How to complete this application form All students need to complete sections 1 to 6, these ask about you and your course. You ll need to have your bank or building society details to hand. Sections 7 and 8 will ask about your household financial details. Depending on your situation, either you (and your partner, if you have one) or your parent(s) will need to complete just one of these sections. We ll tell you which sections you need to complete as you go through the form. Follow the instructions. If you don t complete a section you ve been asked to, this could delay your grant payments. If you re a third party completing this form on the student s behalf you need to also complete section 11. Use the notes iwhere you see this you should check the notes for specific information to help you complete a question or section. e Where you see this you need to send us documents to support your application. Use the notes to find out exactly what you need to send. All documents you send must be originals, we can t accept copies. We send your originals back to you. What if I can t send the documents you need now? Return your application without them. By doing this we can make a start on your application, but we won t be able to pay you until we see the documents we ve asked for. Applications received after this date will not be accepted. The final deadline date for sending supporting documentation for academic year 2014/15 is 31 August 2015 (12 months after your course start date). Section 5 Your independence details 5.1 On the first day of your course, will you: be aged 25 or over? be responsible for a child? e have financially supported yourself for more than three years? e be living under the care of a Local Authority? e be, or have been, married or in a civil partnership e be either estranged from your parents or both your parents are deceased? e By estranged we mean that you ve had no contact with your parents for at least 12 months! Section 6 Bank/building society details This account must be in your own name and be able to accept direct credits. You need to provide your bank or building society details, or we won t be able to pay you. If these change at any time you need to tell us immediately. If there are exceptional circumstances which mean your payments would be made to someone else s account, tick here This section should be completed with the bank or building society details of someone who is authorised to hold funds on your behalf. We use this information to work out whose financial details we need to calculate your grant entitlement. Section 7 Household financial details for independent students i As you re an independent student you (and your partner, if you have one) need to complete this section. By partner we mean either your husband, wife, civil partner or co-habitant. You need to give your expected income details for academic year (AY) 2014/15. Your partner (if applicable) needs to give actual income details for tax year (TY) 2013-14. They ll need to provide details and supporting documents to prove their household income for tax year 2013-14. Where you see i there ll be additional information in the notes to help you complete this form. e Where you see your partner will need to provide supporting documents for each amount stated in this section, see the notes for more information. You (the parent(s)) need to give actual income details for tax year 2013-14. You need to provide details and supporting documents to prove your household income for tax year 2013-14. Where you see i there ll be additional information in the notes to help you complete this section. Where you see e you ll need to provide supporting documents to show your income, see the notes for more information.
