The MassMutual Matching Gift Program is designed to recognize and demonstrate appreciation for the work carried out by nonprofit organizations throughout the United States. The program serves to encourage and match eligible participants charitable contributions to educational, health care, environmental, arts, cultural, community, civic, and social service organizations. As a national company, MassMutual is committed to providing support that enables non-profit organizations to better serve their local communities. We encourage participation in this effort through the Matching Gift Program. Under this Program, MassMutual will match personal contributions of $25 or more by eligible participants to a qualifying organization at a 1:1 ratio based on calendar year limits up to an annual company maximum. How do I participate in The MassMutual Matching Gift Program? Please read the Matching Gift Guidelines (see online, home page) to find out more about eligibility. There is also additional information noted below. How do I make a Donation and Register for a Matching Gift? You are able to donate via credit card and request a Matching Gift through the online Matching Gift Program. If you would prefer to write a check donation to the non-profit please send your check to the non-profit and then enter your match request online (instructions below). How to access the Matching Gift Program access is through single sign on through MX (Mutual Exchange), Employee Tools or FieldNet. Employees through Mutual Exchange: 1

Enter Matching Gift in search criteria on right of home page and click on the Matching Gift Program link. or 2. Go to the Marketing tab, then Why MassMutual and then Community Responsibility and expand the Matching Gift area and click on the Matching Gift Online site within the text. How do I register for a matching gift? You can register for a matching gift through the online system (instructions below). Online Registration Send your gift to the qualified organization of your choice. 1. Enter the matching gift online site employees through MX (Mutual Exchange) and Field representatives through FieldNet (see above how to access the matching gift program online) 2. From the home page - check to make sure your e-mail address is updated under my account. If work e-mail does not allow you update or change enter it under preferred e-mail. Click update to save. 3. Click on the Make a Donation and/or request a Matching Gift button on the left side of the page. 4. Click on either blue button Donate by credit card and request a matching gift or Request a Matching Gift for a Donation already made by check, cash or credit card 5. Search for the appropriate organization and complete the requested information. 6. If you have given to the organization in the past and requested a matching gift it should be included in your favorites 7. If you don t see the organization you want to support in the list it can be entered for consideration. a. Click on the Submit a New Organization button b. Enter in the legal name of the organization, street address, city, state, zip code and country. c. Click the Next button 8. After you have completed submitting your request. You can check your Giving History or check the Message Center at the top left of the page to see the status of your request. 9. Status are pending agency verification, approved, declined, all information has been received pay out date will be noted or paid. The Matching Gift Center will validate your eligibility and verify your gift with the organization (this can take a while at times so please check Giving History or Message Center to check status of requests.) Once verified the status of the request will then change to approved and the request will be entered into the next quarterly payout Once approved, the matching gift will be sent directly to the organization during the next scheduled payout distribution mid-april, July, October or December. 2
Can I request a match on behalf of my eligible GA, agent or manager? If your GA, agent or manager has delegated to you to request a match on their behalf you can do so using the online system. You must be logged into the computer you are using as the manager or in FieldNet logged in as the GA or agent. Unfortunately, due to privacy issues delegates can no longer request matches on behalf of someone else over the phone. If the GA, agent or manager is unable to access a computer then they can call the Matching Gift center themselves to request the match at (855) 614-6117. How does donate by credit card work? Donations by credit card made through the Matching Gift system are not immediately paid to the non-profit. They are paid out on a monthly basis. Credit card information is not retained on the site. It is stored securely at the payment gateway and processor for your protection. Your credit card is charged right away. The charge will appear on your credit card statement as - DONATE WELL $##.## (donation amount); Donate Well is the vendor that processes the payments for our program. Who is eligible to participate in the MassMutual Matching Gift Program? Pugh Agency Building Award Winner - $10,000; GA s who are not in categories above - $7,500; Chairman s Club - $7,500; Top of Council - $3,500; Blue Chip Council - $2,000; Leaders Conference qualifiers* and Sales Managers/Brokerage Directors invited to Leaders Conference - $1,000 * (based on previous year s production and current year invite to Leaders Conf.) What types of organizations are eligible? Please take a few moments to review our Guidelines to see a sample of the eligible organizations in our program. Eligible recipient organizations must operate on a not-for-profit basis and be located in and serve the people of the United States. They must also be certified for tax-exempt status under Section 501(c)(3) of the Internal Revenue Code. Organizations eligible for matching may include: Arts & cultural organizations, accredited educational institutions (including primary, secondary, seminaries, theological, technical & specialized schools, two and four year colleges, universities and graduate and professional schools), community and civic organizations, hospitals & health care agencies, social service organizations and environmental organizations. Additional higher education-related eligible organizations include: 3
Alumni Funds (Not dues or memberships), Alumni Foundations and Alumni Associations under the administration of an eligible higher education organization. Except for public K-12 schools, all organizations must be tax-exempt under Section 501(c) (3) of the Internal Revenue Code. All organizations must agree to assist the Company in ensuring the Company s guidelines are met and fully cooperate with the Company regarding gift inquiries. Restrictions and ineligible organizations MassMutual WILLNOT contribute matching funds in these cases: Matching Gift requests registered after December 31 st of the year the donation was made Gifts made with funds provided to the employee for donation purposes by other individuals (i.e. pooled/grouped funds). Gifts to churches and religious organizations Gifts to political or fraternal organizations or athletic booster clubs Gifts to organizations that are not tax-exempt under Section 501(c) (3) of the Internal Revenue Code, or are otherwise ineligible. Gifts to individuals. Alumni dues, individual memberships or subscription fees for publications. Payments that cover the cost of services, tuition, books, or student fees. Ticket purchases or gifts where direct value is received. Gifts made with the primary intent to receive specific merchandise, such as school memorabilia, status auto tags, special edition publications, etc. Gifts to scholarship funds resulting in direct benefit to self or a family member. In-kind services, materials, supplies or pledges. Payments to satisfy legal obligations. Gifts to federated campaigns (i.e. United Way, United Negro College Fund, The American Indian College Fund, Hispanic Association of Colleges and Universities, etc.) Donations to donor advised funds (i.e. Schwab Charitable Fund, Fidelity Charitable Gift Fund) When is my gift matched? Your gift will be approved for a match when the Matching Gift Processing Center receives verification from the non-profit organization. The Matching Gift Center must receive verification within 180 days of the date the gift is registered or it will be declined for a match. Matching gift payments are made quarterly, in mid-april, July, October and December. For example, if you register a gift in January and it is verified by the non-profit organization in February, it will be matched in April. If your gift is not verified until April, it will be matched in July. How do I get a detailed history of my past gift applications? Log onto the online system and click on appropriate year Giving History tab. What are the annual matching gift limits? MassMutual distinguishes match limits for different levels of eligible associates as noted below. Is there a Cap on the Matching Gift Program? There is an annual cap of $1 million dollars in support of the program. Once that limit is expended, requests for matching funds will not be fulfilled What is a 501 (c) (3)? A 501(c)(3) is a section of the Federal Tax Code that establishes the criteria for tax-exempt charitable organizations. An organization must have this status in order to receive matching gift funds. This status allows donations and funds received to be tax-deductible. If my name changes, how do I update my profile? You must contact your Human Resources department and follow their procedures to change your name.
