Health Leads envisins a healthcare system that addresses all patients' basic resurce needs as a standard part f quality care. Health Leads missin is t catalyze this healthcare system by cnnecting patients with the basic resurces they need t be healthy, and in ding s, build leaders with the cnvictin and ability t champin quality care fr all patients. Abut Health Leads New England New England is the ldest and largest f Health Leads fur regins. It has grwn frm a card table in the waiting rm at Bstn City Hspital staffed by 10 students, t a rbust peratin that last year alne mbilized mre than 250 cllege students t serve ver 4,000 families in Bstn and Prvidence, Rhde Island. The Health Leads Mdel The Health Leads Desk is a resurce and advcacy prgram that helps families access cmmunity resurces critical t their health, safety, and ecnmic survival. Operating in clinics and hspitals, the Health Leads desks use a patient s visit t the dctr as an pprtunity t address the nn-medical issues that have an impact n health. At the desk, Health Leads Advcates talk with individuals and families abut their resurce needs, and cnnect them with apprpriate services related t fd, husing, health insurance, childcare, adult educatin, jb training, and ther services. III. Advcate Jb Descriptin Psitin Overview The Advcate prvides high quality service t her/his clients that results in their access t the basic resurces needed t be healthy. The Advcate participates in a team f 8-10, managed by a student Team Crdinatr (TC). The Advcate reprts t the site s Prgram Manager, a paid Health Leads staff member. The Advcate brings cmmitment, creativity, tenacity, and humility t wrk with families, and, thrugh this wrk, strengthens her/his cnvictin and ability t champin quality care fr all patients. Psitin Availability 8 new Advcates are needed fr the desk serving the Pediatric Clinic at Bstn Medical Center (BMC) in Bstn s Rxbury neighbrhd. 4 new Advcates are needed fr the desk serving the Obstetrics Clinic at BMC. 8 new Advcates are needed fr the desk serving the Pediatric Clinic at The Dimck Center, a cmmunity health center in Bstn s Rxbury neighbrhd. Cmmitments Fr Advcates at the desk in the BMC Pediatric Clinic and BMC-Obstetrics Clinic: Able and willing t cmmit a minimum f 6 hurs f service a week, including ne 2.5-hur shift at the desk and at least 1.5 hurs a week f case fllw-up n campus. This is a prcess mandated by the hspitals and clinics in which we wrk. T wrk at BMC-Pediatrics r BMC-Obstetrics, yu must cmplete these tasks: Meet TB testing requirements. Submit certain immunizatin recrds. Submit dcumentatin f a recent flu sht. Cmplete ther n-barding steps which invlve at least tw visits t BMC. We will prvide yu with guidance in cmpleting these tasks. If infrmatin is missing, please bring the dcument back t the clinic that administered the test.
