2080 / 2100 REVISED: FEBRUARY, 2017 PAGE 1 OF 6 POSITION DESCRIPTION DEPARTMENT: TITLE: REPORTS TO: SUMMARY: Physicians Administration Registered Nurse Registered Nurse BSN (non-exempt) Clinic Manager Responsible for all Clinic Services Departments providing patient care. Provides supervision to all Clinic staff members. Is available to the physicians as an assistant in the many aspects of providing patient care, if needed. This position is responsible for performing front and back office tasks in accordance with established policies and procedures. The responsibility also includes providing nursing care by utilizing the nursing process to care for patients in the Clinics. Will do various patient follow-up call-backs. Demonstrates the ability to meet the needs and provide services to all age groups-infancy through geriatrics. Competent in providing services for the culturally diverse population Lodi Health serves. If assigned to any specialty focused clinic, refer to the major duties and responsibilities OR scope of care, i.e. Wound Treatment Center, OB Clinic, etc. specific to the specialty clinic. EDUCATION, TRAINING AND EXPERIENCE: MINIMUM REQUIRED: 1. Current and valid licensure as a Registered Nurse in California. 2. At least one year Medical-Surgical Nursing experience. 3. Excellent interpersonal skills. 4. BCLS Certification. 5. Keyboarding skills. 6. If assigned to Wound Center, must be willing to complete hyperbaric chamber training within six months of beginning Wound Care employment. 7. If assigned to the OB Clinic, must have one year perinatal experience within the past three years AND must be willing to complete Comprehensive Perinatal Services Program (CPSP) requirements set forth by the State of California. 8. Fetal monitoring certification in order to perform Non-Stress Tests (NST s). PREFERRED: 1. Minimum qualifications, and; 2. Bachelors Degree in Nursing. 3. At least three years of experience in an outpatient clinic operation. 4. Spanish speaking. 5. If performing antepartum testing such as Non-Stress Tests (NST s), must be currently competent and certified in Perinatal Fetal Monitoring. 6. Certification in Hyperbaric Technology or ability to acquire such certification in a reasonable period of time. 7. Certified as a Wound Care Nurse (CWCN) or certified as a Wound and Ostomy Care Nurse (CWOCN).

Page 2 of 6 MAJOR DUTIES AND RESPONSIBILITIES: 1. Maintains a clean and safe clinic environment with stocked exam rooms. 2. Provides patient education and documentation as appropriate and/or ordered at time of visit. 3. Documents interactions with patients and medical care provided appropriately including summary lists, medication lists, and assessment forms at time of visit. 4. Documents translator usage when appropriate. 5. Provides proper documentation on patient records and log sheets, when immunizations are given. 6. Attends staff meetings, staying updated on department information. 7. Performs control testing and point of service testing accurately and according to policy. 8. Collects information regarding patient assessments including pain assessments and patient s history. 9. Administer and documents IM, SQ, PO and IV medications and immunizations as necessary utilizing the 5 Rights. 10. Performs as an advocate for patients and a mentor to staff. 11. Demonstrates adherence to policies, procedures, standards of behavior, mission, vision, values, and customer service expectations. 12. Participates in Performance Improvement processes. 13. Understands and practices principles of Standard Precautions; wearing appropriate personal protective equipment when appropriate. 14. Completes and documents prescription renewals appropriately and accurately. 15. Works within the team promoting quality relationships and work environment. 16. Possesses cultural diversity skills, working with various cultures, languages, and social situations. 17. If assigned to the OB Clinic including the Galt site, using the nursing process of assessment, nursing diagnosis, planning, implementation and ongoing evaluation, provides direct patient care of the outpatient prenatal and postpartum patient. Demonstrates competence in technical, critical thinking, and communication skills in patient care to a culturally diverse patient population. 18. If assigned to the OB Clinic including the Galt site, must perform case coordination, i.e. scheduling patients, authorization management, lab review, client orientation, client assessments and reassessments, etc. for the obstetrical outpatient population compliant with CPSP in an independent manner. Conducts hospital and department business in an ethical and lawful manner, and willing to report any knowledge of real or potential fraud or abuse according to the Lodi Health Compliance Code of Conduct. 2. Will not use or disclose individually identifiable health information of patients over and above that which is minimally required to perform your assigned position responsibilities. WORKING ENVIRONMENT: Typical medical office environment. Works in clean, well-lighted, ventilated building. May require frequent trips to other departments within the hospital and/or between facilities. Exposure to weather when traveling between hospital buildings. Potential exposure to infectious diseases, blood-borne pathogens, and hazardous substances.

