DEAR COLLEAGUE, 2 It is my pleasure to invite you to the 2014 Multi-Specialty Physician Conference. This event will be held February 5-9, 2014 at The Lodges in Deer Valley, Utah. This conference is designed to provide you with exceptional education and networking opportunities. The 2014 Multi-Specialty Physician Conference will provide a forum for physicians of various specialties to discuss the current trends and updates in various medical and surgical specialties. Addison, M.D. Course Director STATEMENT OF NEED Continued advances in technology and in the understanding of disease states have resulted in an ever changing healthcare environment. This conference is designed as a forum to assist physicians in learning about advances and new concepts in prevention, diagnosis and treatment of diseases. This conference will also address current or future healthcare issues and their impact on patient care. TARGET AUDIENCE & COURSE DESCRIPTION This conference is designed for physicians and healthcare professionals of all specialties and subspecialties with an interest in expanding their knowledge regarding medical and surgical updates. This symposium provides a forum in which physicians are able to discuss and share medical, surgical and general healthcare information among colleagues. EDUCATIONAL OBJECTIVES Specific educational objectives will be provided for each topic. After attending this conference participants should be able to: Discuss the etiology and treatment options for valvular disease. Explore the diagnosis and treatment options for various medical and surgical issues such as sepsis, allergy and sinus conditions and joint issues. Identify surgical options for obesity, including patient selection, follow-up and outcomes. ACCREDITATION Mississippi Baptist Medical Center is accredited by the Mississippi State Medical Association to provide continuing medical education for physicians. 4 COSTS Registration: $300.00 registration fee per Mississippi physician or healthcare professional. Entitles attendee, spouse or guest and children to participate in conference activities. $750.00 per out-of-state physician or healthcare professional, based on availability after January 6, 2014. Conference registration is limited to a maximum of 60 participants. Accommodations: Lodge/Hotel Room: $289.00 per night One Bedroom Condominium: $439.00 per night Two Bedroom Condominium: $619.00 per night Three Bedroom Condominium: $759.00 per night Note: Rooms at the Silver Baron are available at a higher rate. A block of rooms has been reserved at The Lodges at Deer Valley until January 6, 2014 and are available on a first-come basis. A three night minimum stay is required. Rate is available three days pre and post conference dates, based on availability. To reserve your room complete the Lodging Reservation Form which will be emailed to you after registration is received. All rooms include cable TV, wireless internet access, robes, hair dryers, coffeepot and mini refrigerator. All condominiums include a fully equipped kitchen, washer/dryer, living area with fireplace and two or three baths. Bedding varies in each unit. Special requests will be noted, but are not guaranteed. Deposit/Cancellation Policies: An initial deposit of $50.00 is charged at the time of booking followed by one night s room and tax charged three days later. Lodging rates do not include taxes and fees, including a resort fee, of 17.666%. The resort fee includes the following services: wireless Internet, fitness facility, on-demand shuttle service, heated underground parking, daily housekeeping, weekday newspaper delivery and gratuities for bell service, shuttles and housekeeping. Final payment is due 45 days prior to arrival. Reservations made within 45 days of arrival are charged full payment at time of booking. Individual cancellations made more than 45 days prior to arrival will forfeit one night s lodging plus $50.00. Cancellations made less than 45 days of arrival will forfeit total lodging cost. There will be no refund for late arrivals, early departures, no-shows or downgrades. Check-in time is 4:00 p.m. and check-out time is 10:00 a.m. Travel Insurance is available at a cost of 7% of total lodging and is non-refundable. Please indicate on the registration form if you would like to purchase insurance. Transportation: Attendees should make their own flight arrangements. Ground transportation is available from Salt Lake City International Airport through Park City Transportation at a discounted rate of $35.00 each way ($70.00 round trip) per adult and $26.00 each way ($52.00 round trip) per child under 12 years of age.
