ABOUT THE WELLNESS PROGRAM Saint Francis University is committed to being a University where employee health and wellness is a top priority. For this program year, the University is changing the incentive program, adding ways to manage and earn points to earn monetary incentives for participating in programs, and making it easier for you to make choices to better your own personal health and wellness. The WebMD Rewards program offers you ways to improve your own health and wellness no matter what your goals are! INCENTIVES AND STEPS INVOLVED For the 2015 program year, which runs from February 1 through, the University has decided on a points based system for participation in the WebMD Rewards program. This program is open to all full-time employees of the University, in addition those full-time employees with Highmark medical insurance through a different employer will also be incentivized for their participation in the program. Starting on February 1, 2015, full-time employees with Highmark insurance will be able to: Complete the Wellness Profile full-time employees with Highmark insurance will be able to access the Wellness Profile online using your Highmark ID and Password for managing your insurance benefits Complete the Biometric Screening Complete Preventive Exam Complete a variety of programs to better your health and wellness to reach your own personal goals Employees will earn points for participation in the WebMD Rewards program, and could potentially earn up to a maximum of $200 for participating in the program. The more programs that individuals participate in, the more points can be earned. Employees can receive the monetary incentive in ONE OF THREE ways: $200 in your HSA account OR $200 off of your premium contribution for health insurance OR $200 monetary payment. The maximum incentive to be earned is $200, but individuals will be compensated based off of the points they have accumulated through the program. For example, if you earn 100 points you will earn $100. PROGRAM CATEGORIES There are many different definitions of wellness and it means different things to different people. There are many different program offerings to complete the incentive program and focus on your own health and wellness goals. To make the program easier to understand this year, we have organized the wellness programs into categories, with points assigned to each one. The points you earn will make you eligible for prize drawings throughout the 9 month wellness program. It is recommended that you begin with taking the online wellness profile and participating in a biometric health screen. After completing these two steps, you can choose from any of the programs that interest you and will help you better your own personal health and wellness. Please note, you do not need to participate in a wellness program from each different category. 2 On the following page, you can see a charted description of the programs that are offered through the WebMD Rewards program. This is not an all encompassing list, as different programs and options may be added throughout the year, but these are the main programs that will be offered to individuals to complete the program.

How to Register 1. Go to the Highmark member website: www.highmarkbcbs.com 2. Click on the Members tab. 3. Click on the Register Now link. 4. Complete the registration form. You will need your Highmark member ID card. Refer to your insurance card and enter only numbers. 5. Click Submit 6. Note: If you have registered in the past, you just need to click on Log in and use your user name and password Even though it is not required to complete the program in this order, we highly recommend that you start earning points by first taking the online health and wellness assessment on the Highmark website. After taking the online health and wellness assessment, we recommend that you set up an appointment for a biometric screening either at the Student Health Center on the date specified below or with your primary care provider. By first starting with these two parts of the program, you will earn points towards prizes, but also will learn more about your own baseline health WELLNESS PROFILE Points Earned Towards Participation: 30 points The online health and wellness assessment is a confidential questionnaire administered by Highmark. This questionnaire assesses the current status of your health, and will give you a ranking based off of your peers. The assessment takes about 15-20 minutes to complete, and employees may take it during work time. By taking this assessment, you will receive immediate, personalized feedback about your risks for developing certain conditions or diseases. In addition, you will receive suggestions on programs to participate in to help you achieve your personal health and wellness goals. BIOMETRIC HEALTH SCREENING Points Earned Towards Participation: 55 points In order to gain a better understanding of what your own individual health measures are, such as total cholesterol, blood pressure, BMI, and fasting blood glucose, we ask that you participate in a free biometric health screening that will be offered at Saint Francis University. Free screenings are available through the Student Health Center, and will be conducted on Friday, February 13th. You may also schedule a screening with your physician or at a lab facility of your choice, however your deductible would apply for both the PPO Plan and the High Deductible Health Plan. You can earn 55 points when you complete this assessment by April 30, 2015. If you choose to schedule a screening at a your own health care clinic, you must submit a form to Melinda Krimmel that is completed by your physician in order to obtain the 55 points by April 30, 2015. PREVENTIVE EXAM Points Earned Towards Participation: 50 points Preventive care means getting the right kinds of preventive health services screenings, counseling, immunizations and preventive medicine at the right times. It s about protecting one s health by knowing what you need and when to get it. One step in the WebMD Rewards program is that Saint Francis asks each employee obtain a preventive physical exam each year at their primary care physician s office. In order to receive credit for obtaining a preventive physical exam, we ask that you take the Preventive Exam form to your appointment to have your primary care provider sign the form stating that you received your preventive exam. This form needs to be returned to Melinda Krimmel by. 4 The merits of preventive exams and services are clear; it s estimated that 70% of chronic conditions are related to lifestyle and therefore, preventable. Preventive exams should also be covered at no charge under your current health insurance program, as long as you let your physician know that you are interested in obtaining a preventive exam when scheduling your appointment. Preventive care adds value to the dollars spent on health care services because premature death and illness may be avoided. However, use of services is low, and we hope to encourage all employees to stay up-to-date with preventive care.
