1 Getting Started In order to begin service you will need to send an initial reading of your heart. Prior to recording and transmitting that reading proper skin preparation is vital. 1 Skin Preparation - Getting Started Ensure that skin is clean, dry and free from any lotion, powder, or oil. This may help the electrodes fi rmly adhere to your skin, and allow your heart readings to appear clearer to CardioNet s Monitoring Center. Wash/Shave: Electrodes are placed in the shaded areas pictured Wash and dry these areas Do not use powder or lotion If you have chest hair, shave these areas Please Note: Do not use the adhesive remover wipes prior to placing electrodes on your skin. These should only be used to remove excess adhesive from your skin after removing electrodes if needed. PAGE 2

1 Skin Preparation Electrodes: Remove electrode pack from the supply bag. Tear open electrode pack and remove 2 electrodes. - Getting Started GETTING STARTED RECORDING & TRANSMITTING Lead (Wires): Snap the lead (wires) end onto the 2 electrodes THINGS TO KNOW Insert Plug: Insert lead (wires) plug into side of the Monitor FREQUENTLY ASKED QUESTIONS PAGE 3

1 Attaching Electrodes - Getting Started White (Right) Peel the adhesive backing off the white electrode. Place the white electrode about 3 fi ngers width below your right collar bone. Red (Left Side) Peel the adhesive backing off the red electrode. Place the red electrode on the lower left side of your rib cage. PAGE 42

1 Inserting Battery :) Take A Break Before proceeding to the next step, wait 10-15 minutes while your electrodes fully adhere to your skin. This increases the likelihood of a successful initial reading. 1. Open the battery door on the back of your Monitor, by pressing down and up on the battery compartment - Getting Started GETTING STARTED RECORDING & TRANSMITTING 2. Install the battery pack provided in your kit into the Monitor 3. Ensure that the battery orientation is correct, use the plus (+) and minus (-) as a guide THINGS TO KNOW 4. 2 Recording & Transmitting Any ECG recording is categorized as an event ; the fi rst event recorded is considered a baseline, because it is the initial reading of your heart. Sending an initial reading, allows CardioNet to have a baseline reading that can be compared to symptomatic ECG recordings. 2 Recording a Baseline - Recording & Transmitting Step 2: Scroll down to the bottom of the SELECT SYMPTOMS screen and select Done by pressing the Record/Send button. Step 3: Scroll down to the bottom of the SELECT ACTIVITY screen and select Baseline by pressing the Record/Send button. Once selected a check mark will appear to the left. Scroll down and select Done by pressing the Record/Send button. 2 Recording a Baseline - Recording & Transmitting Step 4: Remain still for roughly 30 seconds while ECG data is being gathered. Please Note: A baseline will automatically be sent to CardioNet s Monitoring Center. CardioNet will contact you if additional recordings are needed. Your Monitor will detect, record, and transmit your heart arrhythmias automatically. MONITORING.. 20 99% 1 Recording Events: It is important that you record events when you are feeling symptoms. 2 Transmitting an Event Transmitting Events: - Recording & Transmitting GETTING STARTED One of the primary benefi ts of your Monitor is that it transmits events automatically by using a built in cellular phone. If you are in an area will poor cellular service the following will occur: The Monitor will attempt to transmit data once every fi ve minutes, three attempts total If the Monitor continues to be unsuccessful you will hear a tone, much like a phone ring You can contact CardioNet to transmit these events over the telephone If you choose to wait your Monitor will attempt to transmit the information after 60 minutes RECORDING & TRANSMITTING Transmitting Events Manually: To transmit data manually, follow the instructions below: 1. Remove lead wires from Monitor 2. Place the Monitor on a fl at surface with the speaker facing up 3. Using a landline telephone contact CardioNet at 1-877-929-0700 4. CardioNet will instruct you on how to transmit 5. Once you have fi nished transmitting the data reinsert the lead wires THINGS TO KNOW FREQUENTLY ASKED QUESTIONS PAGE 11

