1 AEMH ACTIVITY REPORT CPME Meeting, Stockholm 18-19 November 2005 1. AEMH Conferences 2. AEMH Plenary Meeting 3. External Meetings 4. Organisational Matters 1. AEMH-Conferences The AEMH has started in 2004 to organize its first conference combined with the plenary meeting and encouraged by the success has decided to pursue this initiative. The second AEMH conference was thus organized in May 2005 in Athens and entitled The Core Curriculum of CPD covering topics such as -the collaboration between the pharmaceutical industry and the medical profession, -recertification/ revalidation, -quality assurance of CPD, -the American model, -EACCME -the European Perspective of CPD. The programme is here attached. All presentations are available on the AEMH website http://www.aemh.org/navigation/section6/conference.html. Furthermore the outcome has been laid down in a Declaration on Quality Assurance and Quality Control in the process of professional development, which is also attached to this report, and which has been adopted by the AEMH general assembly. The Conference 2006 will be entitled Hospital Management based on Quality and Safety and deal with hospital budgets, quality programmes, implication of doctors in management, labour conditions of doctors and patients safety. The Conference will take place 27 April 2006 in Bratislava. More details will soon be available on the AEMH website.

2 2. AEMH Plenary Meeting The 58 th Plenary meeting took place in May 2005 in Athens.. The important decisions were Approval for the collaboration with FEMS and a common secretariat; Establishment of a reference group to draft a statement on The future of the AEMH in a new environment of European Medical Organisations. Establishment of a working group to amend the AEMH statutes; Establishment of a working group studying Quality Programmes in hospitals; Adoption of a Declaration on Quality Assurance and Quality Control in the process of professional development Adoption of a statement on the Bologna process; Adoption of a statement on Tobacco control (smoke-free hospitals); Adoption of the Joint Declaration CPME /EFPIA; Adoption of the WMA Statement concerning the relationship between Physicians and Commercial Enterprises. Elections President 2007-2009 : Dr Lies was re-elected Vice-President 2006-2008 : Prof Nolte (Germany) re-elected Internal auditor : Dr Morresi (Italy) 3. External Meetings The Patient Safety Conference in Luxembourg. The AEMH-President chaired the workshop on patient safety in hospitals. After the different presentations and the discussions, all stakeholders involved agreed that everybody must take his/her responsibility to improve the current situation. This kick-off must now be followed by common proposals elaborated by all the representatives of the different sectors. The participants especially in the hospital working group insisted very strongly that mentalities must change in all the different hospitals sectors. The AEMH is a member of the stakeholders group, which drafted a position paper on patients safety. Consensus Conference in San Remo The AEMH_President attended this conference, where a project of a common European Ethical Code has been discusses. The outcome of the conference has been laid down in the SanRemo Charter of European Physicians.

Council of Europe The AEMH has been granted participatory status to the Council of Europe. SG Brigitte Jencik attended the Plenary Conference in Strasbourg. The AEMH is member of the Health Grouping. 3 European Institutions, NGOs and think-tanks The AEMH, represented by its Secretary General, attended numerous meetings in Brussels. 5. Organisational Matters Secretariat The AEMH and the FEMS having mostly the same focus and pursuing the same objectives to protect the interests of doctors and patients in the hospital sector, have decided to rationalize their works and costs. The FEMS joined the AEMH Secretariat located within the premises of the CPME and established its first Permanent Secretariat as from 1 st July 2005. The Future of the European Medical Organisations The debate on the reorganization of the different European Medical Organisations has been a major topic also within the AEMH and resulted in the establishment of a reference group, which reflects all different tendencies within the AEMH and will therefore process in a consensual manner. Its task is to define a common position and to establish concrete proposals to the CPME and the other European Medical Organisations. Publications -The European PharmacoTherapy The AEMH-President drafted the foreword to the 2005 issue, in which also an article on The Core of CPD for Physicians and its relationship with the Pharmaceutical Industry and Legal Authorities by Dr Zilling and an announcement of the AEMH Conference 2005 have been published. For the edition 2006 Dr Lies has been asked in the Advisory panel. The AEMH will again advertise its Conference 2006. - Hospital The AEMH President drafted an article on the Patient safety Conference in Luxembourg, which has been published in Hospital, the Official Journal of the European Association of Hospital Managers.

