MISSOURI HOUSING TRUST FUND (MHTF) APPLICATION GUIDANCE INTRODUCTION Purpose: The Missouri Housing Development Commission (MHDC) hereby notifies interested organizations of the availability of funds to provide housing assistance to very low-income Missourians. The funds will be allocated in accordance with the Allocation Plan (MHTF-110). The funds are as indicated below: $4,000,000 from the Missouri Housing Trust Fund (estimated) Deadline: Applications for funding will be accepted by MHDC until 5:00 p.m. CDT on January 12, 2018. All applications received after the deadline will be rejected and returned to the sender. Decisions regarding funding of proposals are scheduled to be made by the Commission early 2018. Requirements: Applicants must be a nonprofit or for-profit corporation or partnership entity formed pursuant to applicable Missouri law, must be an entity in good standing with the state of Missouri and provide housing or housing services. Missouri Housing Trust Fund monies will not be awarded to individuals. All intended recipients assisted by the Missouri Housing Trust Fund must have incomes at or below 50 percent of the area median income for the geographic area adjusted for family size, and 50 percent of the recipients must have incomes at or below 25 percent of the area median income for the geographic area adjusted for family size. All proposals must be submitted on the current year s application form that is posted on the MHDC website (http://www.mhdc.com/housing_trust_fund/index.htm) and in compliance with the Application Guidance (MHTF-115). SECTION I Applicant Requirements Overview of Funds The Missouri Housing Development Commission (MHDC) is charged with the responsibility of administering the Missouri Housing Trust Fund. The state Legislature created the Missouri Housing Trust Fund in 1994, pursuant to RSMo 59.319. Monies in the fund shall be used solely for the purposes established by RSMo 215.034-215.039. Missouri Housing Trust Fund monies shall be used to provide housing and housing services to individuals and families with incomes at or below 50 percent of the area median income. Fiftypercent of the funds must be used for individuals and families at or below 25 percent of the area median income. Establishment of Funds The Missouri Housing Trust Fund is supported by a recording fee of $3 for each real estate document filed in the state of Missouri. Allocation of Funds MHDC has identified the following grant types to meet the needs of Missourians: 1. Housing Assistance: available to organizations that provide assistance to literally homeless individuals and families in permanent housing 2. Emergency Assistance: available for organizations that provide assistance to individuals and families at immediate risk of becoming homeless 3. Operating Funds: available for organizations that provide housing or housing services for the purpose of paying salaries and benefits necessary for operation of the organization 4. Home Repair or Modifications: available to organizations that provide housing services for the payment of certain repairs or modifications of homeowner-occupied single-family homes 5. Construction / Rehabilitation: available for organizations that provide housing for the purpose of payment of costs of new construction or modification or rehabilitation of existing facilities It is the purpose of the Missouri Housing Trust Fund to serve the greatest housing and housing service needs in the state with attention given to the lowest-income residents in the areas where those needs exist. These established grant types allow the Missouri Housing Trust Fund s limited resources to be dedicated to services that best serve the population in greatest need. Administration MHTF Housing Assistance, Home Repair and Emergency Assistance grantees are eligible, but not required, to spend up to ten percent of each grant request for administrative expenses associated with administering the grant(s). Cap on Funds Requested Applicants may not request funds in excess of $150,000 per grant type with a maximum of $300,000 per entity, per grant cycle. 3 Effective Date: 11/6/2017

Genevieve and Washington Counties South Region: Barry, Barton, Butler, Carter, Cedar, Christian, Dade, Dallas, Dent, Douglas, Dunklin, Greene, Hickory, Howell, Jasper, Laclede, Lawrence, McDonald, Mississippi, New Madrid, Newton, Oregon, Ozark, Pemiscot, Polk, Reynolds, Ripley, Scott, Shannon, Stoddard, Stone, Taney, Texas, Vernon, Wayne, Webster and Wright Counties Distribution Percentage: 22% 15% 16% 20% 27% Homeless Management Information System The Homeless Management Information System (HMIS) is a system that collects data on services provided to households that are homeless or in danger of becoming homeless. MHTF Rental Assistance and Emergency Assistance grantees are required to enter client data into Homeless Management Information System (HMIS) for each Continuum of Care (CoC) that is served with MHTF. The HMIS provider for each service area as determined by the CoC will provide MHDC staff with confirmation that applicants are or are not HMIS users. If an applicant serves a protected population such as victims of domestic violence, the HMIS lead in their service area will provide confirmation to MHDC as to whether or not the applicant enters into a comparable database. Continuum of Care Participation MHDC encourages coordination and collaboration between the CoC and service providers. In order to determine applicants participation in its CoC, MHDC will request feedback from each applicant s designated CoC regarding the level of involvement of the applicant in CoC activities. Such activities may include, but are not limited to, attendance in regular meetings, Point-In-Time Count, Coordinated Entry, and agency representation on sub-committees. 4 Effective Date: 11/6/2017

