Purpose The Community Support Worker actively supports and promotes a quality of life for people with a disability by providing quality services using person centered approaches that focus on developing a person s potential to participate as independently as possible within their home and the community. Community Support Workers are employed under the Social, Community, Home Care and Disability Services Industry Award 2010. Sunnyfield Community Support Workers can work in the following programs : o Community based group homes, drop in support services, congregate care facilities, day programs, respite, centre based respite services, and Attendant care program o work a varied roster of shifts which may include weekends and overnight Community Support Worker s employment is with Sunnyfield, inclusive of all Sunnyfield services. Current services are based in Northern Sydney, Western Sydney, Central Coast, Lower Hunter, New England areas and the ACT. Community support Workers may be requested to work in more than one service or transfer to a different service should the need arise. Community Support Workers may be requested to support clients on holidays, this could be nationally and internationally. Non-Government organisations. Allied Health Care Professionals. Generic community services Position Dimensions Community Support Workers report to a Service Manager or Service Coordinator. Relevant Experience Previous experience within a human services environment preferably supporting children or adults with disability or people who are ageing. Experience in the daily operation of a household e.g. meal preparation, cleaning, vacuuming, shopping, organising resources, washing, ironing, transporting people to community activities, participating in recreation programs with others. Knowledge & Skills Understanding of service provision for people with a disability Effective written and spoken communication Demonstrated experience in using standard computer applications Effective time management skills within competing priorities Demonstrated ability to work independently and as an effective team member. Exhibit a personal commitment to service, integrity and professional development. Awareness and acceptance of NSW Disability Services Act 1993, NSW Disability Service Standards, Disability Services Act 1991 (ACT), Disability Services Regulation 2014 (ACT), Disability Services Approved Standard 2014 (ACT) and other relevant legislation Qualifications Current First Aid Certificate (desirable) Current Driver s Licence. Certificate IV in Disability Services (or willingness to obtain). Other Relevant Qualification Role and Responsibilities Clients 1. Uphold the rights and responsibilities of clients (adults and children) in line with the NSW Disability Service Standards and Disability Approved Standard 2014 (ACT), relevant legislations and organisation policies and procedures. 2. 3. Assist clients with their personal care needs as required i.e. mealtime, toileting personal hygiene and medication. 4. The position requires heavy lifting, manual handling and repetitive work. 5. Encourage, support and assist our clients to develop and maintain family and community relationships. 6. Involve clients in decision making in every aspect of the service they receive. 7. Perform or assist in a key worker role as per organisational policy and procedure when required. 8. Transportation of clients to community facilities, events and medical appointments. 9. Follow all plans that are developed to provide support for health care, communication, meal time and any areas that require specific support plans. 10. Support clients using a positive approach to behaviours of concern. 11. Follow restrictive practice protocols in implementation and monitoring of these practices. 12. Assist clients to achieve their personal goals and increase their independence through support and participation in decision making. 13. Assist and prompt clients to maintain the home environment which will include completing and assisting with tasks if the person is unable to complete. 14. Ensure that quality service is provided to meet individual s specific health needs. 15. Assist clients to maintain a high level of personal hygiene, personal grooming and personal care. 16. Support and assist clients with personal medication as required in compliance with Sunnyfield Medication procedures. 17. Assist clients to maintain good nutrition. 18. Assist clients to safely manage their personal finances according to policy and procedure. 19. Promote a positive image of our clients and the organisation. 20. Participate in staff development training as required. 21. Support the advocacy process. 22. Maintain professional boundaries in line with the code of conduct. Ensure private information is maintained in a secure manner. Assist in maintaining a safe, clean and tidy workplace. Identify and report hazards, near misses and incidents in your area. If injured at work comply with your return to work plan and responsibilities identified in the Workers Compensation and Return to Work Procedures. Follow relevant policies and procedures to ensure quality service outcomes to clients. Communication Maintain positive communication with all stakeholders including our clients, families, guardians, carers, advocates and all statutory authorities. Communicate about issues affecting clients and any work related issues with Service Manager/Service Co-ordinator Promote a harmonious and supportive workplace. Maintain documentation as required. Ensure client confidentiality is respected and maintained. Other Roles and Responsibilities Comply with Sunnyfield s Code of Conduct Conduct all work related activities in accordance with Sunnyfield s Policies and procedures. Contribute positively to the day to day operation of the service, including household tasks and household decision making. Complete all tasks as set out in the duty rosters, daily support schedules, diary, and communication book or CRM. Attend all staff meetings and training as required. Complete other tasks as requested by Service Manager /Co-ordinator Key Worker Responsibilities A Key-worker is a Community Support Worker who is chosen by the client, in consultation with their circle of support and the Service Manager/Coordinator to advocate for and provide support on behalf of a specific clients. Keyworkers take a lead role in assisting the client to improve their quality of life. Assists the client to have regular Person Centered planning meetings, participating in these meetings and supporting the client to achieve their choices. Identify the changing wants and needs of the person and ensuring that these are addressed at regular Person Centred Review meetings. Provide regular reports to the Service Manager/coordinator and family, to address and feedback for residents meetings and to review what is working and not working so well for the person. To monitor and maintain client s belongings and to assist and or organise assistance for the person to purchase items as needed. Ensure all medical appointments are arranged as required. This includes completion of the annual medical review, following through on referrals, follow up appointments, medical tests as requested, medication monitoring and medication changes. Attend all medical appointments where practicable. If the keyworker cannot attend appointments a suitable permanent employee should be arranged by the service manager /cocoordinator.