HIGHMARK INTERACTIVE PROGRAMS Tobacco Cessation Only (50 points) Tobacco Cessation Programs: By calling Blues on Call at 1.888.8.3428 members will have access to specially trained Health Coaches. Your personal Health Coach is available from 9:00 a.m. to 9:00 p.m. to discuss any of your health and wellness questions and concerns. If you re interested in making further lifestyle changes, your Health Coach can guide you to programs for nutrition, weight management, physical activity, stress management and tobacco cessation. They will assist you in creating goals, help you overcome barriers, and guide you on a path to lifelong wellness. Make time for your health today! The WebMD Digital Health Assistant can make a real difference in helping your employees improve their health. The online, self-guided program begins with a series of interactive questions to help members create a customized program based on their health focus and desired participation level. Tobacco Cessation is one of the six topics available. Log on today to get started! Points Earned for Participating: 10 points per program Eat Well for Life Campaign The Eat Well for Life Campaign is an 8-week interactive, self-guided newsletter campaign designed to help participants raise awareness of their current eating behaviors with the goal of building a better diet. The campaign includes weekly action-oriented newsletters, activities and strategies in an easy-to-read style that demonstrates how to make healthy eating simple and fun. Color Your Plate Color Your Plate is a six-week program that offers valuable information and resources for you to help improve or enhance your nutritional habits. Most individuals fall far short of achieving the nutrition recommendation goals established by health and nutrition experts to consume five to ten servings of fruits and vegetables daily to protect against major diseases. Color Your Plate is designed to help meet that goal. You will receive weekly newsletter articles, motivational tips and strategies, and a healthy recipe guide to make healthy eating simple and fun. You will use tracking logs to gain insights about your consumption. Maintain Your Mind Real Food 101 An interactive 6-week health education campaign for individuals to take steps to preserve and promote brain health. It includes six informative, engaging and fun newsletters, which focus on healthy lifestyle choices and other factors that may help preserve and promote brain health. Each newsletter includes brain challenges, fun mind games, daily vitamins for the brain and other fundamentals of brain health. Real Food 101 is a series of five informative and educational newsletters that teach employees the basics of determining if food is "real" in a food market that has an abundant supply of processed foods. This package includes electronic promotional materials that can be customized by the client and five newsletters that help employees make educated decisions when it comes to food. 5
SELF-DIRECTED WELLNESS ACTIVITY AND EMPLOYEE SPONSORED PROGRAMS Points earned: 10 per program You can earn a maximum of 80 points per plan year by participating in a self-directed wellness activity. For this program, you must manually log your activity and submit it to Melinda Krimmel by. Eligible activities include: Biggest Winner Challenge Participate and complete the 10-week Biggest Winner Challenge sponsored by the University. This program focuses on nutrition and physical activity changes to help employees manage their weight. Employees can also receive points for participating in an alternative weight management program, such as Weight Watchers. Medical Nutrition Therapy One-on-one dietary counseling with a Registered Dietitian that is offered through Highmark insurance for individuals that meet the following criteria: BMI 30 and a referral from their primary care provider. Deductibles would apply prior to this service being covered under your insurance plan. Individuals can also pay out of pocket for this service if so desired. Race or Other Athletic Activity: Complete an event such as a 5K, 10K, or half marathon. Group Fitness Classes: Participate in an hour of group class activity at least once a week for 6 months. Fitness Center Usage: Participate in at least 150 minutes of exercise a week for 6 months. Open Swim/Water Aerobics: Participate in an hour of activity at least once a week for 6 months. You will receive your points for this exam after you turn in the completed form to Melinda Krimmel. For all other programs that you participate in, for example Biometric Screens at Saint Francis, 10,000 Steps, the Biggest Winner Challenge, Medical Nutrition Therapy, Lunch and Learn Programs, and the Highmark Directed Programs, Melinda Krimmel will track points based off of the registration lists for each of these programs.
