MISSION We help adults with mental illness live their own best lives PURPOSE JAMHI s purpose is defined by its mission statement. JAMHI is the state-designated Community Center for Juneau. Grant funding from the State of Alaska, Division of supports: Psychiatric Emergency Services/Crisis Intervention- all ages Services to Severely Mentally Ill Adults; Outpatient assessment, treatment and referral; case management and rehabilitation services. Designated Evaluation and Treatment at Bartlett Regional Hospital General mental health services and a Drop-In Center are funded through a grant from the City/Borough of Juneau. JAMHI provides a continuum of housing ranging from Community Living/Group Homes to Supported Living and Independent Housing. GOALS JAMHI has long supported the following client and family oriented goals: Promote a welcoming, empathic and integrated approach to the treatment of individuals with mental illness, co-occurring disorders, traumatic brain injuries dementia and other cognitive impairments, and fetal alcohol spectrum disorder; Provide emergency mental health evaluation for youth and adults as provided by statute; Promote the well-being and recovery from mental illness and co-occurring disorders; Support stable and safe living in the community for Severely Mentally Ill and Severely Emotionally Disturbed adults served by JAMHI; Encourage the inclusion of family members with decision-making in the care and coordination of the treatment of their significant others and/or family members; Demonstrate respect for the rights and dignity of all individuals; Incorporate the culture and value system of the individual; and, Focus services on individual choice, satisfaction, safety and positive outcomes. 2012 JAMHI Annual Report 3

BOARD OF DIRECTORS JAMHI is governed by a nine-member board of directors, who promote significant representation of the views of the community, including persons with disabilities and their family members. Board Directors serve three year staggered terms. Board Directors may succeed themselves for additional three year terms. Three members are elected each year. STAYING THE COURSE JAMHI S overarching goal for 2012 and beyond, was to build on our strengths and maximize opportunities for excellence and growth, using our organizational mission as the guiding compass. Last year s success with our CARF accreditation helped provide a framework upon which to implement our plan. Two priorities stood out from others in 2012: improving access to services and strengthening the role of innovation and technology in providing services while maintaining financial stability. JAMHI continues outreach and partnership with multiple community organizations and coalitions. In 2012, the following access and service enhancements were initiated and came to fruition (in addition to those services outlined in following sections). Same Day Access: Reducing or eliminating barriers to service for same day/next day walk-in access for clinical assessments; no appointments needed. This was accomplished through: Centralized Scheduling: Gaining efficiency through centralized scheduling of appointments No Show Management: Including development of Treatment Readiness Group with Evidence-Based Motivational Interviewing protocol and increased appointment reminder calls to keep staff schedules available for service delivery Collaborative Documentation: Mutual (client and clinician) recording of treatment plan development and measurement of progress toward planned wellness goals Streamlining of intake procedures JAMHI was fortunate to experience progress and success with our identified goals in 2012. We believe this is due to the outstanding commitment and hard work of our staff. In this constantly changing healthcare and fiscal environment, adaptability is necessary for survival and growth, and at JAMHI, we adapt and thrive. The Board of Director s support of the mission and our staff underlies JAMHI s ability to achieve our goals and provide more and better services to the people we serve. LOOKING AHEAD TO 2013 In 2013, JAMHI plans to implement an Electronic Health Care System, Carelogic, that will provide state of the art client record storage. Additionally, JAMHI plans to initiate Telebehavioral Health Services focused on serving Alaskan Veterans and other rural residents living in our catchment area. JAMHI will be engaged in preparing for re-accreditation in 2014. Additionally, JAMHI is assessing properties owned and operated to ensure cost of services provided does not exceed our capacity for repair and management. JAMHI Senior Leadership continues to make every effort to stay tuned to local, state and federal changes and trends regarding behavioral health services, especially regarding funding and treatment. 2012 JAMHI Annual Report 6