Association Européenne des Médecins des Hôpitaux European Association of Senior Hospital Physicians Athens, 12 May 2005 SPEAKERS LIST AEMH Conference 2005 CONTINUING PROFESSIONAL DEVELOPMENT The Core Curriculum of CPD Raymond LIES, M.D. trained as General Surgeon in France and Germany and has been practising his profession since 1976. He defends the position of hospital physicians since 1992 and has represented the Luxembourgian hospital physicians within the Union of the AMMD (Association of Physicians and Dentists of Luxembourg) and as delegate of Luxembourg within the AEMH (European Association of Hospital Physicians) whose President he is since 1998. Dr. Lies chaired from 1992 to 2003 the Foundation in charge to build a new hospital in Luxembourg, which opened in July 2003. Currently he is the Medical and General Director of the Kirchberg Hospital in Luxembourg (450 beds). He is also President of the Board of l INCCI (Institut de Chirurgie Cardiaque et de Cardiologie Interventionnelle). Thomas ZILLING, M.D., PhD is holding the position as Associate Professor at the Department of Surgery at Lund University Hospital. He is a general surgeon with his special interest in oesphageal- and gastric surgery. Dr Zilling is vice president of the Swedish Association of Senior Hospital Physicians. Since 1997 he is a board member of the Committee of Education and Research of The Swedish Medical Association as an expert on postgraduate education and continuing professional development (CPD). Dr Zilling is Chief editor for the Swedish journal The Hospital Physician (Sjukhusläkaren). In 2004 Dr Zilling was elected Vice-President of the AEMH by the annual Representative meeting. Marie-Claire PICKAERT is an economist by training from the Vrije Universiteit te Brussel. She has worked in the Belgian association of pharmaceutical industry, where she was Director of the Economic & Social department. For twelve years, she was industry s spokesperson in the medicines pricing committee and was involved in negotiations re inclusion of medicines in the positive list and reimbursement conditions. She joined the European Federation of Pharmaceutical Industries and Associations (EFPIA) to create its Economic & Social department. After five years with EFPIA, she took the position of Deputy Director General.

Richard BERGSTRÖM is a pharmacist by training (MSc) from the University of Uppsala, Sweden. He has worked as a regulator at the Swedish Medical Products Agency in charge of the approval process and later as an executive in the pharmaceutical industry. For nine years he lived in Switzerland and worked with both Novartis and Roche in various regulatory management positions. For several years he was in charge of Roche's authority relations in the EU. Returning to Sweden in 2002, he took on the role as Managing Director of the Swedish Association of the Pharmaceutical Industry (LIF), including representation at industry's European and international associations EFPIA and IFPMA. Daniel VASMANT, M.D.,PhD, graduated as MD specialized both in Pediatric and Nephrology in 1980. From 1980-1984 Dr Vasmant has been Assistant Professor of pediatric nephrology in the Teaching Hospital Trousseau in Paris and Visiting Professor at the University of Washington in Seattle during the summer 1983. In 1984 he graduated in clinical pharmacology and in 1991 as Ph.D. of General Management. Dr Vasmant has been Professor at the Faculty of Medicine Saint Antoine, University Paris VI from 1992-94, where he created the University Diploma for Pediatric Clinical Pharmacology. Dr Vasmant is since 2001 the Scientific Relation Director for Public Affairs of the Aventis Pharma SA Sanofi Aventis Group. Furthermore he is Vice-Chairman of the EFPIA Paediatric Priority Action Team, Chairman of the LEEM Paediatric task force, Member of the LEEM CME/CPD task force, President of the National Meeting for clinical pharmacology at Giens, Founding member of the French Society for Pediatric Research and Vice-President of the French Association for Pharmaceutical Medicine AMIPS. Hans Asbjørn Holm, MD PhD, is Director of the Department of professional affairs, in the Norwegian Medical Association (NMA), where he held since 1985 various positions related to medical education and quality improvement. Dr. Holm was trained as an internist and worked within gastroenterology and haematology, where he did his PhD on studies of heparin treatment of deep venous thrombosis. He has been active in medical education since he graduated from medical school in 1967. He is coeditor of the book Learning in Medicine 1993 and was guest-editor of a series of articles on continuing medical education in British Medical Journal in 1997. He wrote the section on Postgraduate Training in the International Handbook of Research in Medical Education published on Kluwer Academic Publishers in 2001. Dr. Holm was a board member of the Association of Medical Education in Europe 1996-99. He served as Vice-President of the European Union of medical Specialists (UEMS) 1996-2002 and chaired the Subcommittee of medical training, continuing professional development and quality improvement subcommittee of the Standing Committee of European Doctors (CPME) 1998-03. Since 1999 he has been member of the International Advisory Board of the European Forum on Quality Improvement in Health Care. Magnus Sederholm, MD, PhD is holding the position as CEO of The Institute for the professional development of physicians in Sweden, IPULS, since the establishment in 2002. The institute is owned by three pivotal organisations in Swedish healthcare - The Swedish Society of Medicine, The Swedish Medical Association and The Swedish Federation of County Councils. Dr Sederholm graduated from the Karolinska Institute 1973. With a clinical background in cardilogy, he presenteted his PhD thesis in 1984. Before joining IPULS, he has spent 20 years in leading positions in the pharmaceutical and medical device industry, and has founded two medical companies, one of which has been introduced on the Swedish stockmarket.

