Ongoing Achievement OAR Assessment of Practice contributes towards 50% the academic component of the program. It assesses your ability to demonstrate understanding, skills and values, commensurate with your stage in education. It also outlines the Standards/Proficiencies you are required to meet by the end of placement. You are assessed on a met/not met basis at the formative stage of your placement and a pass/fail at the summative stage. The formative assessments should provide you with a clear picture of your strengths and AREAS OF DEVELOPMENT. It may also help you identify areas of concern. Areas of concern may relate to either aspects of practice in which you are not achieving in the anticipated time frame or elements of your conduct that is not considered professional. These are related to you professional Competency APT. If concerns are identified, mechanisms will be put in place to work with you and your CLR/Mentor. All the proficiencies detailed in this document have been mapped to theoretical modules and to HCPC standards and must be passed before a student can progress to the next stage of the programme. PROGRAMME AIMS AND OBJECTIVES Diagnostic Radiography Pathway Aims and Learning Outcomes Aims The aims in the Diagnostic Radiography Pathway are: To develop safe, capable, confident, collaborative medical imaging practitioners, who put the needs of the patient at the heart of their professional practice. They must be fit for practice, purpose, award and entry onto the Health Professions Council register; To develop the necessary knowledge, personal and professional skills to enable medical imaging practitioners to work collaboratively, play an increasingly pivotal role in the assessment, examination and management of patients and adapt appropriately to changes in policy and the imaging environment; They should demonstrate a sound preparation for the development of lifelong learning and continuing professional development; Learning Outcomes Knowledge and Understanding On completion of the programme students should be able to: Demonstrate a critical understanding of the theories, models, principles and concepts underpinning medical imaging; Appraise the evidence base for professional practice in medical imaging; Understand the public policy and organisational context for health and social care practice and service delivery related to medical imaging; Explain the process and practice of medical imaging; Explain and review theories about interprofessional relations and models of collaborative practice. 15

Cognitive Skills On completion of the programme students should be able to: Critically analyse information, concepts and processes in relation to medical imaging; Make and justify sound evaluative judgements about the health and welfare needs of patients and carers; Adhere to and, where appropriate, challenge the boundaries, constraints and obligations inherent in professional practice; Analyse critically, reflect upon and evaluate research findings utilising these, where appropriate, to underpin an evidence based approach to practice; Employ skills of critical self-reflection. Subject Specific Skills On completion of the programme students should be able to: Integrate the necessary professional knowledge, values and skills required to undertake a role in medical imaging; Practise safely, competently and confidently in medical imaging, within the boundaries of legal, ethical and professional frameworks, meeting the requirements of the subject benchmarks for the radiography curriculum 1,2 and be eligible to apply for entry to the professional register; Critically appraise the needs of patients and carers, using sound professional reasoning to initiate appropriate interventions in a range of situations across the scope of professional practice; Communicate and work collaboratively with patients, carers and fellow professionals in an appropriate and context specific way; Exhibit the appropriate professional values, attitudes and behaviours expected of an integrated member of the interprofessional health care team; Demonstrate a commitment to lifelong learning and effectively manage their continuing professional development 16

Graduate Skills The following six key skills are embedded progressively within the modules and evidenced in module assessments: 1. Communication; 2. Use of information technology (it); 3. Application of number; 4. Working with others; 5. Problem solving; 6. Improving own learning and performance. Year 1 is a steep learning Curve. All aspects of Radiography are new and to be explored. By the end of the first year, students should be able to: Work in a safe manner Be able to communicate with patients and staff. Carry out a range of basic examinations under supervision Be familiar with the local rules of their hospital. Have a clear understanding of human anatomy and the skeleton. By the end of the second year, students should be able to: Carry out all routine plain film imaging procedures under direct supervision; Carry out a range of mobile radiographic examinations under direct supervision; Assist with fluoroscopic examinations; Begin the development of initial image interpretation skills associated with the commonly seen pathology and trauma; Demonstrate appropriate communication skills and awareness the needs of service users and carers Demonstrate appropriate graduate skills On completion of the programme, students should be able to: Convey ideas and information appropriately by adapting written and verbal communication formats to meet desired outcomes; Select and use appropriate it skills to enhance the achievement of desired aims and outcomes; Demonstrate sound numerical skills and understanding of statistical techniques; Contribute appropriately to the work of interprofessional teams; Utilise current research and other sources of evidence to optimise medical imaging practice; Critically analyse, reflect upon and evaluate published literature from a range of sources; Seek and use self and peer-assessment in supervision for continuing professional development; Critically reflect on own experience and practice. 17