SERVICES Outpatient Clinic & Administrative Offices The JAMHI Outpatient Clinic is located at 3406A Glacier Highway. It is a 2-story building with clinical, psychiatric, and nursing offices located on the first floor and administrative offices on the second floor. It is situated at Salmon Creek, adjacent to Summit K-2, the 8-Plex Supported Living buildings. The clinic shares the same driveway with those buildings and a Community Living group home, Salmon Creek Housing, aka The Lodge. JAMHI provides Outpatient Mental Health services to over 400 adult members of the community. The Outpatient Clinic is the primary location where outpatient mental health assessments, individual and group counseling, psychiatric and clinic nursing services are provided. Medical and Nursing Psychiatric care is provided at the JAMHI Clinic and at offsite JAMHI Group Home locations through a contract with Bartlett Regional Hospital. It includes psychiatric assessment, medication management, and case consultation. Psychiatrists are available 24/7 on an on-call basis to the nurse and clinicians for support consultation. A Registered Nurse (RN) provides nursing services to assess clients psychiatric and side effect symptoms and over-sees medications provided to those residing in JAMHI residential programs. The nurse provides education and support to persons receiving medications and assists those utilizing the Patient Assistance Medication Service. 2012 JAMHI Annual Report 9

SERVICES Emergency Mental Health JAMHI provides 24/7 on-call response with immediate screening and crisis intervention for mental health emergencies. These services are short-term, intensive psychiatric services provided to individuals during an acute mental health crisis episode. They include clinical assessment to determine the need for psychiatric hospitalization and coordination of services for support and referral. JAMHI staff works very closely with Bartlett Regional Hospital (BRH) Emergency Department and Mental Health Unit staff to facilitate the most appropriate level of care, which may include emergency and involuntary commitments, for persons presenting a mental health emergency. The BRH Mental Health Unit is a state Designated Evaluation and Treatment facility. In CY12, JAMHI staff responded to 402 Emergency Services calls and completed 522 Emergency Services assessments, for a total of 924 contacts for the year 2012 JAMHI Annual Report 10

SERVICES Case Management and Support A team of case managers and clinicians provides community support for severely mentally ill adults through outreach services. Case management also provides training in life skills that promote independent living. Coordination and integration of services throughout the community and among other agencies is a key element of service. JAMHI provides support to family members as well as clients. In accordance with client choice, JAMHI includes family members in the recovery, health and support of persons served. At the end of 2012, JAMHI was serving 105 seriously mentally ill adults, of which 72 lived the community and 32 were living at JAMHI residential facilities. JAMHI s Case Management Supervisor played a key role in the Coordinated Resource Project (AKA Juneau Mental Health Court). JAMHI served all 10 individuals who participated in the court project. Drop-In Center The Drop-In Center, funded through a grant from the City & Borough of Juneau, is located at The Lodge and is open from Noon to 1:30 pm, five days a week. The Center provides a daily lunch to participants. Interpersonal communication skills, socialization, and good nutrition are encouraged and supported. Participants have access to recreational and exercise equipment, computer, and art supplies. The Drop-In Center is located on the bus line for easy access from downtown Juneau, Douglas, and the Mendenhall Valley. 2012 JAMHI Annual Report 11

RESIDENTIAL SERVICES Group Homes Salmon Creek Housing - The Lodge The Lodge is located at 3406 Glacier Highway, Building D. It is a two-story group home constructed in 2005. The Lodge is fully ADA accessible, and is home to six persons experiencing severe and chronic mental illness, some with co-occurring alcoholism and/or other substance we disorders, FASD, TBI, criteria and/or Alzheimer s Disease or Age-Related Dementia. The program at The Lodge is identical to that of Salmonberry House, with the same admission criteria. Timberline The top floor of the 1962 Lemon Creek Road property is occupied by Timberline Group Home. Timberline is a separate 3-bed, long-term group home for persons experiencing severe and chronic mental illness, persons with co-occurring alcoholism or other substance use disorders, FASD, and/or TBI, but who do not suffer from age-related dementia or related disorders. This setting offers residents the opportunity to expand their independent living skills and work toward living in a less restrictive environment. A fully equipped pottery art studio is on the same floor and is available for use by all JAMHI residents, with supervision. Timberline is staffed 24/7 for clients whose symptoms would make it otherwise difficult for them to maintain themselves safely and appropriately in the local community. Rehabilitation services are provided daily. Unlike Salmonberry House, Timberline has no ADA accessible units but is a home-like setting with a living room, kitchen, dining room and 4 bedrooms. Residents have access to an open, landscaped courtyard and gazebo. Salmonberry Salmonberry is a 5-bed group home facility on the ground floor of 1962 Lemon Creek Road. It serves persons with severe and chronic mental illness, some of whom were previously permanent residents of Alaska Psychiatric Institute. Salmonberry residents require 24/7, sight and sound monitoring by staff. Residents generally have co-occurring substance use disorders, FASD, TBI, and/or ADRD. The 24- hour staff provides treatment services for clients whose symptoms would otherwise make it virtually impossible for them to maintain themselves safely and appropriately in the local community. Salmonberry is a home-like setting with five bedrooms, a living room, kitchen, and dining room. Residents have access to an open, landscaped courtyard and gazebo. Rehabilitation services are provided daily by trained, compassionate staff. 2012 JAMHI Annual Report 12