Christos A. PISSIOTIS, M.D., F.A.C.S. is Professor of Surgery at the Athens School of Medicine and Head C of the Department of Surgery at the Hygeia General Hospital since 1977. In 1980 he became a Fellow of the American College of Surgeons. Prof. Pissiotis is also visiting Professor at the Univ. of Illinois School of Medicine, Univ. of Wisconsin School of Medicine, Univ. of Montreal School of Medicine, and the Univ. of Tulane L.A. School of Medicine. He has been Chairman of the Dept. of Surgery at the Evangelismos General Hospital 1989-1996. Prof. Pissiotis is active in several Medical Societies and Organisations in which he held different mandates, from which Chairman 1990-1993 of the Committee on Undergraduate Medical Education (A.C.M.T), Vice Chairman 1996-2000 of the CPME Committee of Medical Training and Continuing Medical Education and since 2000 Member of EACCME (European Accreditation Council for Continuing Medical). Dimitrios A. LINOS, M.D., F.A.C.S. has been awarded M.D. at the Athens Medical School in 1975. From 1975-1981 he pursued his professional education as Clinical and Research Fellow in the USA where he became diplomate of the American Boards of Surgery in 1981. From 1981-1983 Prof Linos has been Assistant Professor in Surgery in the Case Western Reserve University, U.S.A. Currently Prof Linos is Associate Professor in Surgery at the Athens Medical and Lecturer and Consultant in Surgery at the Harvard Medical School in Boston, USA. Since 1994 he is Director of the 1st Surgical Clinic HYGEIA Hospital, Athens, Greece. Prof Linos is active in 21 Medical Societies and Organizations, in which he holds different mandates from which Past and Present President of the Hellenic American Medical Society, Honorary Member of the Turkish Association of Endocrine Surgeons and member of the Prolepsis Managing Board (Institute of Preventive Medicine, Environmental and Occupational Health in Athens). Hannu HALILA, M.D., PhD studied at the University of Helsinki and became a licensed physician in 1983. He did his PhD thesis on ovarian cancer in 1987 also at the University of Helsinki. Dr Halila was an Associate Scientist at the Wistar Institute of Anatomy and Biology, Philadelphia, USA in 1987-88. He received his specialist degree in obstetrics and gynecology in 1993. Since 1993 he has been first Head of the educational department and since 2003 Director of Education and Research at the Finnish Medical Association. He has also been a private gynecologist since 1993. Dr. Hannu Halila has been the Finnish representative to the ACMT (Advisory Committee of Medical Training) in 1996-2001. He was a board member in the Nordic Federation of Medical Education in 1994-1997. Dr. Hannu Halila has been the Finnish representative to the UEMS (European Union of Medical Specialists) Management Council since 1994. In October 2002 he was elected President of UEMS for a three year term. Since 1999 he has also been member of the Advisory Council for EACCME (European Accreditation Council for CME). Bernard GREWIN, M.D., graduated from his medical training at the Karolinska Institute in 1973. He is a qualified specialist in Internal medicine as well as in Rheumatology. He holds a clinical position as consultant in rheumatology at the Karolinska University Hospital in Stockholm. Dr. Grewin has held several trustee positions within the Swedish Medical Association. He was a Board member of the local medical association in Stockholm 1982-1996. He also served as a Board member of the Swedish Association of Senior Hospital Doctors 1992-1998 being its Chairman 1996-1998. He was a member of the Board of the Swedish Medical Association since 1996 and served as the President of the SMA from June 1998 - May 2004. As President of the SMA he was also Chairman of the SMA s Delegation for Training and Research and of the Foundation for Quality in Medical Training as well as a Board member of the Swedish Health Technology Assessment Agency. He holds a position on the Board of the IPULS (Institute for the Professional Development of Physicians).

Quality assurance is a way for the individual doctor to demonstrate that the medical competence is up-dated in a proper way. 2. Quality Assurance should be an integrated part in the CPD process (CPD=Continuing Professional Development). CPD follows after formal qualifications have been obtained. The methodology for acquiring knowledge is based on the educational principles characteristic of adult learning, including self controlled learning, problem orientated learning, teamwork and on the job learning. 3. The process of Quality Assurance should be target orientated. All medical specialists should plan for CPD in peer dialogue and keep logbooks for planned and completed education. 4. Quality Assurance in the process of personal development can best be visualized through personal development plans which are drawn up, implemented and followed within the framework of the organisation. 5. CME-credit points is an insufficient instrument to measure Quality Assurance and Quality Control (CME=Continuing Medical Education). A high score in the meaning of many CME-points carries the risk of giving the false impression of high quality. The score usually indicates the extent of education in hours, and is therefore only a measure of time spent. 6. The definition Quality Assurance should be kept apart from Quality of current practice. The later focus more on how National Authorities organize medical care to guarantee a sufficient patient volume to create a learning environment and guarantee adequate experience within the organisation. 7. AEMH believes that if the medical profession focus on a well functioning CPDprocess combined with Quality Assurance, Quality Control will not be necessary. Quality Control and its variety of obligation or recertification is expensive for the society and calls for heavy bureaucracy. 8. There is today no evidence that recertification or revalidation methods are helpful in the early detection of incompetent / underperforming doctors.
