2015 Year Meaningful Use Checklist The attestation period for Meaningful Use Year 2015 is January 4 to March 11, 2016. Here are some helpful tips to assist you: 1. The PCM MU report card updates daily. Run your report card OFTEN to confirm that you are passing attestation requirements. 2. Need additional assistance with MU requirements? Please attend our LIVE MU Workshop with Q & A or view our Recorded MU Workshop. 3. Practice entering your MU information on your Attestation Worksheet prior to submitting your attestation. 4. Have you registered? If not, please click here for Medicare or here for Medicaid. 5. Ready to attest? You can find the attestation login page here. 6. Confirm on your MU report card you have achieved all Meaningful Use requirements: 10 Objectives, unless you qualify for exclusions. 2.29.16 2

7. Take screen shots or print out any necessary items from Patient Chart Manager on the day you complete your CMS Attestation. Save a dated copy of this information. a) MU Report Card 8. If you are claiming an exclusion to either of the public health measures (Immunization Exchange or Syndromic Surveillance Exchange) be prepared with documentation that you are excluded from both measures. a) If you are claiming an exclusion because your county is currently not accepting Immunizations or Syndromic Surveillance Exchange from EPs, please contact their office to obtain a letter to keep on file in case of a future audit. 9. Security Risk Analysis, Objective 1 (Protect Patient Health Information): a) Please complete and keep a record of your documents for reference. Documents to complete this measure are found on our website > Meaningful Use Tools > Modified Stage 1 & 2 MU. 10.CQMs: a) Run your PQRS Module in PCM and submit your CQMs along with your attestation. Please keep in mind, running each report in PCM can take a few days to a week or more to run. NOTE: CMS requires that you keep all dated documents on file for 6 years 2.29.16 3

Stage 1: Eps (Eligible Professionals) Objectives & Measures for the EHR Incentive Programs 2015 2017 for EPs in Stage 1 in 2015 Certain measures include alternate exclusions and specifications to allow providers who were previously scheduled to be in Stage 1 EHR reporting period for 2015 to use a lower threshold for certain measures; or to allow providers to exclude Modified Stage 2 measures in 2015 for which there is no Stage 1 equivalent. 2.29.16 5