Clinical Placement Structure Year 1 begins with a 4-week OBSERVATIONAL placement, a further two placements of 8 and 4 weeks in duration will occur during the first year. This pattern will allow the student time to reflect, consolidate and begin to apply theory to practice before building on this experience with further academic learning. Students will rotate through a minimum of two clinical sites over the duration of the course. Year 1 will be spent at a single site to allow for consolidation of theory and basic skills. 1. Total Placement requirement for Year 1 is 448 documented hours. You will work shifts between 8-8, 4 days a week as part of your allocation any changes or additional hours must be approved by your CLR. 2. Applications to modify the working week must be made prior to the start of the placement block and must be agreed in writing. This can then be added to the portfolio as evidence. 3. Students must normally sign in on a register provided by the department by 9am unless the agreed hours are different. a. It is recommended that you document morning and afternoon shifts as separate. b. Lunches and breaks are not to be included in the documented hours. 4. Min numbers must be completed in your modalities as per your attendance book. a. 20 hours per week is the Minimum requirement of each student. Due to rotas this may not be possible at all sites as a full allocation. b. If you have not been allocated a placement please speak to your CLR/Link or Placement Lead to organise a shift to allow the Worksheet to be complete 5. No more than 40 working hours are to be completed in any 7-day period without prior approval from your Link Tutor or Placement Lead. 6. All hours must be documented in your Attendance Book and signed. 7. You will be required to fit in with the shift pattern of the department or modality in which your placement is allocated. 8. You will be required to provide evidence of WARD placement hours undertaken during the duration of year 1. a. On completion of your session you must fill in the form supplied in the OAR. Hours are recorded within your attendance book. b. This consists of 28 hours or 3 full shifts whichever is the longest. 9. All hours are worked/taken at the discretion of the dept. Uniform policy must be strictly observed. You are training for a profession where you have responsibility for the well-being of sick and vulnerable patients. Therefore you must be competent to practice Students who do not complete the required clinical hours will not be able to qualify. If necessary, you will be required to makeup and resit periods of placement during holiday time or the Examination Board may defer your qualification of any student, until they have acquired the experience designated within the course documentation. 18

Lieu Time 1. Accrued time may only be taken during a placement block if agreed with the CLR of the placement, in advance. 2. It is not appropriate to ring up in the morning stating you are taking a day off. This will be noted as unauthorised leave, which will be worked back. 3. Time owing is not transferable to another academic year. 4. Written agreement for more than a 2 day period must be obtained. You are training for a profession where you have responsibility for the well-being of sick and vulnerable patients. Therefore, you must be competent to practice Students who do not complete the required clinical hours will not be able to qualify. If necessary, you will be required to makeup and resit periods of placement during holiday time or the Examination Board may defer your qualification of any student until such time as they have acquired the experience designated within the course documentation. All hours stated are Minimum hours required to complete the course, not an ultimate cut off point. ASSESSMENT OF PRACTICE TOOL (APT) Evidence to demonstrate competence in practice This section of the Portfolio will provide evidence of practice experience and practice competence gained within the second year of your programme of study at Canterbury Christ Church University. Successful completion is essential for the award of the degree. The section contains material for each clinical placement. Guide to the Assessment Process Assessment for a placement should be completed by the end of the placement but it is recognised that due to variation in a student experience this may not always be possible and the process allows for incomplete activities to be carried forward to the next placement. Thus assessments for the end of placements can described as Formative or Practice, meaning it may be passed or failed, it informs your progress and guides you in evaluating your needs and planning for your next placement after which the assessment is Summative or Assessed, it must be passed. A lot of material is duplicated, this is done deliberately to ensure that development of your skills and knowledge is proceeding in a timely fashion and that you can demonstrate this not once but twice. This is particularly the case for the assessments of clinical competence which are undertaken twice, with a recommended minimum of 5 Days between assessments. Professional Competency Tool This is not to be completed for your first OBSERVATIONAL placement. Be aware of the requirements and ensure your working practices meet expectations. This sheet needs to be completed at the end of every placement block. (apart from placement 1, year 1) Once you have completed your mentor review session your mentor will then meet with your CLR and together will sign this competency sheet. 1. Your Mentor and Clinical Liaison Radiographer will meet and sign during the final week of Placement. 2. If a competency is deemed NOT MET, then this is considered a failed placement. A competency can be passed With Conditions if minor misdemeanours are identified, allowing the competency to be passed overall. 3. If an overall Fail is recorded, students will be offered a tripartite meeting with the Link Tutor and Placement Lead to discuss a Learning Action Plan. a. Support will be put in place for subsequent placements. b. Identify areas of weakness to be improved. c. A recommendation to the Board of examiners with regard to the lenght of the resit period will be decided upon. d. Student Fitness to Practice Policy will be followed if deemed appropriate. e. This if appropriate, it will be instigated immediately and escalated to Head of Dept 4. Failed Placement will be carried to end of year and will be recorded and sent to end of year Board of Examiners. 20

