South West Accuracy Checking Pharmacy Technician Scheme Transfer of Accreditation Transfer of a SWMIT accreditation around the South West Accreditations awarded by training and assessment programmes that meet the standards of the Nationally Recognised ACPT Framework represent a transferable skill across organisations. The framework is recognised by all NHS regions in England, Wales and Northern Ireland. It is recommended that when individuals with an accreditation awarded by SWMIT are transferring between South West Trusts, departments/units or a satellite dispensary, the ACPT undergo a locally agreed period of probation. During this probationary period the ACPT will become familiar with local policies, SOPs and complete an accuracy checking log of 200 final accuracy checked items (with no errors) to reflect local practice. Once complete, local and personal records must be updated to reflect the change/addition to the individual s scope of practice. NB: This is only considered necessary if SOPs are different or if the specialities/prescription types are significantly different before the ACPT assumes their new role. SWMIT should be notified of a change of Trust to ensure the database and individual records are updated accordingly. Transfer of accreditations awarded by other training providers If a candidate with an ACPT accreditation awarded by another training provider (which meets the national framework standards) wishes to transfer to a SWMIT accreditation and practice as an ACPT in the South West Trusts, a locally agreed probationary period must be completed. During this probationary period the ACPT will become familiar with local policies, SOPs and complete an accuracy checking log of 200 final accuracy checked items (with no errors) to reflect local practice. Once complete, the following documentation should be submitted to SWMIT for assessment: The 200 item accuracy checking logs A copy of the original ACPT certificate and most recent reaccreditation records* GPhC registration number (or a copy of qualification certificates) A completed Transfer of Accreditation Form (available in the Transfer of Accreditation Toolkit at http://www.swmit.nhs.uk/techchk.htm) *Applicants whose reaccreditation is pending within the next 6 months should also complete the SWMIT reaccreditation process and submit this documentation in addition to the above. SWMIT will assess the documentation and confirm that the original accreditation meets the national framework standards. Successful applicants will be awarded a SWMIT ACPT certificate and be added to the South West database. The individual s chief pharmacist will be notified of the successful transfer. 2
South West ACPT Scheme Transfer of Accreditation Form This form is to be completed when an individual wishes to transfer their Accuracy Checking Pharmacy Technician (ACPT) accreditation when moving between Trusts, either from within or outside the South West region. Guidance from the Nationally Agreed Framework for Checking Dispensed Items recommends that when a pharmacy technician transfers between trusts or wishes to transfer an accreditation, the following tasks must be achieved: A probationary period of a minimum of 2 weeks, during which time the pharmacy technician must: Familiarise themselves with local policies and procedures Complete an accuracy checking log for a minimum of 200 second checked items (with no errors) to reflect local practice Information in support of your application (to be completed by candidate) Please record your checking activities for a minimum of two weeks and complete the following: I have read the local policies and procedures regarding the dispensing and the ACPT role. I have completed a two week probationary period. During this time I have checked items. Since transferring trusts I believe that I have met the recommendations of the Nationally Agreed Framework for Checking Dispensed Items and demonstrated my skills as a competent Accuracy Checking Pharmacy Technician. I would like to gain approval for transfer of my accreditation from the Dispensary Manager and Chief Pharmacist.
