrating an understanding of local Human Resources policies Allocates caseload and delegates appropriate tasks to more junior staff, Physiotherapy support workers and students Directs and advises more junior staff regarding clinical decision making and organisational issues Seeks advice from more experienced colleagues/peers

COMMUNICATIONS AND RELATIONSHIPS Patients and relatives/carers Communicates complex clinical information e.g. related to diagnosis, treatment, prognosis, effectively and appropriately, with patients and their carers using a range of highly developed verbal, non-verbal, written and presentation skills. This may involve conveying complex terminology into lay terms e.g. communication difficulties relating to stroke patients Utilises appropriate communication methods to ensure patient compliance with treatment programmes Identifies and modifies the most appropriate communication method depending on the individual requirements e.g. hearing or visual impairments, learning difficulties, language differences, disinterest, cognitive impairment or perceptual problems Provides support, reassurance and encouragement to patients and their carers as part of the rehabilitation process. Uses skills to convey comprehensive detail of physiotherapy treatment programmes in a manner and at a rate which is appropriate for every individual, emphasising and reiterating points as and when necessary to ensure a full understanding. Encourages and motivates patients to maximise outcome recognising those who are in pain, cognitively impaired or those who are afraid or reluctant and require reassurance, motivation and persuasion to comply with treatment. Conveys information in a particularly sensitive manner when it is contradictory to patient and carer expectations and desires. Provides advice and guidance to relatives and carers relating to on-going rehabilitation programmes, positioning and moving and handling Listens effectively with a high degree of skill to patients and carers needs and concerns and reacts/intervenes appropriately Develops an excellent rapport with patients and carers, instilling trust and confidence. Demonstrates effective communication strategies to deal with anxious patients and carers in stressful or emotional situations

PHYSICAL DEMANDS OF THE JOB Manual handling of patients on a daily basis. This may include assisting patients with significant physical, cognitive or behavioural impairment. Patients may be very immobile, obese or unwilling to move e.g. patients who require support for lengthy periods often from lying to sitting position, sitting to standing and transfer onto chair and back in one session; Assisting patients with walking and stair practice this may include sudden and unpredictab changes in movement or faints/falls which require physical support and/or lowering to the floor to avoid injury to patient and staff.
