PL07 PROGRAMME General information total allocation 217.600.286 PLN = 52 545 225 EUR min. project amount 1.552.950 PLN= 375.000 EUR max. project amount 31.059.000 PLN = 7.500.000 EUR 3 areas of intervention (1) Amelioration of perinatal care preventive services, diagnostics and treatment with a view to increase the number of births (2) Better adaptation of health care system in order to meet the needs of fast-growing population of chronically-ill and dependent elderly persons (3) Cancer prevention aimed at reducing cancer morbidity and mortality indicator in Poland

52 545 225 EUR for better diagnosis and prevention Already started the implementation of 34 Projects, which are carried out throughout the country, including rural areas. Most in the Malopolska province, Wielkopolska and Lower Silesia. Projects with partners - 4 The only project to be implemented via partnership with Norwegian partner is project Enlargement and recostruction of building no 10 designed for elderly and chronically ill patients [Norwegian partner - hospital owned by a non-profit organisation]

PL07 PROGRAMME Implementation of those projects will contribute to the improvement of perinatal care (diagnostics and treatment), to improve access to cancer prevention and to improve access to health care for the chronically-ill and dependent elderly persons. The largest group of beneficiaries are: medical entities, nongovernmental organizations, limited liability companies and local government units

EVALUATION AND SELECTION OF PROPOSALS Announcement of calls for proposals The Applicant number of submitted applications 585 Team for Formal Appraisal 457 have positively passed (including supplementations 405 and expertise - 90) Team for Content Related Appraisal Content Related Appraisal (submitted appelas 292, whereof 286 were approved and 6 were rejected (not subject to verification)

PROGRAM PL07 SELECTION COMMITTEE Draw up a ranking list of projects Projects were ranked according to the number of points they were granted Committee for the Selection of Projects. during the appraisal, based on criteria relating to: 1) healthcare aspects of the project and 2) financing and management of the project.

IMPLEMENTATION OF PROJECTS Contracts between Beneficiaries and Programme Operator were signed; Currently Programme Operator is busy working on advance payments; In January 2015 first payment claims for a period September-December are expected.

PL13 PROGRAMME PREDEFINED PROJECT RESTRICTED CALL FOR THE IMPLEMENTATION OF THE PILOT PROJECTS concerning health promotion and / or prevention of diseases in local communities PL13 PROGRAMME

PROJECTS SELECTED IN THE COMPETITIVE PROCEDURE ELIGIBLE APPLICANTS: Local goverment units Poviats PILOT PROJECTS: In the restricted call for proposals for pilots-projects was a possibility to apply for financing the implementation of community-based health promotion and / or disease prevention pilot projects consisting of: - prophylaxis programmes and/or, - educational actions on health topics.

PROJECTS SELECTED IN THE COMPETITIVE PROCEDURE GRANT VALUE: Grant rate (financing level) 100 % ELIGIBLE PROJECTS COSTS The minimum project value (total project eligible costs) is 300 000 EURO The maximum project value (total project eligible costs) is 1 000 000 EURO

CALL FOR PROPOSALS The restricted call for proposals in Programme PL13 lasted from 31 March 2014 till 02 June 2014. Total number of submitted applications: 98. 1 application have been withdrawn by Project Promoter Average project value: 2 603 054 PLN i.e. 618 185 EUR (Exchange rate: 4.2108 PLN /1 EUR) 5 projects have been rejected after fomal assessment procedure 92 projects were the subject of content related appraisal

SELECTION COMMITTEE 27.11.2014 The Selection Committee made a recommendation to financing 24 projects with a total value of 63.081.547 zł; The remaining allocation amount of 1.862.489 zł is not sufficient to finance the cost of the next project from the ranking list which was submitted by the District of Kutno; The Committee recommended the project for funding conditionally, if the Minister of Health agrees to increase the share of national resources in its financing above 15% total eligible costs.

IMPLEMENTATION Signing the project agreement by the Project Promoter and the Programme Operator Reporting system: Payment claims Final report Payment claims: reporting periods: January - April; May - August; September - December. Payment claims include financial and and descriptive part. Expenses are presented in PLN

CALLS FOR PROPOSALS During the 1st call for applications at the PL07 programme level we have received 5 projects in total, 2 have received funding: 1) Vascular disease in rheumatology Pomeranian Medical University (60726 PLN of EEA, Norway, Grants, MoH financing, 10717 PLN of own resources; in total 71 443 PLN of eligible expenses), 2) TeleNRS Care Medical University of Łódź (83011 PLN of EEA, Norway Grants, MoH financing, 14649 PLN of own resources; in total 97660 PLN of eligible expenses).

CALLS FOR PROPOSALS Until November 28th there were two ongoing FBR calls: 2nd call within PL07 to the amount of 754 812 PLN and 1st call within PL13 to the amount of 272 571 PLN (65000 EUR). These calls have concluded on Nov 28th. We have so far received three applications for PL07 FBR. We are looking forward to receiving PL13 applications as well (two are on its way)