Coordinated Entry System Participation Each Missouri CoC if federally mandated to establish and operate a Coordinated Entry (CE) System. The CE system is intended to reduce barriers and difficulties that homeless households may experience when attempting to access housing and services. An effective CE system will assess homeless individuals and families for the most appropriate housing intervention in a consistent manner, prioritize households based on vulnerability and severity of needs, align housing resources within the system, and reduce the amount of time spent by household and agencies in determining which programs they are eligible to receive assistance from. MHTF Housing Assistance and Emergency Assistance grantees are required to participate in their CoC s CE system. Those service victims of domestic violence, dating, violence, sexual assault, stalking and other life-threatening situations are encouraged to participate but may choose not to participate in the CE system of their CoC. Victims service providers are encouraged to work in collaboration with the CE system in order to create and implement policies and procedures that are client-centered. SECTION II Instructions General Completion Instructions 1. Submit one application for all grant types. The FY2018 MHTF application is designed to allow grantees to include all funding requests in one application. Applicants requesting funds for more than one grant type only need to submit one application for all MHTF funding requests. If requesting funds for multiple grant types, only one set of supplemental documents will be required. Although all requests will be submitted in one application, each grant type request is still considered separately. 2. The application should be completed in sequential order. The application has been designed with the ability to populate information based on entered detail. For example, in order to complete Excel tab 5A. Geographic Information, applicant must first complete Excel tab 4A. Proposal Information so that the grant amounts requested populate on to tab 5A. 3. Do not tamper with, modify or delete elements in the application. In order to maintain the integrity of the electronic application, many cells are locked and inaccessible by the applicant. Any attempt to access or tamper with the formulas, modify the document or delete tabs will be a cause for automatic reduction of points. 4. Applicants must respond to each question on the application and checklist unless otherwise specified below (#5). Each answer should be concise, self-supporting and not refer to any other area of the application. 5. No blue boxes in application. All questions are required to be answered in the application except for: a. Orange Program Tabs (Excel tabs 1G. 5G.) Only the program tabs for the grant types which your agency is applying for should be completed. Example: If an applicant is requesting funds for the Rental Assistance (1G.) and Operating Funds (2G.) grant types, then the Emergency Assistance (3G.), Home Repair (4G.), and Construction/Rehabilitation (5G.) program sections do not need to be completed, printed or acknowledged. 5 Effective Date: 11/6/2017

Obtaining and Navigating the Application Directions for printing, navigating and electronically copying the application: 1. Use FY2018 Missouri Housing Trust Fund Application. All applicants are required to use the 2018 MHTF Application Form provided by the Missouri Housing Development Commission (MHDC) at its web site under the heading FY2018 Items : http://mhdc.com/housing_trust_fund/index.htm. 2. Save application to computer or removable storage. To save the file on your computer, click the Application link under the heading FY 2018 Items from the above website. When the dialog box appears, you may either choose to Save As and save it on your computer or removable storage, or click Open. If you choose to open the file, you must then choose Save As from Excel s file tab to save a copy on your computer or removable storage device. Please note that the file must be saved in.xlsx format. 3. Print. To print the application, left click on the File option on the top menu in Excel with the workbook open. Then click on the Print option. When the screen shows print details, select the Entire Workbook option in the lower left portion of the screen showing print details. This will print each and every worksheet within the workbook. To print just a single worksheet within the workbook, follow the procedure described above but select Active Sheet(s). To preview the entire application before printing, right-click on one of the worksheet tabs, select Select All Sheets, select the File option on the top menu and then the Print option from the drop down menu, and you will be click on the arrows on the bottom of the screen to preview each page of the application. 4. Navigating Tabs. To navigate between the different tabs in the workbook, point the mouse cursor at the desired tab at the bottom of the screen and left click. Also, the small black arrows at bottom left will assist in moving between the tabs. Application and Supplemental Document Checklist Below are the required items to be completed/included by the applicant. The hard copy application should be submitted in a binder with labeled dividers for each section specified below. The divider labels should reflect the BOLDED, CAPITALIZED headings on tab 2A. Proof of Site Control Application Detailed Instructions The descriptions of all sections and supplemental documentation required in the application are below. Please note that everything in the checklist must be included in the application; nothing should be left out unless otherwise specified. Specific questions or topics within each section that may need more detailed explanation are listed under each section topic. Core Application: 1A. Cover Sheet No information is entered on the cover sheet. Information from the completed application is pulled into the cover sheet. Please ensure that the information displayed on the printed cover sheet accurately reflects the information in your proposal (i.e., do not print your cover sheet before the remainder of your application is final). Missing or mismatched information from the cover sheet and the rest of the application will result in a point loss on the application. 2A. Checklist The checklist must be completed with no blue boxes. Each box has a dropdown which allows applicants to select Yes, No, or N/A. If a section is not required to be completed, applicant can select N/A. For example, if applicant is not applying for Construction/Rehabilitation funds, then N/A can be selected for all boxes in the Construction/Rehabilitation Supplemental Divider Checklist. 3A. Agency Information This section collects general information about your agency. Question 1 Applicant s Legal Name: The agency s legal name should be listed and should match exactly with the name that is listed on the Secretary of State s website. 7 Effective Date: 11/6/2017