5. Resit opportunities for a failed placement will be given at the failed placement site. Two consecutive failed placements in a year will be automatically referred to Fitness to Practice Procedures. The Assessment of Practice Tool (APT) Assessments are designed to measure knowledge and understanding, cognitive skills, professional skills, graduate skills and health informatics. Each module reflects these learning outcomes, when appropriate, and utilises pertinent assessment methods. The modules are assessed by a variety of means and will require the student to apply theory to practice experience. The modules are also assessed by means of the APT. The summative assessments emphasise the clinical skills and competence based on essential knowledge, understanding, behaviours and attitudes. The Assessment of Practice Tool (APT) is a document intended to demonstrate that the students clinical learning meets the minimum requirements set down by various interested parties, in particular the Health Professions Council Standards of Proficiency (2008), The Society and College of Radiographers Statements for Professional Conduct (2002) and Curriculum Framework for Radiography (2003), the National Occupational Standards for Clinical Imaging(2003) and from the Quality Assurance Agency for Higher Education(QAA),Subject Benchmarks for Radiography and the University Graduate Skills framework Items within it should be completed by any qualified Radiographer, Medical Practitioner or Nurse as and when the student demonstrates the appropriate skills, knowledge or behaviours. The final certification of this document rests with the student s Mentor, or in their absence, with the Link Lecturer. In order to ensure Fitness to Practice the degree cannot be awarded unless the student completes successfully the clinical competencies outlined in the assessment schedule within the Assessment of Practice Tool (APT). Health Professions Council Standards of Proficiency (2013). Clinical Competence Checklists As part of completion of the APT the student must demonstrate competence in a number of radiographic examinations throughout their education and training. These have been selected to represent the range of skills, knowledge and behaviours that a newly qualified Radiographer must have. Certain examinations are specified for different parts of the three-year course. By the end of year 1 a student has already demonstrated competence in six common procedures, by the end of year 2 a student must demonstrate competence in a further seven common procedures on two separate occasions. These are: Hand or Wrist, Elbow, Foot, Ankle, Knee, Chest, 21

(Formative assessments MUST be completed before any summative assessment is considered) Clinical Competence check sheets are provided for the use of Radiographers to observe, assess and certify the ability of students. The supervisor will assess competence using a yes/no, tick box. There will be no assessment of film quality nor will there be a discussion related to the examination. The assessment is deemed complete and passed if there are less than five points assessed as no (except those that are technical fails). Resubmission is permitted for incomplete formative assessments on as many occasions as necessary to achieve completion provided that the supervising radiographer is of the opinion that satisfactory progress is being made. Students will be required to provide evidence of reflection on each incomplete clinical assessment prior to resubmiss Safeguards for the patients It is obligatory that the patient's permission is obtained prior to the assessment performance and that the patient can withdraw that consent at any time. The assessment must be terminated immediately, if, in the supervisor's judgement, the patient is seen to be at risk during the procedure. Audit of Practical Experience and Record of examinations Aims and objectives The purpose of this is to: a) Monitor the performance of students; b) Assess specific aspects of the radiographic process; c) Ensure consistency in the quality of examinations performed by students and provide objective feedback to the student. Students are required to complete a record of examinations that they perform during years 1, 2 and 3. Examinations have been grouped and minimum target levels set for each group. Students must indicate whether the examination was aided/unaided, although some examinations/procedures, e.g. interventional, will always be aided. A minimum of 240 examinations must be completed, of which at least 170 must be unaided, with information recorded in the format below. 22

Minimum targets towards completion are as follows: 70 by the end of Year 1 170 by the end of Year 2; 240 by the end of Year 3. Year 1 Audit Year 1 There will be one combined audit discussion in total 40 minutes taken from the following areas: Upper Limb Lower Limb The format of the discussion is free and guided by the particular cases chosen by the university tutor but specific topic areas must be covered and signed by the tutor if deemed of a satisfactory standard and complete. The forms for audit discussion are included in the relevant placement sections. Topic areas not completed to a satisfactory standard on the initial occasion will be subject to a further discussion to be completed by a given deadline. It is vital that the discussion sticks closely to the parameters in the discussion sheet and that all areas are covered and judged Some areas such as image recording system used and contrast, density and sharpness will probably be judged the same on both sheets but the anatomy and pathology sections be judged differently in the different anatomical regions. Remembering that this is an end of second year examination, the judgement is, has the student satisfactorily answered your questions and demonstrated theoretical and practical knowledge about the case, Yes or No. The use of n/a can only be valid in the pathology section if none of the presented cases contains pathology or normal variants. In year 2 the expectation is that abnormal appearances will be simply identified, classification of pathology and differential diagnosis is not required. 24

Remember that this audit is a part of the examination system and that the professional integrity of the examiner in a viva situation is to be relied on. If the student fails a resit opportunity will be offered. This resit will have a second examiner present, this Moderator will not be involved in the audit but is there as an observer for quality monitoring purposes. This is an official examination and failure to pass will lead to withdrawal from the course subject to Exam Board recommendations after the resit option has been fulfilled. 25

Prior to Clinical Placement 1 in University block 1 The student is prepared for placement within the University; this involves discussion of the requirements of clinical placement education and explanation of Ongoing Achievement Record (OAR). Drawing upon the notes made in the Clinical Induction Day and the Introduction to the APT afternoon the student will complete their learning contract for placement 1 of the first year. A bank of mentors will be available at each of the clinical sites and the student will be allocated a mentor in week one of their placement at the clinical site by the Clinical Liaison Radiographer (CLR.) Professional Competency Guidelines 1. This sheet needs to be completed at the end of every placement block apart from Placement 1. 2. Your Mentor and Clinical Liaison Radiographer will meet and your CLR will sign during the final week of Placement. 3. If a competency is deemed NOT MET, then this is considered a failed placement.