3 Key Tips and Reminders - Things to Know Here is a list of all the things you will need to know and do while on service with CardioNet. 3 Concluding Service Returning the Device - Things to Know You will receive an automated reminder call about a week prior to your service completion. Upon completion return the CardioNet Monitor to CardioNet s distribution center immediately. Place the device and unused supplies back in the CardioNet kit Place the kit in the shipping bag provided in your kit and drop off at any UPS location GETTING STARTED RECORDING AND TRANSMITTING Please Note: Your device cannot be returned to your Physician, it must be sent to CardioNet directly. 4 CardioNet Service FAQ - Frequently Asked Questions What happens if a lead wire falls off? You will hear a tone. Check to see that the leads are fi rmly attached. Your leads may be attached to the electrode, but the electrode may not be fully adhered to your skin. Rub the electrodes in a circular motion, if the problem continues change your electrodes. Why is my Monitor making noise? There are several reasons your Monitor will make noise. Aside from recording events, here are the most common reasons: 1. A lead wire is disconnected 2. Monitor recordings need to be transmitted 3. Battery change needed 4. Text message received Can I shower with the Monitor on? No, the Monitor is not waterproof and will be damaged if exposed to excess moisture. Before showering, bathing, or participating in any aquatic activity. Unplug the lead wires from the Monitor Remove the lead wires from the electrodes When you are ready to reconnect: Attach the lead wires to the electrodes (change the electrodes if they are loose or falling off) Insert the lead wires into the Monitor When do I record events? Record events when you are feeling symptoms. PAGE 18