Question 3 Grant Contact: The person listed here as your grant contact should be program administrator for the MHTF grant throughout the entire funding period. This is not the grant writer, unless they are also the program administrator. When typing in the phone number (area code included), do not list any characters other than numerical digits (i.e., parentheses, hyphens, etc.). Question 5 This must be a live/original signature from Executive Director / CEO / Board President only. Signature from other persons will not be accepted, unless a Board Resolution from the agency s Executive Board giving signing authority to an individual other than the Executive Director / CEO / Board President is included. 4A. Proposal Information This section collects information about your request and general program questions. Question 1 List the beginning and ending month and date of your agency s fiscal year. Question 2 This is the amount of funding that is being requested by grant type. Please enter zero for any grant type that is not being applied for. PLEASE NOTE the following criteria for the FY2018 application cycle: 1. A single grant type funding request cannot exceed $150,000. 2. The total combined requests for an organization cannot exceed $300, 000. 3. The total funding request must not exceed 20 percent of the agency/organization budget, as calculated in the application. If funding request(s) exceed any of the above criteria, these sections will turn red indicating that changes needed to be made to the amount of funding that is being requested. Failure to adhere to the above criteria will result in significant point loss on the application(s). Question 3 Please list the projected total agency budget for the MHTF grant year (April 1, 2018 March 31, 2019). Question 4 This represents the projected percentage of persons served in these two income categories for this specific program. All MHTF funds must be used to serve persons at or below 50 percent of area median income, so the two categories added together must equal 100 percent. If funded, the organization will be held to within ten percentage points of these original projections. Questions 6 and 7 MHDC strongly encourages agencies to utilize certified minority-owned and women-owned business for goods and services. Please list the percentage of the total agency budget that will utilize certified minorityowned and women-owned businesses. If funded, the organization will be held to within ten percentage points of the original projection. 5A. Geographic Information This section collects information about the MHTF regions your organization will serve throughout the FY2018 grant year. Due to limited funding, applicants may be funded at 100, 80, 65, 50, or zero percent of each of their funding request(s). The 100, 80, 65 and 50 percent column headings are referring to these potential MHTF funding levels. For the grant types for which funds are being requested, the percentage of funds to be spent must be completed for each funding level (100, 80, 65, and 50 percent). Applicants should only complete the region(s) where funds are proposed to be spent. If funded, applicants will be held to the regions in which funds are requested and at the amounts specified. For a full list of counties in each region, please refer to the Geographic Allocation chart on page 4. Grant Type Program Information (only complete the sections for the requested grant types) 1G. Emergency Assistance is intended to assist individuals and families at immediate risk of homelessness. Eligible uses may include one-time payment of up to six months of rental and utility arrears, rental and utility assistance, hotel/motel vouchers and mortgage assistance. Question 11 Describe the actual construction project, not the program. We want to know specifically what is going to be constructed or rehabilitated. Example New Construction: Constructing six, one bedroom accessible units for disabled veterans. Site is acquired and 75 percent of funds have been secured, and MHTF funds will be used for windows, flooring, plumbing and finishing. Construction has not yet began but is tentatively scheduled to begin in summer 2018. Example Rehabilitation: This is a rehabilitation of a facility housing youth aging out of foster care. MHTF funds would be used for Phase II of the project and would rehabilitate eight of twelve, two-bedroom units. Work would include: upgrading windows, flooring, kitchen, bathroom and paint. Phase I funding has been secured for rehabilitation of four units; rehabilitation of those units will begin in spring of 2018. Phase II rehabilitation will begin in summer of 2018 and is expected to be completed in fall of 2018. Question 12 Describe the timeline of the project including the current status of the project. Please include timeline details for the portion of the construction or rehabilitation project that MHTF funds are being requested for. It is MHDC s intention that MHTF funds for Construction/Rehabilitation are used within the 2018 grant year, therefore this section assists in evaluating whether the applicant s timeline is consistent with the MHTF funding period. Questions 20-22 If applicant will be doing the work internally, applicant must provide applicant information in the Contractor and Architect information sections. Supplemental Application Sections Supplements included as tabs in application: 1S. Sources and Uses Applicant will complete this program budget showing the MHTF request for funds and the total program budget at each of the following potential funding levels: 100 percent, 80 percent, 65 percent, and 50 percent. All sources of funding that are used in the program where MHTF would be going should be listed in the Sources sections at the top. The general uses of each funding source should be listed underneath Revenue Uses. All MHTF grant type funding requests should be listed on the same form and then reduced by 80, 65, and 50 percent for each of those Sources and Uses. Any uses of MHTF should be eligible uses of the grant. If applicant is unsure what eligible uses of the MHTF grant are, please refer to the grant type tabs of the application. 2S. Agency Budget Applicant to complete their projected agency budget for FY2018 MHTF grant year (April 1, 2018-March 31, 2019) 3S. Board of Directors/Decision Making Body The applicant must submit a complete list of the organization s current board or governing members. The list must be of the executive board or decision-making body, not an advisory board. 12 Effective Date: 11/6/2017

