JOB DESCRIPTION Job Title: Division/Department: Responsible to: Accountable to: Specialist Nurse - Asthma (Paediatrics) Children s Specialist Community Nursing Service (CSCNS) Shabnam Sharma - General Manager for Children Services, Ealing Kay Larkin Lead Nurse Band: 7 Hours: 37.5 Location: Ealing Hospital In order to meet the needs of the Trust s services you may be required from time to time to work at different locations to your normal place of work. JOB SUMMARY: The aim of this post is to deliver community-based paediatric asthma support that raises awareness, improves care and helps to reduce admissions and readmissions for children with asthma in Ealing. The post holder will facilitate multi-professional working across the community, primary and hospital sectors. The post holder will provide expert professional advice to patients, carers and professionals. This will include: o Developing and maintaining clinical guidelines, protocols and policies in relation to paediatric asthma. o Leading by example as an expert practitioner. o Providing education, training to staff and clients o Ensuring maintenance of clinical excellence. o Work as part of a multidisciplinary team across acute, primary and community care settings. To work closely with local schools, GP surgeries and community organisations to improve children s inhaler use and ensure treatment complies with best practice. To be accountable for the local delivery of relevant Key performance Indicators (KPIs) Please Note: The role may change subject to discussions with Ealing CCG around future service developments 1

KEY RESPONSIBILITIES The Asthma Specialist Nurse has a clear focus on improving the care of children and young people and to support them and their families to manage their asthma from diagnosis to transition to adult services. ORGANISATIONAL 1. Assess, plan, implement and evaluate all the programmes of care, actively seeking research findings that will support and promote clinical evidence based practice 2. Ensure asthma care plans are in school for education staff to know how to manage children if they have an asthma attack during the school day 3. To promote and monitor, the safe and appropriate use of paediatric asthma therapies within the Trust, based on clinical need. 4. To lead the planning of paediatric asthma service in the community and therefore be responsible for the caseload ensuring the KPIs are met and the hospital discharge care bundle is delivered 5. The post-holder will be responsible for reviewing and updating all protocols and standard operating procedures for the community based paediatric asthma service, promoting evidence based best practice. 6. Initiate community Clinical Audit to ensure consistently high standards of care are maintained. 7. To support the Itchy Sneezy Wheezy project led by Northwick Park Hospital to ensure a seamless service 8. Be responsible for dealing with all clinical and dosing queries raised by the community providers. 9. Put policies in place to ensure all patients have access to equal safe treatment within the service at all times. 10. Setting, monitoring and maintaining acceptable standards of care within the service provided. 11. Producing and reviewing Clinical Guidance documents/ Patient Group Directions/ Policies and Protocols as necessary. 12. To monitor National initiatives and guidance, implement, review and update the Trusts paediatric asthma policies and protocols. 13. Create and maintain accurate records of care/ treatment monitor the quality of care and action audit information to achieve agreed standards. 2

14. To act as a resource for queries and concerns about paediatric asthma related practice. 15. Responsible for dealing with all clinical queries in relation to the asthma support provided. 16. Ensure regular review of care as part of the Trusts Clinical Governance processes. 17. The post holder will be responsible for demonstrating the effectiveness of the service both in terms of clinical care, patient satisfaction and data EDUCATION AND RESEARCH 18. Take major role in the delivery and supply of training and education, ensuring that agreed guidelines and protocols are followed. 19. The post holder will ensure that their practical and theoretical skills are kept up to date enabling the sharing of knowledge and teaching of all staff in a formal and informal setting. 20. Ensure systems are in place for the health education of patients and relatives, taking every opportunity to promote a healthy lifestyle. 21. To identify the educational needs of the multi-professional team and to facilitate and participate in the education provision to meet those needs. 22. Act as an educational resource and communicate all new developments to all Multi-Disciplinary Teams 23. To act as a preceptor / mentor to students or other staff as directed COMMUNICATION 24. Build strong influential relationships with service leads, clinical governance leads e.g. consultant paediatricians, clinicians and senior managers in the organisation. 25. Communicate to all relevant staff within the Trust on the asthma service ensuring clarity on the discharge care bundle, expected outcomes and follow up criteria 26. Maintain regular communication with colleagues locally, regionally and nationally, regarding accreditation and appraisal frameworks. 27. Ensure robust feedback is given on the service to all relevant staff within the Trust on the key target work including CCG 28. To communicate effectively with all members of the Trust including GP s. MANAGERIAL 3

To undertake an analysis of the key issues affecting the health of the asthma population and their health needs analysis. 35. Contribute to policy and service reviews as required. 36. Encourage sector wide working with key people in the health community to scope areas for improvement. 37. Maintain knowledge of relevant NHS and social care policies. 38. Work within the policies and guidelines set out by the Trust on the Intranet and any relevant codes of conduct. 39. Be responsible for the health, safety and welfare of self and others in the working environment to follow safe working practices, and to comply at all times with the Trust s Health and Safety Policies and Procedures. 40. Ensure confidentiality in all matters relating to patients and information obtained during the course of employment. 41. Participate in the Trust s staff development and mandatory training schemes. ADDITIONAL RESPONSIBILITIES INFORMATION GOVERNANCE All NHS workers must abide at all times by the Confidentiality: NHS Code of Practice document issued by the Department of Health, and follow the relevant confidentiality and privacy policies specifically adopted by the Trust. Information relating to patients, employees and business of the Trust must be treated in the strictest confidence and under no circumstances should such information be discussed with any unauthorised person(s) or organisations. All information collected, stored and used must be done so in compliance with the Data Protection Act, the Freedom of Information Act (2000) and all relevant Trust Policy. Breaches of confidentiality or information governance protocol may lead to disciplinary action. INFORMATION SECURITY 4

All staff must adhere to the requirements of the Trust s Information Security Policy, which covers the deployment and use of all of the Trust s electronic information systems (i.e. all computers, peripheral equipment, software and data). In serious cases, failure to comply with the Policy may result in disciplinary action and could also result in a criminal offence. HEALTH AND SAFETY AT WORK Act (1974) You are required to take reasonable care for your health, safety and welfare and that of other people who may be affected by your actions or omissions. These responsibilities apply at all times whilst you are at work or on duty and apply to all Trust premises and also whilst working in the community or on any other Trust business. INFECTION CONTROL AND HOSPITAL-ACQUIRED INFECTION Infection Control is everyone s responsibility. All staff, both clinical and non-clinical, are required to adhere to the Trust s Infection Prevention and Control Policies and make every effort to maintain high standards to infection control at all times thereby reducing the burden of Healthcare Associated Infections including MRSA. In particular all staff have the following key responsibilities: Staff must wash their hands or use alcohol hand rub on entry to or exit from all clinical areas and between each patient contact. Staff members have a duty to attend infection control training provided for them by the Trust. Staff members who develop an infection that may be transmissible to patients have a duty to contact Occupational Health. SAFEGUARDING CHILDREN AND VULNERABLE ADULTS We all have a personal and a professional responsibility within the Trust to identify and report abuse. The abuse may be known, suspected, witnessed or be limited to raised concerns. Early recognition is vital to ensuring the patient is safeguarded and any other people (children and vulnerable adults) who may be at risk. The Trust s procedures must be implemented, working in 5

partnership with the relevant authorities. The sharing of information no matter how small is of prime importance in safeguarding children, young people and vulnerable adults.