4 CardioNet Service FAQ How often should I change the electrodes? Change the electrodes every 2-3 days or more frequently if they no longer stick to your skin. Use soap and water and gently lift the old electrodes from your skin When you replace your electrodes, do not put new electrodes in the same locations each time Please refer to the suggested alternate locations in the illustration Please Note: It is very important that you move them from the original locations to protect your skin. - Frequently Asked Questions Can I use my cell phone to transmit data? ECG data is transmitted more clearly over a landline telephone. Some cell phones will allow data to go through and some will not. It is better if you call from a landline phone. Can I travel with the Monitor? Yes you can. If travelling by air, do the following: 1. Unplug lead wires from Monitor 2. Unsnap lead wires from electrodes 3. Remove battery from the Monitor Once you have landed and are ready to reconnect: 4. Plug lead wires into Monitor 5. ASSIGNMENT OF BENEFITS I request that payment of authorized health insurance benefi ts, including Medicare benefi ts, if I am a Medicare benefi ciary, to be made on my behalf to CardioNet for any medical services provided to me by CardioNet. I authorize any holder of medical and/or insurance information about me to release to CardioNet, my health insurance carrier, or the Centers for Medicare and Medicaid Services (CMS) any information needed to determine these benefi ts or the benefi ts payable for related services provided under this agreement. This assignment includes all dates of services rendered by CardioNet for all insurance plans. A copy of this authorization will be sent to CMS or my health insurance carrier if requested. The original will be kept on fi le by CardioNet. I understand that I am fully responsible to CardioNet for any co-payments, co-insurance, deductibles, payments made directly to me by my health insurance carrier for CardioNet services, and, when allowed by law, services not-covered or payable under my health insurance plan. I also understand that by signing this form and/or accepting these terms electronically, I am accepting fi nancial responsibility as explained above for all payment for services received from CardioNet. By signing this document and/or accepting these terms electronically, I acknowledge that I have received a copy of CardioNet s Notice of Privacy Practices. This acknowledgement is required by the Health Insurance Portability and Accountability Act (HIPAA) to ensure that I have been made aware of my privacy rights. NOTICE OF CONFIDENTIALITY AND PRIVACY PRACTICE The privacy of your personal and health information is important to us. Please review it carefully. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Protecting Your Personal and Health Information CardioNet, Inc. understands the importance of keeping your personal and health information private. Personal and health information includes both medical information and individually identifi able information, such as your name, address, telephone number or social security number. This is a notice of confi dentiality and privacy practices, our legal duties and your rights concerning your personal and health information. CardioNet protects your personal and health information in electronic, written and oral forms when used throughout our organization. In accordance with State and Federal law, below are the company s privacy practices. While many of these practices are already in place, certain provisions* take effect April 1, 2003, and will remain in effect unless otherwise replaced or modified. We may modify or change our privacy practices from time to time, particularly as new laws and regulations become effective. Any changes will be effective for all the personal and health information that we maintain, even information in existence before the change. CARDIONET S USES AND DISCLOSURES OF YOUR PERSONAL AND HEALTH INFORMATION CardioNet may use and disclose your personal and health information, without your authorization, only in the following ways: Treatment: We may disclose your personal and health information to a provider who requests this information to treat you or for the coordination or management of healthcare services that we provide to you. Payment: We may use and disclose your personal and health information to submit claims for payment for covered services provided to you. Health Care Operations: We may use and disclose your personal and health information to conduct quality improvement activities, to engage in care coordination and case management, to conduct business management and general administrative activities, and other similar activities. Health & Wellness Information: We may use your personal and health information to contact you with information about health related services or appointment reminders. If you do not wish to receive this type of information, you may request to opt-out of receiving this information by sending an email to privacy@cardionet.com or calling the phone number below. Family and Friends: We may disclose your personal and health information to a family member, friend or other person if you are unavailable to agree such as in a medical emergency or disaster relief. We will disclose this information only to the extent necessary to help with your health care or with payment for your health care. Research; Death; Organ Donation: We may use or disclose your personal and health information for research purposes in limited circumstances. We may disclose the personal and health information of a deceased person or to a coroner, medical examiner, funeral director or organ procurement organization for certain purposes. Public Health and Safety: We may use and disclose your personal and health information to the extent necessary to avert a serious and imminent threat to your health or safety or the health or safety of others. We may disclose your personal and health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect, domestic violence or other crimes. Required by Law: We will use or disclose your personal and health information when we are required to do so by law. Process and Proceedings: We may disclose your personal and health information in response to a court or administrative order, subpoena, discovery request or other lawful process. Law Enforcement: We may disclose limited information to a law enforcement official concerning the personal and health information of a suspect, fugitive, material witness, crime victim or missing person. With Your Written Authorization: We may not use or disclose your personal and health information without your written authorization, except as described in this notice or as otherwise required or allowed by applicable law. In the event that the company asks for your authorization to use or disclose your personal and health information, we would provide you with an appropriate authorization form. Once you ve given us a written authorization, you can revoke that authorization at any time. Your revocation will not affect any use or disclosures permitted by your authorization while it was in effect. INDIVIDUAL RIGHTS Access: You have the right to access your personal and health information. You must make a request in writing to obtain access to your personal and health information. You may obtain a form to request access by using the contact information listed at the end of this notice. You have the right to obtain copies of your personal and health information, with certain exceptions. You may request that we provide copies in a format other than photocopies. Please note that if you request copies, we will charge you a fee for each page. * Disclosure Accounting: You have the right to an accounting of disclosures of your personal and health information made by CardioNet. You can request, in writing, that we provide you with an accounting of instances when CardioNet or our business associates disclosed your personal and health information for purposes other than treatment, payment, health care operations, upon your written authorization, and certain other activities. We will begin maintaining disclosures for up to six years starting April 1, 2003. If you request this accounting more than once in a 12-month period, then we may charge you a reasonable, cost-based fee. * Restriction Requests: You have the right to request restrictions on the use and disclosure of your personal and health information. We are not required to agree to these additional restrictions, but if we elect to do so, we will abide by our agreement (except in an emergency). * Confidential Communication: You have the right to receive certain communications confidentially.