Supplements external from application, to be provided by applicant: 1. Letters of Support Applicant must provide two letters of support from elected officials and/or collaborative agencies. 2. Program Guidelines Applicant must include program guidelines that detail how client eligibility is evaluated, the criteria to obtain assistance and remain in program, termination and grievance procedures. Program guidelines should be included for all program(s) where MHTF funds will be used. Applicant should not include internal policies and procedures. Inclusion of internal agency policies and procedures in lieu of program guidelines will result in a loss of points. 3. Certificate of Good Standing Applicant must include a Certificate of Good Standing that is current within 60 days of the application due date. If applicant does not have a Certificate of Good Standing that is current within 60 days, applicant may include most recent Certificate AND a print out from the Secretary of State website that states that organization is in good standing with a date within 60 days of application due date. 4. Audit/Financials Applicant must include the organizations most recently completed and board approved audit including the independent auditor s report. If the most recent fiscal year audit is in progress and not complete yet, applicant should submit last completed, board approved audit in addition to unaudited financial statements (i.e., balance sheet, income statement, and statement of cash flows). The unaudited financial statements should cover the gap in time from when the last audit ended until 30 days before the application deadline. If the most recently completed and board approved audit resulted in audit findings, please provide a letter detailing how the finding(s) were or will be addressed. Supplements for Construction/Rehabilitation applicants only: 5. Work Write-Up / Architectural Drawings (Rehabilitation and New Construction only) Applicant must include a Work Write-Up or Architectural Drawings for the project. 6. Construction Cost Breakdown (Rehabilitation and New Construction only) Applicant must submit a construction cost breakdown, specifically detailing the costs of the project and preferably the MHTF portion of the project. 7. Financial Commitments (Rehabilitation and New Construction) Applicant should submit letters from any sources of funding that have been committed or have expressed intention to commit funds. The financial commitment letters must list the amount of funding committed, time period, and eligible uses of the funds and should be on official letterhead. All financial commitment letters must be dated within 90 days of application deadline. 8. Time Frame / Construction Schedule (Rehabilitation and New Construction) Applicant must submit a Time Frame or Construction Schedule. applicable. The MHTF portion of the project should be included in this Time Frame or Construction Schedule. 9. Photographs (Rehabilitation [existing buildings]; New Construction [site]) Applicant must submit photographs of the existing building and areas intended to be renovated if it is a rehabilitation project OR photographs of the site if it is a new construction project. 10. Site Plan (Rehabilitation only) Applicants for rehabilitation projects must submit a site plan for the completed rehabilitation. 11. Proof of Site Control (Rehabilitation and New Construction) All Construction applicants must show proof of site control such as a vesting deed or a purchase agreement. SECTION III Evaluation of Applications Overview MHDC wants to ensure that the limited pool of funding received is used in the most effective way possible to help organizations provide the necessary services to low income individuals and families in the state of Missouri who are literally homeless or at risk of becoming homeless. In order to do this, funds awarded will be based on the allocation plan, types and amounts of funding requests, and points awarded within various criteria. Individual scores by program may be assessed for completeness of the grant application, past performance and composition of proposal. Applicants requesting funds for multiple grant types may receive funds for one, but not all grant types. If an application is received after the deadline of January 5, 2018 at 5:00 p.m. the application will be rejected and will not be scored, no exceptions. Preparation of an application does not guarantee a grantee will receive funds. Grants will be awarded competitively to those with the highest rankings as determined in an objective scoring process using the Allocation Plan (Form: MHTF-110). The hard copy and electronic copy of the application must match exactly. Failure to submit fully complete applications as designated below will result in a point reduction when the application is evaluated. MHTF applicants should not contact commissioners or MHDC staff members during the seven days leading up to MHDC s scheduled commission meeting in regards to MHTF funding decisions. Award Notification Notifications of approval or denial of funding will be sent via email to each agency s executive director and grant contact in early 2018. Availability of Grant Funds If approved, grant funds will be available April 1, 2018, however, prior to receiving funds, the grant agreement and all required additional paperwork must be completed by the agency and returned to MHDC by the deadline.