RESIDENTIAL SERVICES Supported Living These residences serve adults who experience serious mental illness with co-occurring alcoholism and/or other substance use disorders, Fetal Alcohol Syndrome Disorder (FASD), Traumatic Brain Injury (TBI), and Alzheimer s Disease and Related Dementia (ADRD) or other cognitive challenges. Summit K1 and Summit K2 are intended to provide a safe, healthy, and supportive environment in which residents can live and develop the skills and supports necessary to move into greater independence in the community, if possible. Clients residing in Supported Living apartments have an individual treatment plan designed to support their goals, with the objective of gaining the skills and resources needed to move into the larger community. Residents are assisted in actively working to achieve personal recovery, health, and resilience, improved functioning, and integration into the community. Summit K-1 Located at 1968 Lemon Creek Road. It is a variable-term Supported Living residence for adults experiencing severe mental illness who do not need the 24/7 sight-and-sound monitoring required by those residing at the Group Home level of care. It is the older of JAMHI s two supported living residences. Summit K-1 has 4 two-bedroom apartments, for a total of eight beds. Each apartment has its own kitchen, bathroom, living room, and dining room. Residents have access to an open, landscaped courtyard and gazebo. Two of the beds (1 apartment) are designated as transitional housing, allowing residents to qualify for governmental housing assistance in the community. One unit is fully accessible. Summit K-2 Located at 3406 Glacier Highway in buildings B and C, Summit K-2 is a variable term Supported Living residence for adults experiencing serious mental illness who do not need 24/7 sight and sound monitoring. Summit K-2 consists of two identical 8-plex apartment buildings, constructed in 2001. Each building has eight single bedroom apartments. One of these sixteen apartments is used as a group therapy/activity room and office for staff, leaving fifteen beds for residents. Two of the apartments are designated as transitional housing. 2012 JAMHI Annual Report 13

RESIDENTIAL SERVICES Independent Living Douglas Terrace Douglas Terrace is located at 2460 Douglas Highway on Douglas Island, across Gastineau Channel from Juneau. Douglas Terrace is a 3-story, 15-unit apartment building constructed in 1997. All apartments are single bedroom and provide housing for adults experiencing severe and chronic mental illness with co-occurring alcoholism/other substance use disorders, FASD, TBI, ADRD or other cognitive impairments. Residents also houses clients who have demonstrated the ability to live independently, with or without case management services. Douglas Terrace has no on-site staff. Two of the units at Douglas Terrace are ADA accessible. Rocovich Apartments The Rocovich Apartment building is located at 413 Seventh Street in downtown Juneau. Built in 1938, the Rocovich is a four-story, eight-unit apartment building with eight single-bedroom, single-bath apartments, housing adults with severe and chronic mental illness, some with co-occurring chronic alcoholism/ other substance use disorders, FASD, TBI, and/or ADRD or other cognitive impairments. One of the apartments is designated as transitional housing. Rocovich residents enjoy downtown living with walking access to many community amenities. The Rocovich houses clients who have demonstrated the ability to live independently, with or without case management services. There is one on-site staff at the Rocovich. 2012 JAMHI Annual Report 14
