Strategic Framework Vision Together we are building a bridge of services for incarcerated and formerly incarcerated persons Mission Forge positive change in system delivery of services to incarcerated persons and their families. Reducee recidivism and increase public safety in the Solano County community. With our public and nonprofit partners we will increase opportunities in education, employment; health, mental health, substance abuse, housing; and support for children and families. Goals Coordinated Discharge Reduced Recidivism Holistic Care for Individuals, Families and Communities Catalytic Projects Performance Measures % of discharge plans one month prior to discharge Felony crime rate % of successful outcomes within 12 months Objectives: Achievee 90% coordinated discharge plans of formerly incarcerated persons by 2015. Reduce the recidivism rate of formerly incarcerated adults by 15% byy 2015. Reduce the recidivism rate of Drug Court participants by 20% by 2015. Achieve a 65% success rate of clients reaching their case management outcomes. 3 of 22

Background The Solano County Re-entry Council came into existence through the recognition of a need in our communities to collaborate and coordinate reentry services. This has been effort on the part of our service providers and the community since 2006. Our coalition is made up of representa atives from over 50 agencies and organizations currently providing services to thee residents of Solano County. Together we have hundreds of years of cumulative experience in fields directly related to criminal justice and reentry issues. We would like to participate in the development of this strategic plan for the provision of reentry services. Our Focus and Mission is to forge positive change in system delivery off services to incarcerated persons and their families. We will reduce recidivism and increasee public safety in Solano County community. With our public, and nonprofit partners we willl increase opportunities in the areas including education/ employment; health/mental health/substance abuse; housing; and children and family support. Responsibilities Solano Reentry Council takes on the task of developing and implementing the Strategic Plan for Reentry Services in Solano County. The Strategic Plan is based on input from Council Members, Community Stakeholders, County & State Stakeholders and the Executive Committee Members. are determined by the subcommittees (champions). In July of each year, the champions submit a first DRAFT of next year s activities to the Executive Committee for additional input and review. The subsequent Council Meeting finalizes and approves the Strategic Plan for the coming year. 4 of 22

Coordinated Discharge FY 2010 / 111 Strategies Create a gender responsive discharge and treatment plan Create discharge agreements with the Californiaa Departmentt of Corrections ( CDCR) and Solano Sheriff Department for inclusion of assessment data in discharge / treatment for triaged referrals system-wide Collaborate with CDCR / Sheriff to create contact visitations for children and parents Establish real time information on available housing in Client System Centralized housing specialist coordinate efforts between criminal justice, community-based partners and landlords Establish a standardized assessment and triaged referrals system wide, establish re-assessment protocol with Frequent User Team Create a system wide mentor / parolee support group which links offenders to the community Collaborate with DA/Probation & Public Defender to develop a county-wide clean slate program to reduce barriers to employment. Evaluate the coordination efforts for housing and the level of collaboration between justice, community-based partners and landlords Evaluate efficacy of integrated assessment and referral system / TBD Calendar follow up training with Dr. Covington Submit quarterly evaluations to DOJ. Coordinated Discharge FY 2012 / 13 Strategies Evaluate / report outcomes of gender responsive discharge and treatment Recommend improvements to referral system based on evaluation of discharge and treatmentt outcomes 7 of 22

Recommend improvements to contact visitations for children and parents based on visitation outcomes Combine permanent housing plan of Safety Net Consortium with aftercare plan, recommend system improvement Based on evaluation of the housing coordination efforts, recommend improvements to the referral system Recommend improvements to standardized assessment and triaged referrals Recommend improvements to gender responsive treatment protocol and re-assessment protocol Conduct focus groups with mentors, alumni, community members on the efficacy of the re entry program Use clean slate evaluation as a basis for creating other access programs; survey participants Actively seek additional funding directed at providing additional medical services in Solano County for formerly incarcerated persons. Health Establish collaborative medical service centers and support through grants and/or other funding applications. Continue to strengthen and expand partnerships to meet federal and state health and safety code regulations. Stabilize and demonstrate medical outcomes of services and service providers for formerly incarcerated persons 8 of 22

Coordinated Discharge FY 2013 / 14 Strategies Publish comprehensive report that includes program outcome evaluations and Social Return of Investment on: - Integrated Discharge / Referral - Contact Visitations - Coordinatedd housing plan - Coordinatedd Aftercare plan - Standardized assessments - Re-assessment protocols - Coordinatedd clean slate program The report includes percentage of coordinated discharge plans by fiscal year and the percentage of discharge plans that were one month prior to discharge each fiscal year. The report includes recommendations of focus groups for system wide improvements. Publish report on clean slate program evaluation 9 of 22

Secure funding to plan and initiate a Project What chapter in Solano County Initiate a United Through Reading program at CSP Solano in collaboration with faith based communities and explore the possibility of establishing such a program in the Solano County Jail and at Juvenile Halll starting August 2010. Hold a second annual Get on the Bus event in June 2011 at CSP Solano. Publish and post on-line a current resource guide that is available and accessible through several venues and distribution locations Health Assign a person to annually verify provider information and add new provider s services to the current resource guide. Organize the User Friendly guide by service and or category in such a way that the guide will assist with reentry planning for the formerly incarcerated and their support team. Establish relationship with CDCR and seek assistance to print the resource guide. The resource guide shall be made available to any agency and orr person seeking information. 17 of 22
