Assessing Needs Before we start working with you, one of our assessment team will contact you to complete an assessment of your needs. Our fully trained team will put support in place for you to learn or relearn the skills necessary for daily living. We will review your progress regularly with you and if you need further support in the longer term, we will help you to access the appropriate services. As a user of the service, you will have a copy of your own personal support plan, which sets out how the service will be delivered to ensure your desired outcomes are met and your lifestyle is sustained. A risk assessment will also be carried out at our first visit. What are the circumstances that may result in services stopping? The service may be withdrawn where there is a serious risk of harm to you as the user of the service and/or reablement workers. The potential harm may result from environmental factors, contamination, infection or service user/third party behaviour.
