LMR Description: The LMR (Longitudinal Medical Record) is a web-based ambulatory electronic medical record system used across Partners HealthCare. With the LMR, providers can maintain patient records electronically and manage workflow with ease. The structured patient data and clinical tools such as charting, prescribing, and order entry help enhance data access. As a result, providers are able to improve patient care and streamline their workflow. In addition, providers can easily share data with other clinicians across Partners to facilitate communication and ensure continuity of care. Source: www.lmr.partners.org Accessing the LMR: You can access the LMR remotely by using the link: https://lmr.partners.org. You can also access it within the MEEI Network by clicking on the LMR desktop shortcut or going through the MEEI Quick Links applications shortcuts. LMR Log On Screen: 3
First time users must Registration their account by going to the Partners Profile Registration site at https://myprofile.partners.org. Other users will simply enter their User ID and Password. LMR Schedule: When you first login to the LMR system you will always be brought to the Schedule of that day for your department for the entire practice. 4
Select Location: Click the SELECT tab at the top of your LMR screen then click on LOCATION to select or change locations and go to the schedule for that location. If you are already in the correct location, click the x box to close this screen. 5
Select a Schedule by Date and Provider: Provider: Click on the Provider box where Entire Practice displays and locate your doctor on the drop down list. Click on the doctor of choice. The schedule for that doctor for that day will now display on the screen. Select the date you wish to view by using the up/down arrows or the [1:2] calendar symbol. Click the T for Today s Date To Sort the Schedule by Provider: Click on the drop down box next to Entire Practice to choose from a list of Providers within that practice. Schedule by Date: If you want to see the schedule for another day you can use the up/down arrows next to the date and click Go or click on the [1:2] calendar icon to the right of the date and that will allow you a quick way to choose a month and day in the past or future. If you want to get back to today s schedule, just click on the T to the left of the date. 6
Selecting a Patient: You can access the patient chart from the schedule by clicking on the patient name in blue. To look up a patient chart for someone who is not on the schedule or who is shown on the schedule in black (no MRN given), use Patient Lookup. Patient Lookup: To search for a Patient within the LMR, hover over select then click on Patient 7
Enter the patient s MRN, SSN or PATIENT NAME, (Last name, First name). MRN is the quickest and most direct way to find a patient as all MRNs are unique to each patient, then click the SEARCH button. Click on the underlined patient name to select the patient s Electronic Medical Record (EMR) If you need to view the patient Demographics (i.e.: Insurance etc.) click on the details button on the far right of the screen when the patient name pulls up (If you do not need to do this in your roll, skip this step. Patient Chart Summary: Once you select a patient, you are brought to the Patient Chart/Summary Page. Here you will find pertinent patient information such as medications, health problems, procedures/surgeries, allergies and family/social history. Also on the summary page you will find all notes that have been entered on this patient. Notes include: letters, consults, exam notes, referral letters, pre-op notes and scanned documents. This is also where you will find any dictations uploaded from outside services for this patient. 8
Working with Notes: To View a Note To sort through the list of visit notes for the patient please click on Notes. 9
To sort, click on the box next to Auto Filter Click the drop down arrow under Provider. Uncheck the All Box, then select the provider you want to view notes for. Sort by Provider by clicking the drop down, unselecting all and selecting the provider(s) that you wish to view notes from: 10
This will drop the list of records down to just the provider(s) you selected: Viewing Results (Big Board): To view test results, hover over Results then click on Big Board: The BIG BOARD allows you to see any tests done on this patient and how recent the test were performed (i.e. 4hrs, 1 day, 2 weeks, 1 month, 4 years). 11
To view a result, click on the time frame when the test was performed. Getting back to Schedule from Patients Chart: Clicking on the home tab will return you back to the main schedule page. 12