Page 4 of 6 SUMMARY: WOUND CENTER HYPERBARIC SCOPE OF CARE Under the direction of the Physician, Program Director, and the Clinic Manager as applicable, the individual in this position will act as the outside attendant and control operator during clinical treatment using monoplace hyperbaric (HBOT) chambers. The individual accepts responsibility and accountability for working within the framework of appropriate regulatory body where it applies and follows all Paradigm and wound care, and affiliate hospital policies and procedures. The individual will comply with hospital and nursing philosophy and standards while performing job duties and responsibilities and will demonstrate competence in these essential functions through the ability to perform functions, including, but not limited to, the common examples listed. REQUIRED QUALIFICATIONS: 1. One of the following qualifying licenses: Respiratory Therapist, Diver Medic, Physician Assistant, Corpsman, Medical Service Specialist, EMT, Paramedic, Registered Nurse, LVN/LPN, Physician, Certified Nurse Aide, Medical Assistant, Physiologist, Medical Researcher, or others approved on a case by case basis. 2. Certification in Hyperbaric Technology preferred or ability to acquire such certification in a reasonable period of time. 3. At least one-year experience in a patient care environment and/or diving/medical hyperbaric experience preferred. 4. Working knowledge of various control systems of the chamber or ability to acquire such knowledge in a reasonable period of time. 5. BCLS Certified. 6. Keyboarding skills. PREFERRED QUALIFICATIONS: 1. Reads, writes and speaks Spanish fluently. MAJOR DUTIES AND RESPONSIBILITIES: 1. Demonstrates knowledge and understanding of the Health Strategies of wound care and the role of HBO therapy, and the different approved clinical indications and treatment protocols for HBO treatment. 2. Delivers care to the patient immediately prior to, during, and immediately following the HBO treatment that is consistent with existing policies and Healing Strategies within his/her scope of practice and documents all aspects of that care. 3. Monitors and ensures the proper delivery of treatment gas as ordered. 4. Monitors the patients continually during treatments for symptoms of oxygen intolerance, claustrophobia, hypoglycemic reactions, decompression sickness or any other adverse reaction to HBO treatments. 5. Under the direction of the Clinic Manager/Program Manager or designee, appropriately intervenes in the event of an adverse reaction and follows established protocols. 6. Demonstrates competence in reinforcing age specific patient and family education on common topics related to the HBO therapy and related underlying disease processes.

Demonstrates competence in performing ordered treatments including but not limited to vital signs, blood sugars, and emergency procedures consistent with existing policies and within his/her scope of practice. 8. Documents required elements of each treatment in a clear, concise manner that includes a description of the chamber settings, duration of treatment, patient response to the procedure, and any other relevant observations made. 9. Demonstrates a high degree of skill in HBO therapy, chamber operations, basic EKG monitoring, and emergency care, taking initiatives and working independently in a fast paced environment. 10. Maintains the appropriate levels of competence through professional development activities. 11. Able to complete a two-person lift, wheelchair to exam table or bed to stretcher. 12. Ability to understand and properly apply various emergency procedures outlined in the chamber operation manual to ensure the safety of the patient. 13. Demonstrates working knowledge of various computer software including word processing and spreadsheet functions. CHAMBER OPERATIONS: 1. Demonstrates and applies basic knowledge about HBOT chamber operations to ensure efficacy of treatment and safety of the patient and those in the immediate area. 2. Demonstrates knowledge of the basic physiological principles/concepts of the HBO treatment and incorporates them into the treatment process. 3. Operates controls to pressurize and depressurize the chamber according to prescribed policies for clinical indications and physician orders. 4. Maintains accurate logs and records of the time, pressures, depths, etc. of chamber operations according to established protocols. 5. Performs maintenance of the hyperbaric chamber(s) and related support systems under the guidance of the HBOT Program Director/Safety Coordinator. 6. Cleans chamber following each treatment, according to recommendation of manufacturer, and in accordance with established hospital infection prevention policies. 7. Maintains emergency equipment and supplies located in the HBO area. Documents equipment checks as required by established policies. QUALITY MANAGEMENT/PERFORMANCE IMPROVEMENT: 1. Actively participates in the unit-based quality improvement program by ensuring the accuracy and completeness of information tracked through the quality management processes. 2. Supports the designated HBOT safety director, participating in safety training and emergency drills, keeping current documentation of all training activities, equipment maintenance, and infection control. GENERAL WORK PRACTICE/COMMUNICATIONS: 1. Demonstrates awareness of his/her responsibility to facilitate patient care through accurate, clear, concise and timely written and oral communications and to function as in integral part of the wound care team. 2. Documents accurately, legibly and promptly all patient related information. 3. Reports any untoward events or unusual observations related to patient care to the supervising hyperbaric physician to facilitate review and adjustment of the plan of care as needed.

Facilitates discussion between the patient and the physician, and responds accurately and in a timely manner to patient questions with direct information and/or referral to the appropriate source if additional information is required. 5. Assumes an active role in the department by attendance at and participation in staff meetings, inservices, and other related activities. HEALTH AND SAFETY: 1. Utilizing safe procedures in infection control, exposure control, equipment, supplies, facility, emergency, safety, fire and disaster plans, adhering to guidelines or standards of practice in the community, the hospital and all current wound and/or hyperbaric center policies and procedures. 2. Demonstrating consistent use of OSHA guidelines, Standard/Universal Precautions and proper body mechanics. 4. Demonstrates safe operation of all chamber equipment and associated supplies. 5.
