Address Verification - Graduate Modification This document outlines the modifications to the Undergraduate Address Verification Process for use by Graduate students, specifically Opus College of Business students. Assumptions: OCB will provide the list of students its departments want to go through the process. Addresses collected in this process are not considered SEVIS addresses. SEVIS addresses are recorded in the SEVIS system, and some departments use the VI and VL address types in Banner as well. Murphy Online Login Page Student enters UST ID and Pin 1

Welcome Page After reading the Welcome message, the student selects Click here to Continue to begin the address verification process. Summary Page A summary of the student s information currently in Banner is displayed. The student will be given the opportunity to review and/or update information as he/she goes through the process. If the student selects Click here to View/Update Information the student will move to the Home Primary Address Review form. 2

If the student selects This Information is Correct, Continue the student will move to the Cell Phone Question form. Note: If the student doesn t have a CM address we are not going to show the field on the page. Home Primary Address Review If the student selects The information is CORRECT, the student moves to the Billing Address Question form. If the student selects The information is NOT CORRECT, the student moves to the Update Home Primary Address form. 3

Update Home Primary Address The student updates information appropriately and selects Submit. Note: The address is run through the QAS postal verification system which verifies the address is a USPS deliverable addresses. If QAS does not recognize the address, QAS will walk the student through a process to enter a deliverable address. The student is able to move through the process even if the QAS does not recognize the address. If the student selects The information is CORRECT. The student then moves to the Billing Address Question form. If the student selects The information is NOT CORRECT the student goes back to the Update Home Address form. 4

Billing Address Question If the student selects YES, use my home address the student moves to the Campus Mail Address Review form (only if they have a campus mail address) or the student goes to the Cell Phone Question form. If the student selects NO, use a different address, the student moves to the Update Student Bill Address form. Update Student Bill Address The student enters billing address information and clicks Submit. This updates the BI address type in Banner. The student then moves to the Campus Mail Address Review form if applicable or the Cell Phone Question form. 5

Campus Mail Review Page The Campus Mail address displays if the student has one. If the student believes there is an error with this address, he/she is instructed to contact the UST Post Office. The student selects Click here to Continue and moves to the Cell Phone Question form. Cell Phone Question The student s cell phone number is displayed (if it is in Banner) and asked if they would like to participate in the Emergency Alert Notification Program. If the student selects YES, I would, the student moves to the Emergency Alert Notification Verification form. If the student selects NO, I would not, the student moves to the Emergency Contact Review form. If the student selects NO, but please change my listed phone number the student moves to the update cell phone form. Emergency Alert Notification Verification Page 6

The student is prompted to enter a cell phone number and the cell phone carrier. The default option on the page for ENS is yes, the student can change this to if they desire and opt out of the ENS. The student then selects Submit Form and Continue and moves to the Emergency Contact Review form. Note: The student can also delete the current cell phone listed in banner and choose not to provide a cell phone number on this form. Update Cell Phone Page The student enters a cell phone number then selects Submit and moves on to the Emergency Contact Review Form. Emergency Contact Review The student reviews the Emergency contact information currently in Banner on this page. A student is not required to provide an emergency contact and can leave this section blank if they choose. If the student selects The information is CORRECT, the student either moves to another Emergency Contact Review form (if they have more than one emergency contact in Banner) or moves to the Additional Emergency Contact form. If the student selects The information is NOT CORRECT, the student moves to the Update Emergency Contact form. 7

Update Emergency Contact The student has the option to Remove Contact, or update information as appropriate. When the student selects Submit Changes, the student either moves to another Emergency Contact Review form (if they have more than one emergency contact in Banner) or moves to the Additional Emergency Contacts form. Additional Emergency Contact Form If the student selects YES, I would like to add another emergency contact the student can add another contact. When they are done adding contacts the student moves to the Address Verification Information Page. If the student selects No, I am done the student moves to the Address Verification Information Page. 8

Address Verification Information Page If student selects I certify that the information I have provided is accurate and complete, they move to the Continue to Murphy Online Main Menu form. If student selects Click here to make further changes to addresses they are returned to the beginning of the process. 9
