Updated 9/1/15 UCLA DEPARTMENT OF WORLD ARTS AND CULTURES/DANCE Dance Major UCLA DEPARTMENT OF ARCHITECTURE AND URBAN DESIGN Fall 2016 - Supplemental Application Worksheet Use this worksheet to help prepare your materials for electronic submission via the UCLA Arts Supplemental Application website. Access to the UCLA Arts Supplemental Application is granted on a rolling basis to all applicants who have completed the UC Undergraduate Application between November 1-30 and have selected Dance as their first choice major to UCLA. Approximately 1-5 business days after submitting your UC Undergraduate Application, you may access your department supplemental application at https://supplemental.arts.ucla.edu. To log in, you will need your UC Application ID. Supplemental materials must be uploaded through the website no later than Friday, December 11, 2015, 11:59pm PST. There will be a $55.00 non-refundable processing fee associated with this supplemental application. You will be prompted when you sign in to the online upload system, and this fee is payable by credit card or e-check. If you have received a UC fee waiver, you will not be responsible for this additional fee and will not see the payment prompt. Please turn off your email spam filter and/or include arts.ucla.edu and ucla.edu as accepted recipients. All applicants are required to meet minimum UC academic eligibility to be considered for admission. Prospective transfers, please visit http://www.admissions.ucla.edu/prospect/adm_tr/tradms.htm. Applicants must select Dance as their first choice major to be eligible for review. If you would like to change your major, you must make your request by December 7 (by 5:00pm PST). Supplemental material must be uploaded to the UCLA Arts Supplemental Application website by Friday, December 11, 2015 (11:59pm PST). This is a strict deadline, please plan ahead to avoid any unexpected technical delays at the last minute. The supplemental application process on the website must be fully completed and uploaded before your application will be reviewed. Late, incomplete, or hard-copy application material will not be considered. Make sure you review all categories before you submit your supplemental application. You may log in/log out of the system as many times as needed during the filing period, but once the application is submitted, you will not be able to make ANY corrections or additions. Unofficial Transcripts (in PDF format) If you are a freshman applicant, upload a PDF copy of your current high school transcript. If you are a transfer applicant, upload PDF copies of all colleges you have attended since high school. Do not mail your transcripts. The University of California only requests hard copy transcripts from students who have been admitted and matriculated to a campus. All unofficial transcripts MUST be generated from their respective institutions, and must include the name of the institution as well as the student s full name. Please redact/black out your Social Security number. 3. Personal Statement Respond to Prompt: After reflecting upon the department s mission statement on our website, please explain what makes you an especially well-suited candidate for this particular department. 4. Audition All applicants must submit an audition either in person or by video. LIVE AUDITION he UCLA Department of World Arts and Cultures/Dance will host their Dance auditions on Friday, January 29, 2016, in Glorya Kaufman Hall. Please plan on being on campus for the whole day, until 5pm. Please arrive by 8:00 A.M. for check-in. At the start of the day, you will participate in a group class offering a warm-up and two styles of movement. You are not expected to be experienced in these styles. The faculty will be watching to see how you take on new movement. Attire should be body conscious and in solid, neutral colors. No shoes will be required for these sessions. During a portion of the morning, the School of the Arts and Architecture admission staff will conduct a Q&A session for any parents/guardians who will be here. After the group movement sessions, the individual showings will begin at approximately 10:30 A.M. For your individual showing, you will be asked to perform a one-minute piece of your own choreography in a style most comfortable to you. We encourage the presentation of composition studies that reflect study/interest in any dance traditions. Customary attire for the dance audition is a leotard or otherwise body conscious outfit with bare feet. You also have the option to wear a costume appropriate to your choreography. Please be prepared to introduce yourself and briefly describe the name (if any) of the choreographed piece, the music credit (if any), as well as any additional information you feel might be important to the faculty committee. The faculty may also ask you questions about your work and background. If you are bringing recorded music for your audition, the department will have speakers with.mp3 ports (3.5mm headphone jack).we also have CD players; however you will need to test your CD with a staff member prior to the audition to ensure that it is compatible with our players. Please note that.mp3 players are preferred. Because of the number of applicants, there may be long periods of waiting. You may leave the building during that time. However, you should be back in enough time to be prepared for your scheduled showing. There will be a room available with provided materials about our department and faculty, as well as the UCLA campus. You may also use this room to warm up. After your individual showing, you are free to leave. Freshman applicants are notified in March of their admission status; transfer applicants receive notification in April. VIDEO AUDITION In the event you are unable to attend the audition in person, you may submit a video in lieu of a live showing. The video should be no more than four minutes long. At the beginning of the video, please introduce yourself and briefly describe what we will be seeing. You may show studio or in-performance work. Please include a one-minute piece of your own choreography in a style most comfortable to you. We encourage the presentation of composition studies that reflect study/interest in any dance traditions. PXLT, TCSS/TCS2, G2M3/4 5. Sample Research Paper A research paper you have written for a class, demonstrating your writing and researching skills. - At least three (3) external references required. Your reference list may count towards the total page count. - Place your full name and page number in the header of each page. Other Evidence of Creativity (Optional) In support of your application to the Dance major, you may also choose to include work in alternate genres and forms, such as: Short stories / Creative Writing / Poetry / Visual artwork / Photography / Music You will be able to upload a maximum of two (2) files in PDF, video, or sound format. You may submit any combination of files, ex. PDF + Video, PDF + Video/Sound, PDF + PDF, etc. You will be asked to provide a brief description of the material(s) you are submitting indicating any source(s) and/or inspiration(s) for your work. Your media file may consist of video, sound, or the two compiled together as a reel. The total length of the entire compilation should be no more than four (4) minutes. There is no formal file size limit set for the media files. However, smaller file sizes (less than 480p) may yield faster and easier upload times from most home internet connections, and will be of sufficient quality for the faculty review. 8. Letters of Recommendation (Electronic) Two (2) Letters of Recommendation are required; one of the two letters must come from an academic instructor. Letters of Recommendation are to be submitted electronically through the online supplemental application. The department does not accept hard copy letters. Recommenders must submit their electronic letters by Friday, January 8, 2016 (11:59pm PST). You will list your recommenders in the online supplemental application. A recommendation request will be sent, by email, to each of your references. The email will provide a link where recommenders may upload their letters electronically. The request can be sent at any time provided you meet department deadlines. You can change references or send a reminder through your application. Contact your references ahead of time, so that they can expect your request for recommendation. Page 5 of 10

UCLA DEPARTMENT OF WORLD ARTS AND CULTURES/DANCE DANCE AUDITION INFORMATION Dance auditions are scheduled for Friday, January 29, 2016, and will be held on the UCLA campus in Glorya Kaufman Hall. We are adjacent to the UCLA Anderson School of Management and the intramural fields. The closest entrance to Kaufman Hall is off Sunset Blvd., at Westwood Plaza, which is a signaled entrance onto campus. Shortly after you turn onto campus, you will see a driveway that leads into underground Parking Lot #4. If you look to your left as you go down the driveway, you will be looking at Kaufman Hall. All-day parking is $12 (as of August 2015 parking fees are subject to change without notice). Please leave enough travel time to accommodate for traffic, and for check-in and warm-up. We will provide water, but you will be responsible for your own food. There are a number of campus eateries nearby where you may purchase meals.
